1Colorectal peritoneal metastases:Optimal management review显示文摘The peritoneum is a common site of dissemination for colorrectal cancer, with a poorer prognosis than other sites of metastases. In the last two decades, it has been considered as a locoregional disease progression and treated as such with curative intention treatments. Cytoreductive surgery(CRS) and hyperthermic intraperitoneal chemotherapy(HIPEC) is the actual reference treatment for these patients as better survival results have been reached as compared to systemic chemotherapy alone, but its therapeutic efficacy is still under debate. Actual guidelines recommend that the management of colorectal cancer with peritoneal metastases should be led by a multidisciplinary team carried out in experienced centers and consider CRS + HIPEC for selected patients. Accumulative evidence in the last three years suggests that this is a curative treatment that may improve patients disease-free survival, decrease the risk of recurrence, and does not increase the risk of treatment-related mortality. 2Peritoneal metastases of colorectal origin treated by cytoreduction and HIPEC: An overview显示文摘Colorectal peritoneal carcinomatosis was considered a terminal condition with a merely palliative treatment that included only supportive care, palliative surgery and the best systemic chemotherapy. Since the birth of a new approach, cytoreductive surgery with peritonectomy procedures together with hyperthermic intraperitoneal chemotherapy and/or early postoperative intraperitoneal chemotherapy to treat peritoneal carcinomatosis, many research groups contributed with promising results using this procedure being up to date this strategy the only one that has shown curative benefits on colorectal peritoneal carcinomatosis achieving reported overall survival rates up to 64 mo and fiveyear survival rates up to 51%. 3In vitro effect of amoxicillin and clarithromycin on the 3' region of cagA gene in Helicobacter pylori isolates显示文摘AIM:To evaluate the in vitro effect of amoxicillin and clarithromycin on the cag pathogenicity island(cag PAI).METHODS:One hundred and forty-nine clinical isolates of Helicobacter pylori(H.pylori)cultured from gastric biopsies from 206 Colombian patients with dyspeptic symptoms from a high-risk area for gastric cancer were included as study material.Antimicrobial susceptibility was determined by the agar dilution method.Resistant isolates at baseline and in amoxicillin and clarithromycin serial dilutions were subjected to genotyping(cagA,vacA alleles s and m),Glu-Pro-Ile-Tyr-Ala(EPIYA)polymerase chain reaction and random amplified polymorphic DNA(RAPD).Images of the RAPD amplicons were analyzed by Gel-Pro Analyzer 4.5program.Cluster analyses was done using SPSS 15.0statistical package,where each of the fingerprint bands were denoted as variables.Dendrograms were designed by following Ward’s clustering method and the estimation of distances between each pair of H.pylori isolates was calculated with the squared Euclidean distance.RESULTS:Resistance rates were 4%for amoxicillin and 2.7%for clarithromycin with 2%double resistances.Genotyping evidenced a high prevalence of the genotype cagA-positive/vacA s1m1.The 3’region of cagA gene was successfully amplified in 92.3%(12/13)of the baseline resistant isolates and in 60%(36/60)of the resistant isolates growing in antibiotic dilutions.Upon observing the distribution of the number of EPIYA repetitions in each dilution with respect to baseline isolates,it was found that in 61.5%(8/13)of the baseline isolates,a change in the number of EPIYA repetitions lowered antibiotic pressure.The gain and loss of EPIYA motifs resulted in a diversity of H.pylori subclones after bacterial adjustment to changing conditions product of antibiotic pressure.RAPD PCR evidenced the close clonal relationship between baseline isolates and isolates growing in antibiotic dilutions.CONCLUSION:Antibiotic pressure does not induce loss of the cag pathogenicity island,but it can leadin most cases-to genetic rearrangements within the3’region cagA of the founding bacteria that can affect the level of tyrosine phosphorylation impacting on its cellular effects and lead to divergence of cagA-positive subclones.Javier Andrés Bustamante-Rengifo Andrés Januer Matta Alvaro Pazos Luis Eduardo Bravo 2013World Journal of Gastroenterology2013,19,36:5
4Punctual mutations in 23S rRNA gene of clarithromycinresistant Helicobacter pylori in Colombian populations显示文摘AIM To characterize punctual mutations in 23S rRNA gene of clarithromycin-resistant Helicobacter pylori(H. pylori) and determine their association with therapeutic failure.METHODS PCR products of 23S rRNA gene V domain of 74 H. pylori isolates; 34 resistant to clarithromycin(29 from a low-risk gastric cancer(GC) population: TumacoColombia, and 5 from a high-risk population: TuquerresColombia) and 40 from a susceptible population(28 from Tumaco and 12 from Túquerres) were sequenced using capillary electrophoresis. The concordance between mutations of V domain 23S rRNA gene of H. pylori and therapeutic failure was determined using the Kappa coefficient and Mc Nemar's test was performed to determine the relationship between H. pylori mutationsand clarithromycin resistance.RESULTS23S rRNA gene from H. pylori was amplified in 56/74 isolates, of which 25 were resistant to clarithromycin(20 from Tumaco and 5 from Túquerres, respectively). In 17 resistant isolates(13 from Tumaco and 4 from Túquerres) the following mutations were found: A1593 T1, A1653 G2, C1770 T, C1954 T1, and G1827 C in isolates from Tumaco, and A2144 G from Túquerres. The mutations T2183 C, A2144 G and C2196 T in H. pylori isolates resistant to clarithromycin from Colombia are reported for the first time. No association between the H. pylori mutations and in vitro clarithromycin resistance was found. However, therapeutic failure of eradication treatment was associated with mutations of 23S rRNA gene in clarithromycin-resistant H. pylori(κ = 0.71).CONCLUSION The therapeutic failure of eradication treatment in the two populations from Colombia was associated with mutations of the 23S rRNA gene in clarithromycinresistant H. pylori.Andrés Jenuer Matta Diana Carolina Zambrano Alvaro Jairo Pazos 2018World Journal of Gastroenterology2018,24,14:5
5Identification of ta-siRNAs and Cis-nat-siRNAs in Cassava and Their Roles in Response to Cassava Bacterial Blight显示文摘Trans-acting small interfering RNAs (ta-siRNAs) and natural cis-antisense siRNAs (cis-nat-siRNAs) are recently discovered small RNAs (sRNAs) involved in post-transcriptional gene silencing. ta-siRNAs are transcribed from genomic loci and require processing by microRNAs (miRNAs). cis-nat-siRNAs are derived from antisense RNAs produced by the simultaneous transcription of overlapping antisense genes. Their roles in many plant processes, including pathogen response, are mostly unknown. In this work, we employed a bioinformatic approach to identify ta-siRNAs and cis-nat-siRNAs in cassava from two sRNA libraries, one constructed from healthy cassava plants and one from plants inoculated with the bacterium Xanthomonas axonopodis pv.manihotis (Xam). A total of 54 possible ta-siRNA loci were identified in cassava, including a homolog of TAS3, the best studied plant ta-siRNA. Fifteen of these loci were induced, while 39 were repressed in response to Xam infection. In addition, 15 possible cis-natural antisense transcript (cis-NAT) loci producing siRNAs were identified from overlapping antisense regions in the genome,and were found to be differentially expressed upon Xam infection. Roles of sRNAs were predicted by sequence complementarity and our results showed that many sRNAs identified in this work might be directed against various transcription factors. 6T_3-induced liver AMP-activated protein kinase signaling:Redox dependency and upregulation of downstream targets显示文摘AIM:To investigate the redox dependency and promotion of downstream targets in thyroid hormone(T3)-induced AMP-activated protein kinase(AMPK)signaling as cellular energy sensor to limit metabolic stresses in the liver.METHODS:Fed male Sprague-Dawley rats were given a single ip dose of 0.1 mg T3/kg or T3 vehicle(Na OH0.1 N;controls)and studied at 8 or 24 h after treatment.Separate groups of animals received 500 mg N-acetylcysteine(NAC)/kg or saline ip 30 min prior T3.Measurements included plasma and liver 8-isoprostane and serumβ-hydroxybutyrate levels(ELISA),hepaticlevels of m RNAs(q PCR),proteins(Western blot),and phosphorylated AMPK(ELISA).RESULTS:T3 upregulates AMPK signaling,including the upstream kinases Ca2+-calmodulin-dependent protein kinase kinase-βand transforming growth factor-β-activated kinase-1,with T3-induced reactive oxygen species having a causal role due to its suppression by pretreatment with the antioxidant NAC.Accordingly,AMPK targets acetyl-Co A carboxylase and cyclic AMP response element binding protein are phosphorylated,with the concomitant carnitine palmitoyltransferase-1α(CPT-1α)activation and higher expression of peroxisome proliferator-activated receptor-γco-activator-1αand that of the fatty acid oxidation(FAO)-related enzymes CPT-1α,acyl-Co A oxidase 1,and acylCo A thioesterase 2.Under these conditions,T3 induced a significant increase in the serum levels ofβ-hydroxybutyrate,a surrogate marker for hepatic FAO.CONCLUSION:T3 administration activates liver AMPK signaling in a redox-dependent manner,leading to FAO enhancement as evidenced by the consequent ketogenic response,which may constitute a key molecular mechanism regulating energy dynamics to support T3preconditioning against ischemia-reperfusion injury.Luis A Videla Virginia Fernández Pamela Cornejo Romina Vargas Paula Morales Juan Ceballo Alvaro Fischer Nicolás Escudero Oscar Escobar 2014World Journal of Gastroenterology2014,20,46:3
7Approach to early-onset colorectal cancer:Clinicopathological,familial,molecular and immunohistochemical characteristics显示文摘AIM:To characterize clinicopathological and familial features of early-onset colorectal cancer(CRC) and compare features of tumors with and without microsatellite instability(MSI).METHODS:Forty-five patients with CRC aged 45 or younger were included in the study.Clinical information,a three-generation family history,and tumor samples were obtained.MSI status was analyzed and mismatch repair genes were examined in the MSI families.Tumors were included in a tissue microarray and an immunohistochemical study was carried out with a panel of selected antibodies.RESULTS:Early onset CRC is characterized by advanced stage at diagnosis,right colon location,low-grade of differentiation,mucin production,and presence of polyps.Hereditary forms represent at least 21% of cases.Eighty-one percent of patients who died during followup showed a lack of expression of cyclin E,which could be a marker of poor prognosis.β-catenin expression was normal in a high percentage of tumors.CONCLUSION:Early-onset CRC has an important familial component,with a high proportion of tumors showing microsatellite stable.Cyclin E might be a poor prognosis factor.Jose Perea Edurne Alvaro Yolanda Rodríguez Cristina Gravalos Eva Sánchez-Tomé Barbara Rivera Francisco Colina Pablo Carbonell Rogelio González-Sarmiento Manuel Hidalgo Miguel Urioste 2010World Journal of Gastroenterology2010,16,29:3
8Conservative management of perforated duodenal diverticulum: A case report and review of the literature显示文摘Duodenal diverticula are a relatively common condition. They are asymptomatic, unless they become complicated, with perforation being the rarest but most severe complication. Surgical treatment is the most frequently performed approach. We report the case of a patient with a perforated duodenal diverticulum, which was diagnosed early and treated conservatively with antibiotics and percutaneous drainage of secondary retroperitoneal abscesses. 9Genomic variability of Helicobacter pylori isolates of gastric regions from two Colombian populations显示文摘AIM To compare the genomic variability and the multiple colonization of Helicobacter pylori(H. pylori) in patients with chronic gastritis from two Colombian populations with contrast in the risk of developing gastric cancer(GC): Túquerres-Nari?o(High risk) and Tumaco-Nari?o(Low risk).METHODS Four hundred and nine patients from both genders with dyspeptic symptoms were studied. Seventy-two patients were included in whom H. pylori was isolated from three anatomic regions of the gastric mucosa,(31/206) of the high risk population of GC(Túquerres) and(41/203) of the low risk population of GC(Tumaco). The isolates were genotyped by PCR-RAPD. Genetic diversity between the isolates was evaluated by conglomerates analysis and multiple correspondence analyses. RESULTS The proportion of virulent genotypes of H. pylori was 99% in Túquerres and 94% in Tumaco. The coefficient of similarity of Nei-Li showed greater genetic diversityamong isolates of Túquerres(0.13) than those of Tumaco(0.07). After adjusting by age, gender and type of gastritis, the multiple colonization was 1.7 times more frequent in Túquerres than in Tumaco(P = 0.05).CONCLUSION In Túquerres, high risk of GC there was a greater probability of multiple colonization by H. pylori. 19Highly graphitized carbon nanosheets with embedded Ni nanocrystals as anode for Li-ion batteries显示文摘A C/Ni composite was prepared via thermal decomposition of a nickel oleate complex at 700℃,yielding disperse Ni nanocrystals with an average size of 20 nm,encapsulated by carbon nanosheets as deduced from transmission electron microscopy(TEM)images and confirmed from X-ray photoelectron spectroscopy(XPS).Furthermore,the X-ray diffraction pattern revealed a good ordering of the carbon layers,forced by the Ni encapsulation to adopt a bending structure.Considering the close interaction between the graphitized framework and the metallic nanoparticles we have studied the properties of the composite as an anode for Li-ion batteries.Compared with other nanostructured synthetic carbons,this carbon composite has a low voltage hysteresis and a modest irreversible capacity value,properties that play a significant role in its behaviour as electrodes in full cell configuration.At moderate rate values,0.25 C,the electrode delivers an average capacity value around 723 mAh·g^−1 on cycling,among the highest values so far reported for this carbon type.At higher rate values,1 C,the average capacity values delivered by the cell on cycling decrease,around 205 mAh·g^−1,but it maintains good capacity retention,a coulombic efficiency close to 100%after the first cycles and recovery of the capacity values when the rate is restored from 3 to 0.1 C.Francisco Javier Soler-Piña Celia Hernández-Rentero Alvaro Caballero Julián Morales Enrique Rodríguez-Castellón Jesús Canales-Vázquez 2020Nano Research2020,13,1:1
