1Identification of a novel coronavirus causing severe pneumonia in human:a descriptive study显示文摘Background:Human infections with zoonotic coronaviruses(CoVs),including severe acute respiratory syndrome(SARS)-CoV and Middle East respiratory syndrome(MERS)-CoV,have raised great public health concern globally.Here,we report a novel batorigin CoV causing severe and fatal pneumonia in humans.Methods:We collected clinical data and bronchoalveolar lavage(BAL)specimens from five patients with severe pneumonia from Wuhan Jinyintan Hospital,Hubei province,China.Nucleic acids of the BAL were extracted and subjected to next-generation sequencing.Virus isolation was carried out,and maximum-likelihood phylogenetic trees were constructed.Results:Five patients hospitalized from December 18 to December 29,2019 presented with fever,cough,and dyspnea accompanied by complications of acute respiratory distress syndrome.Chest radiography revealed diffuse opacities and consolidation.One of these patients died.Sequence results revealed the presence of a previously unknownβ-CoV strain in all five patients,with 99.8%to 99.9%nucleotide identities among the isolates.These isolates showed 79.0%nucleotide identity with the sequence of SARS-CoV(GenBank NC_004718)and 51.8%identity with the sequence of MERS-CoV(GenBank NC_019843).The virus is phylogenetically closest to a bat SARS-like CoV(SL-ZC45,GenBank MG772933)with 87.6%to 87.7%nucleotide identity,but is in a separate clade.Moreover,these viruses have a single intact open reading frame gene 8,as a further indicator of bat-origin CoVs.However,the amino acid sequence of the tentative receptor-binding domain resembles that of SARS-CoV,indicating that these viruses might use the same receptor.Conclusion:A novel bat-borne CoV was identified that is associated with severe and fatal respiratory disease in humans.Li-Li Ren Ye-Ming Wang Zhi-Qiang Wu Zi-Chun Xiang Li Guo Teng Xu Yong-Zhong Jiang Yan Xiong Yong-Jun Li Xing-Wang Li Hui Li Guo-Hui Fan Xiao-Ying Gu Yan Xiao Hong Gao Jiu-Yang Xu Fan Yang Xin-Ming Wang Chao Wu Lan Chen Yi-Wei Liu Bo Liu Jian Yang Xiao-Rui Wang Jie Dong Li Li Chao-Lin Huang Jian-Ping Zhao Yi Hu Zhen-Shun Cheng Un-Lin Liu Zhao-Hui Qian Chuan Qin Qi Jin Bin Cao Jian-Wei Wang 2020Chinese Medical Journal2020,,9:99
2China Stroke Statistics 2019:A Report From the National Center for Healthcare Quality Management in Neurological Diseases,China National Clinical Research Center for Neurological Diseases,the Chinese Stroke Association,National Center for Chronic and Non-communicable Disease Control and Prevention,Chinese Center for Disease Control and Prevention and Institute for Global Neuroscience and Stroke Collaborations显示文摘China faces the greatest challenge from stroke in the world.The death rate for cerebrovascular diseases in China was 149.49 per 100000,accounting for 1.57 million deaths in 2018.It ranked third among the leading causes of death behind malignant tumours and heart disease.The age-standardised prevalence and incidence of stroke in 2013 were 1114.8 per 100000 population and 246.8 per 100000 person-years,respectively.According to the Global Burden of Disease Study 2017,the years of life lost(YLLs)per 100000 population for stroke increased by 14.6%;YLLs due to stroke rose from third highest among all causes in 1990 to the highest in 2017.The absolute numbers and rates per 100000 population for all-age disability-adjusted life years(DALYs)for stroke increased substantially between 1990 and 2017,and stroke was the leading cause of all-age DALYs in 2017.The main contributors to cerebrovascular diseases include behavioural risk factors(smoking and alcohol use)and pre-existing conditions(hypertension,diabetes mellitus,dyslipidaemia and atrial fibrillation(AF)).The most prevalent risk factors among stroke survivors were hypertension(63.0%-84.2%)and smoking(31.7%-47.6%).The least prevalent was AF(2.7%-7.4%).The prevalences for major risk factors for stroke are high and most have increased over time.Based on the latest national epidemiological data,26.6%of adults aged≥15 years(307.6 million adults)smoked tobacco products.For those aged≥18 years,age-adjusted prevalence of hypertension was 25.2%;adjusted prevalence of hypercholesterolaemia was 5.8%;and the standardised prevalence of diabetes was 10.9%.For those aged≥40 years,the standardised prevalence of AF was 2.31%.Data from the Hospital Quality Monitoring System showed that 3010204 inpatients with stroke were admitted to 1853 tertiary care hospitals during 2018.Of those,2466785(81.9%)were ischaemic strokes(ISs);447609(14.9%)were intracerebral haemorrhages(ICHs);and 95810(3.2%)were subarachnoid haemorrhages(SAHs).The average age of patients admitted was 66 years old,and nearly 60%were male.A total of 1555(0.1%),2774(0.6%)and 1347(1.4%)paediatric strokes(age<18 years)were identified among IS,ICH and SAH,respectively.Over one-third(1063892(35.3%))of the patients were covered by urban resident basic medical insurance,followed by urban employee basic medical insurance(699513(23.2%))and new rural cooperative medical schema(489361(16.3%)).The leading risk factor was hypertension(67.4%for IS,77.2%for ICH and 49.1%for SAH),and the leading comorbidity was pneumonia or pulmonary infection(10.1%for IS,31.4%for ICH and 25.2%for SAH).In-hospital death/discharge against medical advice rate was 8.3%for stroke inpatients,ranging from 5.8%for IS to 19.5%for ICH.The median and IQR of length of stay was 10.0(7.0-14.0)days,ranging from 10.0(7.0-13.0)in IS to 14.0(8.0-22.0)in SAH.Data from the Chinese Stroke Center Alliance demonstrated that the composite scores of guideline-recommended key performance indicators for patients with IS,ICH and SAH were 0.77±0.21,0.72±0.28 and 0.59±0.32,respectively.Yong-Jun Wang Zi-Xiao Li Hong-Qiu Gu Yi Zhai Yong Jiang Xing-Quan Zhao Yi-Long Wang Xin Yang Chun-Juan Wang Xia Meng Hao Li Li-Ping Liu Jing Jing Jing Wu An-Ding Xu Qiang Dong David Wang Ji-Zong Zhao On behalf of China Stroke Statistics 2019 Writing Committee 2020Stroke & Vascular Neurology2020,5,3:181
3Chemical composition of PM_(2.5) during winter in Tianjin,China显示文摘PM2.5 samples for 24 h were collected during winter in Tianjin,China. The ambient mass concentration and chemical composition of the PM2.5 were determined. Ionic species were analyzed by ion chromatog-raphy,while carbonaceous species were determined with the IMPROVE thermal optical reectance(TOR) method,and inorganic elements were measured by inductively coupled plasma-atomic emission spec-trometer. The daily PM2.5 mass concentrations ranged from 48.2 to 319.2 g/m3 with an arithmetic average of 144.6 g/m3. The elevated PM2.5 in winter was mostly attributed to combustion sources such as vehicle exhaust,heating,cooking and industrial emissions,low wind speeds and high relative humidity(RH) ,which were favorable for pollutant accumulation and formation of secondary pollutants. By chem-ical mass balance,it was estimated that about 89.1% of the PM2.5 mass concentrations were explained by carbonaceous species,secondary particles,crustal matters,sea salt and trace elements. Organic material was the largest contributor,accounting for about 32.7% of the total PM2.5 mass concentrations. 5Nitrate leaching of winter wheat grown in lysimeters as affected by fertilizers and irrigation on the North China Plain显示文摘Proper application of nitrogen(N) fertilizers and irrigation management are important production practices that can reduce nitrate leaching into groundwater and improve the N use efficiency(NUE). A lysimeter/rain shelter facility was used to study effects of the rate of N fertilization, type of N fertilizer, and irrigation level on key aspects of winter wheat production over three growing seasons(response variables were nitrate transport, N leaching, and NUE). Results indicated that nitrate concentration in the soil profile and N leaching increased with the rate of N fertilization. At the end of the third season, nitrate concentration in the top 0–75 cm layer of soil was higher with manure treatment while urea treatments resulted in higher concentrations in the 100–200 cm layer. With normal irrigation, 3.4 to 15.3% of N from applied fertilizer was leached from the soil, yet no leaching occurred under a stress irrigation treatment. The manure treatment experienced less N leaching than the urea treatment in all cases except for the 180 kg N ha-1 rate in 2011–2012(season 3). In terms of grain yield(GY), dry matter(DM) or NUE parameters, values for the manure treatment were lower than for the urea treatment in 2009–2010(season 1), yet were otherwise higher for urea treatment in season 3. GY and crop nitrogen uptake(NU) were elevated when the rate of N fertilizer increased, while the NUE decreased; GY, DM, and NU increased with the amount of irrigation. 7Implementation of Envelope Analysis on a Wireless Condition Monitoring System for Bearing Fault Diagnosis显示文摘Envelope analysis is an effective method for characterizing impulsive vibrations in wired condition monitoring(CM)systems. This paper depicts the implementation of envelope analysis on a wireless sensor node for obtaining a more convenient and reliable CM system. To maintain CM performances under the constraints of resources available in the cost effective Zigbee based wireless sensor network(WSN), a low cost cortex-M4 F microcontroller is employed as the core processor to implement the envelope analysis algorithm on the sensor node. The on-chip 12 bit analog-to-digital converter(ADC) working at 10 k Hz sampling rate is adopted to acquire vibration signals measured by a wide frequency band piezoelectric accelerometer. The data processing flow inside the processor is optimized to satisfy the large memory usage in implementing fast Fourier transform(FFT) and Hilbert transform(HT). Thus, the envelope spectrum can be computed from a data frame of 2048 points to achieve a frequency resolution acceptable for identifying the characteristic frequencies of different bearing faults. 10Relationship between cytokines gene polymorphism and susceptibility to hepatitis B virus intrauterine infection显示文摘Background The influences of genomic background are confirmed in more diseases. Immunologic tolerance after intrauterine infection of hepatitis B virus is considered to occur in T cells. Cytokines work effectively in eliminating virus by immune system after hepatitis B virus infection. To explore the relationship between cytokines (tumor necrosis factor-α, interferon-γ, interleukin-4 and interleukin-10), which expressed abnormal quantity in the peripheral blood to intrauterine hepatitis B virus infectious children, gene single nucleotide polymorphism (SNP) and susceptibility to hepatitis B virus intrauterine infection. Methods This is a cross sectional study of molecular clinical epidemiology. The subjects in this study were selected from outpatients of hepatitis B vaccine follow-up special clinics of our hospital in the period. According to intrant criteria, the high risk children of hepatitis B virus (HBV) intrauterine infection were divided into immune failure group (group Ⅰ); and immune effective group (group Ⅱ) and non high risk children belonged to the control group. Four gene SNP sites of TNF-α -238, IFN-γ +874, IL-4 -590 and IL-10 -1082 were determined by real-time quantitative fluorescent polymerase chain reaction (PCR). Results The significant differences of TNF-α -238 A allele frequency were found between group Ⅰ and groupⅡ (χ2=6.797,P<0.05) and between groupⅠ and the control group (χ2=9.513,P<0.05). No evident differences of TNF-α -238 A were found between group Ⅱ and control group (χ2=0.047,P>0.05); the significant differences of IFN-γ +874 A allele frequency were found between groupⅠ and groupⅡ(χ2=7.238,P<0.05), and between groupⅠ and the control group (χ2=5.199,P<0.05). No evident differences were found between groupⅡ and the control group (χ2=0.602,P>0.05); the significant differences of IL-4 -590 C/T allele frequency were not found between groupⅠand group Ⅱ(χ2=0.632,P>0.05), also groupⅠ and the control group (χ2=0.584,P>0.05), and the groupⅡ and the control group (χ2=0.004,P>0.05)respectively; The significant differences of IL-10 -1082 G allele frequency were found between groupⅡ and groupⅠ (χ2=10.359,P<0.001), and between groupⅡ and the controls (χ2=35.418,P<0.001), but the significant differences were not found between groupⅠand the control group (χ2=1.759,P>0.05). Conclusions This study suggested the possibility that the TNF-α -238 A allele and IFN-γ +874 A allele were associated with HBV intrauterine infection. 11Secukinumab demonstrates high efficacy and a favorable safety profile over 52 weeks in Chinese patients with moderate to severe plaque psoriasis显示文摘Background:Psoriasis is a chronic inflammatory skin disease,affecting about 0.6%of the Chinese population.Many patients are not well controlled by conventional treatments,thus there is need for new treatment regimens.In this study,we assessed the efficacy and safety of secukinumab in Chinese patients with moderate to severe plaque psoriasis.Methods:This study was a 52-week,multicentre,randomized,double-blind,placebo-controlled,parallel-group,Phase 3 trial.A sub-population of study participants(≥18 years)of Chinese ethnicity were randomized to receive subcutaneous injections of 300 or 150 mg secukinumab,or placebo.The co-primary endpoints were psoriasis area severity index(PASI)75 and Investigator’s Global Assessment(IGA)0/1 at Week 12.Results:A total of 441 Chinese patients were enrolled in this study.Co-primary outcomes were achieved;300 and 150 mg secukinumab were superior to placebo as shown in the proportion of patients that achieved PASI 75(97.7%and 87.2%vs.3.7%,respectively;P<0.001),and IGA 0/1(82.3%and 69.7%vs.2.7%;P<0.001)at Week 12.Treatment efficacy was maintained until Week 52.There was no increase in overall adverse events with secukinumab relative to placebo throughout the 52-week period.Conclusion:Secukinumab is highly effective and well tolerated in Chinese patients with moderate to severe plaque psoriasis.Lin Cai Jian-Zhong Zhang Xu Yao Jun Gu Quan-Zhong Liu Min Zheng Shi-Fa Zhang Jin-Hua Xu Cheng-Xin Li Hao Cheng Qing Guo Wei-Li Pan Shen-Qiu Li Ruo-Yu Li Zai-Pei Guo Zhi-Qi Song Shan-Shan Li Xiu-Qin Dong Linda Wang Rong Fu Pascaline Regnault Pascal Charef Ratal Mazur Manmath Patekar 2020Chinese Medical Journal2020,,22:21

13Precise nanomedicine for intelligent therapy of cancer显示文摘Precise nanomedicine has been extensively explored for efficient cancer imaging and targeted cancer therapy, as evidenced by a few breakthroughs in their preclinical and clinical explorations. Here, we demonstrate the recent advances of intelligent cancer nanomedicine, and discuss the comprehensive understanding of their structure-function relationship for smart and efficient cancer nanomedicine including various imaging and therapeutic applications, as well as nanotoxicity. In particular, a few emerging strategies that have advanced cancer nanomedicine are also highlighted as the emerging focus such as tumor imprisonment, supramolecular chemotherapy, and DNA nanorobot. The challenge and outlook of some scientific and engineering issues are also discussed in future development. 14Baicalin attenuates high fat diet-induced insulin resistance and ectopic fat storage in skeletal muscle,through modulating the protein kinase B/Glycogen synthase kinase 3 beta pathway显示文摘Insulin resistance is the pathophysiological basis of many diseases.Overcoming early insulin resistance highly significant in prevention diabetes,non-alcoholic fatty liver,and atherosclerosis.The present study aimed at evaluating the therapeutic effects of baicalin on insulin resistance and skeletal muscle ectopic fat storage in high fat diet-induced mice,and exploring the potential molecular mechanisms.Insulin resistance in mice was induced with a high fat diet for 16 weeks.Animals were then treated with three different doses of baicalin(100,200,and 400 mg·kg^(-1)·d^(-1)for 14 weeks.Fasting blood glucose,fasting serum insulin,glucose tolerance test(GTT),insulin tolerance test(ITT),and skeletal muscle lipid deposition were measured.Additionally,the AMP-activated protein kinase/acetyl-CoA carboxylase and protein kinase B/Glycogen synthase kinase 3 beta pathways in skeletal muscle were further evaluated.Baicalin significantly reduced the levels of fasting blood glucose and fasting serum insulin and attenuated high fat diet induced glucose tolerance and insulin tolerance.Moreover,insulin resistance was significantly reversed.Pathological analysis revealed baicalin dose-dependently decreased the degree of the ectopic fat storage in skeletal muscle.The properties of baicalin were mediated,at least in part,by inhibition of the AMPK/ACC pathway,a key regulator of de novo lipogenesis and activation of the Akt/GSK-3β pathway,a key regulator of Glycogen synthesis.These data suggest that baicalin,at dose up to 400 mg·kg^(-1)·d^(-1),is safe and able to attenuate insulin resistance and skeletal muscle ectopic fat storage,through modulating the skeletal muscle AMPK/ACC pathway and Akt/GSK-3β pathway.XI You-Li LI Hong-Xia CHEN Chen LIU Ya-Qun LV Hong-Mei DONG Shi-Qi LUO Er-Fei GU Ming-Bo LIU Hua 2016Chinese Journal of Natural Medicines2016,14,1:19
15Postponed and reduced basal nitrogen application improves nitrogen use efficiency and plant growth of winter wheat显示文摘Excessive nitrogen(N) fertilization with a high basal N ratio in wheat can result in lower N use efficiency(NUE) and has led to environmental problems in the Yangtze River Basin, China. However, wheat requires less N fertilizer at seedling growth stage, and its basal N fertilizer utilization efficiency is relatively low; therefore, reducing the N application rate at the seedling stage and postponing the N fertilization period may be effective for reducing N application and increasing wheat yield and NUE. A 4-year field experiment was conducted with two cultivars under four N rates(240 kg N ha–1(N240), 180 kg N ha–1(N180), 150 kg N ha–1(N150), and 0 kg N ha–1(N0)) and three basal N application stages(seeding(L0), fourleaf stage(L4), and six-leaf stage(L6)) to investigate the effects of reducing the basal N application rate and postponing the basal N fertilization period on grain yield, NUE, and N balance in a soil-wheat system. There was no significant difference in grain yield between the N180 L4 and N240 L0(control) treatments, and the maximum N recovery efficiency and N agronomy efficiency were observed in the N180 L4 treatment. Grain yield and NUE were the highest in the L4 treatment. The leaf area index, flag leaf photosynthesis rate, flag leaf nitrate reductase and glutamine synthase activities, dry matter accumulation, and N uptake post-jointing under N180 L4 did not differ significantly from those under N240 L0. Reduced N application decreased the inorganic N content in the 0–60-cm soil layer, and the inorganic N content of the L6 treatment was higher than those of the L0 and L4 treatments at the same N level. Surplus N was low under the reduced N rates and delayed basal N application treatments. 16Incidence trends and age distribution of colorectal cancer by subsite in Guangzhou,2000-2011显示文摘Introduction:Colorectal cancer(CRC) is the third most common cancer in China.The incidence of CRC has been increasing in recent years.The aim of this study was to explore the incidence trends and the age distribution of CRC by subsite in Guangzhou between 2000 and 2011.Methods:A total of 22,432 incident cases of CRC between 2000 and 2011 from Guangzhou Cancer Registry were identified.Crude incidence and age-standardized rates(ASRs),using the Segi’s world standard population,were calculated for CRC and CRC subsites.The incidence trend was analyzed and the annual percentage change(APC) in incidence was calculated by using JoinPoint software.Results:The crude incidence increased significantly from 23.4/10~5 in 2000 to 37.4/10~5 in 2011 for males and from20.9/10~5 to 30.5/10~5 for females.The ASRs of CRC incidence stabilized during the period of 2000-2011 for both males and females.The ages at the onset of CRC for both males and females during 2010-2011 were significantly higher compared with those during 2000-2002(males:f = 1.95,P = 0.05;females:f = 6.03,P < 0.01).For males aged50-64 years,the CRC incidence increased by 8.50%annually(P = 0.04) during 2000-2004 and by 1.68%annually(P = 0.03) during 2005-2011.For females aged 65 years and older,the CRC incidence increased by 5.77%annually(P = 0.03) during 2000-2004.There were no significant changes for the CRC incidences in males aged 49 and younger and 65 years and older and females aged 64 years and younger during 2000-2004,or for those in all females as well as males aged 49 years and younger and 65 years and older during 2005-2011.The percentage of colon cancer in all CRCs increased significantly for both males and females between the periods of 2000-2002 and 2010-2011.The ASRs of descending colon and sigmoid colon cancer incidences increased significantly for females during 2005-2011(APC,5.51%and 1.08%,respectively,both P < 0.05).Conclusions:The crude incidence of CRC increased significantly between 2000 and 2011 because of the aging,whereas the ASRs kept stable.The percentage of colon cancer in all CRCs increased significantly.Further surveillance,research,and intervention are needed to identify the causes of these changes and to reduce the incidence and mortality of CRC.Qin Zhou Ke Li Guo-Zhen Lin Ji-Chuan Shen Hang Dong Yu-Ting Gu Hua-Zhang Liu 2015Chinese Journal of Cancer2015,34,8:17

18Overexpression of insulin-like growth factor-Ⅰ receptor as a pertinent biomarker for hepatocytes malignant transformation显示文摘AIM:To investigate the dynamic features of insulinlike growth factor-Ⅰreceptor(IGF-ⅠR)expression in rat hepatocarcinogenesis,and the relationship between IGF-ⅠR and hepatocytes malignant transformation at mRNA or protein level.METHODS:Hepatoma models were made by inducing with 2-fluorenylacetamide(2-FAA)on male SpragueDawley rats.Morphological changes of hepatocytes were observed by pathological Hematoxylin and eosin staining,the dynamic expressions of liver and serum IGF-ⅠR were quantitatively analyzed by an enzymelinked immunosorbent assay.The distribution of hepatic IGF-ⅠR was located by immunohistochemistry.The fragments of IGF-ⅠR gene were amplified by reverse transcription-polymerase chain reaction,and confirmed by sequencing.RESULTS:Rat hepatocytes after induced by 2-FAA were changed dynamically from granule-like degeneration,precancerous to hepatoma formation with the progressing increasing of hepatic mRNA or IGF-ⅠR expression.The incidences of liver IGF-ⅠR,IGF-ⅠR mRNA,specific IGF-ⅠR concentration(ng/mg wet liver),and serum IGF-ⅠR level(ng/mL)were 0.0%,0.0%,0.63±0.17,and 1.33±0.47 in the control;50.0%,61.1%,0.65±0.2,and 1.51±0.46 in the degeneration;88.9%,100%,0.66±0.14,and 1.92±0.29 in the precancerosis;and 100%,100%,0.96±0.09,and2.43±0.57 in the cancerous group,respectively.IGF-ⅠR expression in the cancerous group was significantly higher(P<0.01)than that in any of other groups at mRNA or protein level.The closely positive IGF-ⅠR relationship was found between livers and sera(r=0.91,t=14.222,P<0.01),respectively.CONCLUSION:IGF-ⅠR expression may participate in rat hepatocarcinogenesis and its abnormality should be an early marker for hepatocytes malignant transformation.Xiao-Di Yan Min Yao Li Wang Hai-Jian Zhang Mei-Juan Yan Xing Gu Yun Shi Jie Chen Zhi-Zhen Dong Deng-Fu Yao 2013World Journal of Gastroenterology2013,19,36:16
19Neolithic millet farmers contributed to the permanent settlement of the Tibetan Plateau by adopting barley agriculture显示文摘The permanent human settlement of the Tibetan Plateau(TP)has been suggested to have been facilitated by the introduction of barley agriculture^3.6 kilo-years ago(ka).However,how barley agriculture spread onto the TP remains unknown.Given that the lower altitudes in the northeastern TP were occupied by millet cultivators from 5.2 ka,who also adopted barley farming^4 ka,it is highly possible that it was millet farmers who brought barley agriculture onto the TP^3.6 ka.To test this hypothesis,we analyzed mitochondrial DNA(mt DNA)from 8277 Tibetans and 58514 individuals from surrounding populations,including 682 newly sequenced whole mitogenomes.Multiple lines of evidence,together with radiocarbon dating of cereal remains at different elevations,supports the scenario that two haplogroups(M9a1a1c1b1a and A11a1a),which are common in contemporary Tibetans(20.9%)and were probably even more common(40–50%)in early Tibetans prior to historical immigrations to the TP,represent the genetic legacy of the Neolithic millet farmers.Both haplogroups originated in northern China between 10.0–6.0 ka and differentiated in the ancestors of modern Tibetans^5.2–4.0 ka,matching the dispersal history of millet farming.By showing that substantial genetic components in contemporary Tibetans can trace their ancestry back to the Neolithic millet farmers,our study reveals that millet farmers adopted and brought barley agriculture to the TP^3.6–3.3 ka,and made an important contribution to the Tibetan gene pool.Yu-Chun Li Jiao-Yang Tian Feng-Wen Liu Bin-Yu Yang Kang-Shu-Yun Gu Zia Ur Rahman Li-Qin Yang Fa-Hu Chen Guang-Hui Dong Qing-Peng Kong 2019National Science Review2019,6,5:15
20Chinese Stroke Association guidelines for clinical management of cerebrovascular disorders:executive summary and 2019 update of clinical management of spontaneous subarachnoid haemorrhage显示文摘caused by ruptured cerebral aneurysm is a severe subtype of haemorrhagic stroke.Although the incidence of SAH is relatively low among all cerebrovascular diseases,the mortality is the highest.The critical management of SAH is challenging.We provide this evidence-based guideline to present current and comprehensive recommendations for the diagnosis and treatment of non-trauma SAH.Methods A formal literature search of MEDLINE(1 January 1990-30 June 2019)was performed.Data were synthesised with the use of evidence tables.Writing group members met by teleconference to discuss data-derived recommendations.The Chinese Stroke Association’s levels of evidence grading algorithm was used to grade each recommendation.The guideline draft was reviewed by Chinese Stroke Association’s Stroke Fellow Committees.It is intended that this guideline be fully updated every 3 years.results Evidence-based guidelines are presented for the care of patients presenting with non-trauma SAH.The focus of the guideline was subdivided into transfer and systems of care,diagnosis flowchart,aetiology and differentiation,prevention of rebleeding,surgical and endovascular repair of ruptured aneurysms,management of vasospasm and delayed cerebral ischaemia,management of hydrocephalus,management of seizures and management of medical complications.Conclusions The guideline offers a framework for SAH management.Early professional and aggressive care of SAH might help dramatically.Yi Dong Zhen-Ni Guo Qi Li Wei Ni Hongqiu Gu Yu-Xiang Gu Qiang Dong 2019Stroke & Vascular Neurology2019,4,4:15
