1Impact of environmental and dietary factors on the course of inflammatory bowel disease显示文摘Besides their possible effects on the development of inflammatory bowel disease(IBD),some environmental factors can modulate the clinical course of both ulcerative colitis(UC) and Crohn's disease(CD).This review is mainly devoted to describing the current knowledge of the impact of some of these factors on the outcome of IBD,with special emphasis on smoking and diet.Although the impact of smoking on the susceptibility to develop CD and UC is firmly established,its influence on the clinical course of both diseases is still debatable.In CD,active smoking is a risk factor for postoperative recurrence.Beyond this clinical setting,smoking cessation seems to be advantageous in those CD patients who were smokers at disease diagnosis,while smoking resumption may be of benefit in ex-smokers with resistant UC.The role of dietary habits on the development of IBD is far from being well established.Also,food intolerances are very frequent,but usually inconsistent among IBD patients,and therefore no general dietary recommendations can be made in these patients.In general,IBD patients should eat a diet as varied as possible.Regarding the possible therapeutic role of some dietary components in IBD,lessons should be drawn from the investigation of the primary therapeutic effect of enteral nutrition in CD.Low-fat diets seem to be particularly useful.Also,some lipid sources,such as olive oil,medium-chain triglycerides,and perhaps omega-3 fatty acids,might have a therapeutic effect.Fermentable fiber may have a role in preventing relapses in inactive UC.Eduard Cabré Eugeni Domènech 2012World Journal of Gastroenterology2012,18,29:15
2Management of Crohn’s disease in smokers:Is an alternative approach necessary?显示文摘Inflammatory bowel disease(IBD) is a chronic condition with a pathogenic background that involves both genetic and environmental factors.Although important progress has been made regarding the former in the last decade,scarce knowledge is available for the latter.In this sense,smoking remains the most important environmental factor in IBD.Active smoking increases the risk of developing Crohn's disease(CD).Moreover,CD patients who start or continue smoking after disease diagnosis are at risk for poorer outcomes such as higher therapeutic requirements and disease-related complications,as compared to those patients who quit smoking or who never smoked.However,the harmful effect of active smoking is not uniform in all patients or in all clinical scenarios.Interventions designed to facilitate smoking cessation may impact the course of the disease.In this article,the available evidence of the deleterious effects of smoking on CD is reviewed in detail,and alternative therapeutic approaches to CD in smokers are proposed.Pilar Nos Eugeni Domènech 2011World Journal of Gastroenterology2011,17,31:6
3Tea polyphenol-gut microbiota interactions:hints on improving the metabolic syndrome in a multi-element and multi-target manner显示文摘The metabolic syndrome(MS)has become one of the main problems in public health.Tea polyphenols(TPs),the main bioactive components of tea,has been claimed to have the potential to regulate metabolism and effectively prevent or mitigate the MS.However,many studies into the effects of TPs on MS have provided conflicting findings and the underlying mechanism has been elusive.The predominant TPs in unfermentedand and fermented tea are catechins and oxidized polyphenols(theaflavins and thearubigins),both of which have low bioavailability and reach the colon where most gut microbes inhabit.Gut microbiota has been demonstrated to be tightly associated with host metabolism.The interactions between TPs and gut microbiota will lead to the alterations of gut microbiota composition and the production of metabolites including short chain fatty acids,bile acids,amino acids and TPs derived metabolites,accordingly exerting their biological effects both locally and systemically.This review highlighted the contribution of metabolites and specific gut bacteria in the process of TPs intervention on the MS and further discuss how TPs impact the MS via gut microbiota from the viewpoint of gut organ/tissue axis.Hui Ma Yaozhong Hu Bowei Zhang Zeping Shao Eugeni Roura Shuo Wang 2022Food Science and Human Wellness2022,11,1:5
4Arthritis and the role of endogenous glucocorticoids显示文摘Rheumatoid arthritis and osteoarthritis,the most common forms of arthritis,are chronic,painful,and disabling conditions.Although both diseases differ in etiology,they manifest in progressive joint destruction characterized by pathological changes in the articular cartilage,bone,and synovium.While the potent anti-inflammatory properties of therapeutic(i.e.,exogenous)glucocorticoids have been heavily researched and are widely used in clinical practice,the role of endogenous glucocorticoids in arthritis susceptibility and disease progression remains poorly understood.Current evidence from mouse models suggests that local endogenous glucocorticoid signaling is upregulated by the pro-inflammatory microenvironment in rheumatoid arthritis and by aging-related mechanisms in osteoarthritis.Furthermore,these models indicate that endogenous glucocorticoid signaling in macrophages,mast cells,and chondrocytes has anti-inflammatory effects,while signaling in fibroblast-like synoviocytes,myocytes,osteoblasts,and osteocytes has pro-inflammatory actions in rheumatoid arthritis.Conversely,in osteoarthritis,endogenous glucocorticoid signaling in both osteoblasts and chondrocytes has destructive actions.Together these studies provide insights into the role of endogenous glucocorticoids in the pathogenesis of both inflammatory and degenerative joint disease.Eugenie Macfarlane Markus J.Seibel Hong Zhou 2020Bone Research2020,8,3:4
5Use of granulocyte/monocytapheresis in ulcerative colitis:A practical review from a European perspective显示文摘Half of the patients with ulcerative colitis require at least one course of systemic corticosteroids in their lifetime.Approximately 75%of these patients will also require immunosuppressive drugs(i.e.,thiopurines or biological agents)in the mid-term to avoid colectomy.Immunosuppressive drugs raise some concerns due to an increased risk of serious and opportunistic infections and cancer,particularly in elderly and co-morbid patients,underlining the unmet need for safer alternative therapies.Granulocyte/monocytapheresis(GMA),a CE-marked,non-pharmacological procedure for the treatment of ulcerative colitis(among other immune-mediated diseases),remains the only therapy targeting neutrophils,the hallmark of pathology in ulcerative colitis.GMA has proven its efficacy in different clinical scenarios and shows an excellent and unique safety profile.In spite of being a first line therapy in Japan,GMA use is still limited to a small number of centres and countries in Europe.In this article,we aim to give an overview from a European perspective of the mechanism of action,recent clinical data on efficacy and practical aspects for the use of GMA in ulcerative colitis.Eugeni Domènech Joan-Ramon Grífols Ayesha Akbar Axel U Dignass 2021World Journal of Gastroenterology2021,27,10:2
6Early infant male circumcision:Systematic review,risk-benefit analysis,and progress in policy显示文摘AIM To determine whether recent evidence-based United States polices on male circumcision(MC) apply to comparable Anglophone countries,Australia and New Zealand.METHODS Articles in 2005 through 2015 were retrieved from PubM ed using the keyword 'circumcision' together with 36 relevant subtopics.A further PubM ed search was performed for articles published in 2016.Searches of the EMBASE and Cochrane databases did not yield additional citable articles.Articles were assessed for quality and those rated 2+ and above according to the Scottish Intercollegiate Grading System were studied further.The most relevant andrepresentative of the topic were included.Bibliographies were examined to retrieve further key references.Randomized controlled trials,recent high quality systematic reviews or meta-analyses(level 1++ or 1+ evidence) were prioritized for inclusion.A risk-benefit analysis of articles rated for quality was performed.For efficiency and reliability,recent randomized controlled trials,metaanalyses,high quality systematic reviews and large welldesigned studies were used if available.Internet searches were conducted for other relevant information,including policies and Australian data on claims under Medicare for MC.RESULTS Evidence-based policy statements by the American Academy of Pediatrics(AAP) and the Centers for Disease Control and Prevention(CDC) support infant and later age male circumcision(MC) as a desirable public health measure.Our systematic review of relevant literature over the past decade yielded 140 journal articles that met our inclusion criteria.Together,these showed that early infant MC confers immediate and lifelong benefits by protecting against urinary tract infections having potential adverse long-term renal effects,phimosis that causes difficult and painful erections and 'ballooning' during urination,inflammatory skin conditions,inferior penile hygiene,candidiasis,various sexually transmissible infections in both sexes,genital ulcers,and penile,prostate and cervical cancer.Our risk-benefit analysis showed that benefits exceeded procedural risks,which are predominantly minor,by up to 200 to 1.We estimated that more than 1 in 2 uncircumcised males will experience an adverse foreskin-related medical condition over their lifetime.Wide-ranging evidence from surveys,physiological measurements,and the anatomical location of penile sensory receptors responsible for sexual sensation strongly and consistently suggested that MC has no detrimental effect on sexual function,sensitivity or pleasure.United States studies showed that early infant MC is cost saving.The evidence supporting early infant MC has further strengthened since the positive AAP and CDC reviews.CONCLUSION Affirmative MC policies are needed in Australia and New Zealand.Routine provision of accurate,unbiased education,and access in public hospitals,will maximize health and financial benefits.Brian J Morris Sean E Kennedy Alex D Wodak Adrian Mindel David Golovsky Leslie Schrieber Eugenie R Lumbers David J Handelsman John B Ziegler 2017World Journal of Clinical Pediatrics2017,6,1:2
7Osteoblastic glucocorticoid signaling exacerbates high-fat-diet-induced bone loss and obesity显示文摘Chronic high-fat diet(HFD)consumption not only promotes obesity and insulin resistance,but also causes bone loss through mechanisms that are not well understood.Here,we fed wild-type CD-1 mice either chow or a HFD(43%of energy from fat)for 18 weeks;HFD-fed mice exhibited decreased trabecular volume(-28%)and cortical thickness(-14%)compared to chow-fed mice.In HFD-fed mice,bone loss was due to reduced bone formation and mineral apposition,without obvious effects on bone resorption.HFD feeding also increased skeletal expression of sclerostin and caused deterioration of the osteocyte lacunocanalicular network(LCN).In mice fed HFD,skeletal glucocorticoid signaling was activated relative to chow-fed mice,independent of serum corticosterone concentrations.We therefore examined whether skeletal glucocorticoid signaling was necessary for HFD-induced bone loss,using transgenic mice lacking glucocorticoid signaling in osteoblasts and osteocytes(HSD2^(OB/OCY)-tg mice).In HSD2^(OB/OCY)-tg mice,bone formation and mineral apposition rates were not suppressed by HFD,and bone loss was significantly attenuated.Interestingly,in HSD2^(OB/OCY)-tg mice fed HFD,both Wnt signaling(less sclerostin induction,increased PSarah Kim Holger Henneicke Lauryn L.Cavanagh Eugenie Macfarlane Lee Joanne Thai Daphne Foong Sylvia J.Gasparini Colette Fong-Yee Michael M.Swarbrick Markus J.Seibel Hong Zhou 2021Bone Research2021,9,4:2

9PD173074, a selective FGFR inhibitor, reverses MRP7 (ABCC10)-mediated MDR显示文摘Multidrug resistance protein 7(MRP7,ABCC10)is a recently identified member of the ATP-binding cassette(ABC)transporter family,which adequately confers resistance to a diverse group of antineoplastic agents,including taxanes,vinca alkaloids and nucleoside analogs among others.Clinical studies indicate an increased MRP7 expression in non-small cell lung carcinomas(NSCLC)compared to a normal healthy lung tissue.Recent studies revealed increased paclitaxel sensitivity in the Mrp7^(-/-)mouse model compared to their wild-type counterparts.This demonstrates that MRP7 is a key contributor in developing drug resistance.Recently our group reported that PD173074,a specific fibroblast growth factor receptor(FGFR)inhibitor,could significantly reverse P-glycoprotein-mediated MDR.However,whether PD173074 can interact with and inhibit other MRP members is unknown.In the present study,we investigated the ability of PD173074 to reverse MRP7-mediated MDR.We found that PD173074,at non-toxic concentration,could significantly increase the cellular sensitivity to MRP7 substrates.Mechanistic studies indicated that PD173074(1μmol/L)significantly increased the intracellular accumulation and in-turn decreased the efflux of paclitaxel by inhibiting the transport activity without altering expression levels of the MRP7 protein,thereby representing a promising therapeutic agent in the clinical treatment of chemoresistant cancer patients.Nagaraju Anreddy Atish Patel Kamlesh Sodani Rishil J.Kathawala Eugenie P.Chen John N.D.Wurpel Zhe-Sheng Chen 2014Acta Pharmaceutica Sinica B2014,4,3:2
