| 1 | The effects of fermentation and adsorption using lactic acid bacteria culture broth on the feed quality of rice straw显示文摘To improve the nutritional value and the palatability of air-dried rice straw, culture broth of the lactic acid bacteria community SFC-2 was used to examine the effects of two different treatments, fermentation and adsorption. Air-dried and chopped rice straw was treated with either fermentation for 30 d after adding 1.5 L nutrient solution(50 m L inocula L–1, 1.2×1012 CFU m L–1 inocula) kg–1 straw dry matter, or spraying a large amount of culture broth(1.5 L kg–1 straw dry matter, 1.5×1011 CFU m L–1 culture broth) on the straw and allowing it to adsorb for 30 min. The feed quality and aerobic stability of the resulting forage were examined. Both treatments improved the feed quality of rice straw, and adsorption was better than fermentation for preserving nutrients and improving digestibility, as evidenced by higher dry matter(DM) and crude protein(CP) concentrations, lower neutral detergent fiber(NDF), acid detergent fiber(ADF) and NH3-N concentrations, as well as higher lactic acid production and in vitro digestibility of DM(IVDMD). The aerobic stability of the adsorbed straw and the fermented straw was 392 and 480 h, respectively. | 2 | Extrahepatic complications to cirrhosis and portal hypertension: Haemodynamic and homeostatic aspects显示文摘In addition to complications relating to the liver, patients with cirrhosis and portal hypertension develop extrahepatic functional disturbances of multiple organ systems. This can be considered a multiple organ failure that involves the heart, lungs, kidneys, the immune systems, and other organ systems. Progressive fibrosis of the liver and subsequent metabolic impairment leads to a systemic and splanchnic arteriolar vasodilatation. This affects both the haemodynamic and functional homeostasis of many organs and largely determines the course of the disease. With the progression of the disease, the circulation becomes hyperdynamic with cardiac, pulmonary as well as renal consequences for dysfunction and reduced survival. | 3 | Tumor necrosis factor-α inhibitor therapy and fetal risk:A systematic literature review显示文摘Tumor necrosis factor-α inhibitors (anti-TNFs) are effective in the treatment of inflammatory bowel disease (IBD) recalcitrant to conventional medical therapy. As the peak incidence of IBD overlaps with the prime reproductive years, it is crucial to establish pharmacologic regimens for women of childbearing age that achieve effective disease control without posing significant fetal harm. A systematic literature review was performed to identify all human studies with birth outcomes data after maternal exposure to infliximab, adalimumab, or certolizumab pegol within 3 mo of conception or during any trimester of pregnancy. Live births, spontaneous abortions or stillbirths, preterm or premature births, low birth weight or small for gestational age infants, and congenital abnormalities were recorded. Fifty selected references identified 472 pregnancy exposures. The subsequent review includes general information regarding anti-TNF therapy in pregnancy followed by a summary of our findings. The benefits of biologic modalities in optimizing disease control during pregnancy must be weighed against the potential toxicity of drug exposure on the developing fetus. | 4 | Dynamic magnetic resonance defecography in 10 asymptomatic volunteers显示文摘AIM: Evaluation of the wide range of normal findings in asymptomatic women undergoing dynamic magnetic resonance (MR) defecography. METHODS: MR defecography of 10 healthy female volunteers (median age: 31 years) without previous pregnancies or history of surgery were evaluated. The rectum was filled with 180 mL gadolinium ultrasound gel mixture. MR defecography was performed in the supine position. The pelvic floor was visualized with a dynamic T2-weighted sagittal plane where all relevant pelvic floor organs were acquired during defecation. The volunteers were instructed to relax and then to perform straining maneuvers to empty the rectum. The pubococcygeal line (PCGL) was used as the line of reference. The movement of pelvic floor organs was measured as the vertical distance to this reference line. Data were recorded in the resting position as well as during the defecation process with maximal straining. Examinations were performed and evaluated by two experienced abdominal radiologists without knowledge of patient history. RESULTS: Average position of the anorectal junction was located at -5.3 mm at rest and -29.9 mm during straining. The anorectal angle widened significantly from 93° at rest to 109° during defecation. A rectocele was diagnosed in eight out of 10 volunteers showing an average diameter of 25.9 mm. The bladder base was located at a position of +23 mm at rest and descended to -8.1 mm during defecation in relation to the PCGL. The bladder base moved below the PCGL in six out of 10 volunteers, which was formally defined as a cystocele. The uterocervical junction was located at an average level of +43.1 mm at rest and at +7.9 mm during straining. The uterocervical junction of three volunteers fell below the PCGL; described formally as uterocervical prolapse. | 5 | Emerging concepts in liver graft preservation显示文摘The urgent need to expand the donor pool in order to attend to the growing demand for liver transplantation has obliged physicians to consider the use of suboptimal liver grafts and also to redefine the preservation strategies. This review examines the different methods of liver graft preservation, focusing on the latest advances in both static cold storage and machine perfusion(MP). The new strategies for static cold storage are mainly designed to increase the fatty liver graft preservation via the supplementation of commercial organ preservation solutions with additives. In this paper we stress the importance of carrying out effective graft washout after static cold preservation, and present a detailed discussion of the future perspectives for dynamic graft preservation using MP at different temperatures(hypothermia at 4 ℃, normothermia a t 3 7 ℃ and subnormothermia at 20 ℃- 2 5 ℃). Finally, we highlight some emerging applications of regenerative medicine in liver graft preservation. | 7 | Clinicopathological parameters predicting recurrence of pT1N0 esophageal squamous cell carcinoma显示文摘AIM To identify the clinicopathological characteristics of pT1 N0 esophageal squamous cell carcinoma(ESCC) that are associated with tumor recurrence. METHODS We reviewed 216 pT1 N0 thoracic ESCC cases who underwent esophagectomy and thoracoabdominal two-field lymphadenectomy without preoperative chemoradiotherapy. After excluding those cases with clinical follow-up recorded fewer than 3 mo and those who died within 3 mo of surgery, we included 199 cases in the current analysis. Overall survival and recurrencefree survival were assessed by the Kaplan-Meier method, and clinicopathological characteristics associated with any recurrence or distant recurrence were evaluated using univariate and multivariate Cox proportional hazards models. Early recurrence(≤ 24 mo) and correlated parameters were assessed using univariate and multivariate logistic regression models.RESULTS Forty-seven(24%) patients had a recurrence at 3 to 178(median, 33) mo. The 5-year recurrence-free survival rate was 80.7%. None of 13 asymptomatic cases had a recurrence. Preoperative clinical symptoms, upper thoracic location, ulcerative or intraluminal mass macroscopic tumor type, tumor invasion depth level, basaloid histology, angiolymphatic invasion, tumor thickness, submucosal invasion thickness, diameter of the largest single tongue of invasion, and complete negative aberrant p53 expression were significantly related to tumor recurrence and/or recurrence-free survival. Upper thoracic tumor location, angiolymphatic invasion, and submucosal invasion thickness were independent predictors of tumor recurrence(Hazard ratios = 3.26, 3.42, and 2.06, P < 0.001, P < 0.001, and P = 0.002, respectively), and a nomogram for predicting recurrence-free survival with these three predictors was constructed. Upper thoracic tumor location and angiolymphatic invasion were independent predictors of distant recurrence. | 9 | Biodiversity and Invasibility: Distribution Patterns of Invasive Plant Species in the Himalayas, Nepal显示文摘Invasive plant species are exerting a serious threat to biological diversity in many regions of the world. To understand plant invasions this study aims to test which of the two plant invasiveness hypotheses; ‘low native diversity' vs. ‘high native diversity', is supported by the regional distribution patterns of invasive plant species in the Himalayas,Nepal. This study is based on data retrieved from published literatures and herbarium specimens. The relationship between invasive plant species distribution patterns and that of native plant species is elucidated by scatter plots, as well as by generalized linear models. The native plant species and invasive plant species have similar distribution patterns and the maximum number of invasive plant species is found in the same altitudinal range where the highest richness for native tree species is found. There is a clear trend of higher invasive plant richness in regions where native tree species richness is relatively high.Consequently, the native plant richness is highest in the central phytogeographic region, followed by the eastern and the western regions, respectively. The invasive plant species also follows a similar trend.Additionally, the invasive plant species richness was positively correlated with anthropogenic factors such as human population density and the number of visiting tourists. This study supports the hypothesis that ‘high native diversity' supports or facilitates invasive plant species. | 10 | Tumor-stroma ratio as prognostic factor for survival in rectal adenocarcinoma: A retrospective cohort study显示文摘AIM To evaluate the prognostic value of the tumor-stroma ratio(TSR) in rectal cancer.METHODS TSR was determined on hematoxylin and eosin stained histological sections of 154 patients treated for rectal adenocarcinoma without prior neoadjuvant treatment in the period 1996-2006 by two observers to assessreproducibility. Patients were categorized into three categories: TSR-high [carcinoma percentage(CP) ≥ 70%], TSR-intermediate(CP 40%, 50% and 60%) and TSR-low(CP ≤ 30%). The relation between categorized TSR and survival was analyzed using Cox proportional hazards model.RESULTS Thirty-six(23.4%) patients were scored as TSR-low, 70(45.4%) as TSR-intermediate and 48(31.2%) as TSRhigh. TSR had a good interobserver agreement(κ = 0.724, concordance 82.5%). Overall survival(OS) and disease free survival(DFS) were significantly better for patients with a high TSR(P = 0.01 and P = 0.02, respectively). A similar association existed for disease specific survival(P = 0.06). In multivariate analysis, patients without lymph node metastasis and an intermediate TSR had a higher risk of dying from rectal cancer(HR = 5.27, 95%CI: 1.54-18.10), compared to lymph node metastasis negative patients with a high TSR. This group also had a worse DFS(HR = 6.41, 95%CI: 1.84-22.28). An identical association was seen for OS. These relations were not seen in lymph node metastasis positive patients. | 11 | Mapping aboveground biomass and its prediction uncertainty using LiDAR and field data, accounting for tree-level allometric and LiDAR model errors显示文摘Background: The increasing availability of remotely sensed data has recently challenged the traditional way of performing forest inventories, and induced an interest in model-based inference. Like traditional design-based inference, model-based inference allows for regional estimates of totals and means, but in addition for wall-to-wall mapping of forest characteristics. Recently Light Detection and Ranging(LiDAR)-based maps of forest attributes have been developed in many countries and been well received by users due to their accurate spatial representation of forest resources. However, the correspondence between such mapping and model-based inference is seldom appreciated. In this study we applied hierarchical model-based inference to produce aboveground biomass maps as well as maps of the corresponding prediction uncertainties with the same spatial resolution. Further, an estimator of mean biomass at regional level, and its uncertainty, was developed to demonstrate how mapping and regional level assessment can be combined within the framework of model-based inference.Results: Through a new version of hierarchical model-based estimation, allowing models to be nonlinear, we accounted for uncertainties in both the individual tree-level biomass models and the models linking plot level biomass predictions with LiDAR metrics. In a 5005 km2 large study area in south-central Sweden the predicted aboveground biomass at the level of 18 m×18 m map units was found to range between 9 and 447 Mg·ha^-1. The corresponding root mean square errors ranged between 10 and 162 Mg·ha^-1. For the entire study region, the mean aboveground biomass was 55 Mg·ha^-1 and the corresponding relative root mean square error 8%. At this level 75%of the mean square error was due to the uncertainty associated with tree-level models.Conclusions: Through the proposed method it is possible to link mapping and estimation within the framework of model-based inference. Uncertainties in both tree-level biomass models and models linking plot level biomass with LiDAR data are accounted for, both for the uncertainty maps and the overall estimates. | 18 | Impact of pediatric inflammatory bowel disease diagnosis on exercise and sports participation: Patient and parent perspectives显示文摘BACKGROUND Inflammatory bowel disease(IBD)may limit physical activity due to intestinal or extraintestinal manifestations,fatigue,or exercise perception.We sought to evaluate the influence of IBD diagnosis on exercise and sports participation in a pediatric population.We compared patient-reported and parent-reported perspectives.AIM To evaluate the influence of IBD diagnosis on exercise and sports participation in a pediatric population.METHODS Consecutive IBD outpatients(aged 10-18 years)and their parents completed parallel voluntary surveys.A validated,patient-reported functional activity scale,the Hospital for Special Surgery Pediatric Functional Activity Brief Scale(HSS Pedi-FABS)was used to assess children’s activity levels.RESULTS There were 149 completed surveys(75%response rate)with mean participant age of 16.5 years[standard deviation(SD)=4.0]and mean age at IBD diagnosis of 11.8 years(SD=3.4).Most patients(77%)were diagnosed within 12 mo of symptom onset.Current athletic participation was reported in 65%across 65 sports.Participation was greatest before(40%)rather than after(32%)IBD diagnosis,with no reported change in 28%.IBD negatively impacted play/performance in 45%but did not change play/performance in 44%.IBD treatment improved patients’desire to exercise(70%)and subjective capacity for aerobic exercise(72%).Patients and parents agreed that IBD subjects demonstrate normal capacity for aerobic exercise(0.40,95%CI:0.22-0.58)and that treatment improved both participatory desire(0.33,95%CI:0.12-0.54)and capacity for aerobic exercise(0.52,95%CI:0.31-0.71).Almost all(99%)viewed exercise as healthy,and most encouraged its practice.IBD patients demonstrated normal mean HSS Pedi-FABS scores.CONCLUSION After pediatric IBD diagnosis,most patients continue exercise and sports,with mean activity levels comparable to American youth.Treatment positively impacts participatory desire and aerobic capacity.Patients favor the role of exercise in IBD. | Renée M Marchioni Beery Enju Li Laurie N Fishman | 2019 | World Journal of Gastroenterology2019,25,31: | 2 |
