1The history of interventional therapy for liver cancer in China显示文摘In China interventional therapy of liver cancer started in the 1980s. It is well-known that Professor Lin Gui is the founding father of Interventional radiology. Under the leadership of Lin Gui and other professors, interventional therapy of liver cancer has swiftly progressed in China. Indeed, TAI, TAE, TACE and ablation therapy have witnessed great innovations in hardware facil ities, technical means, and therapeutic philosophy, while incorporating Chinese characteristics. As with the development of combined interventional therapy in China, interventional treatment of liver cancer has gradually started the process of precision and individualization. Actually, multidisciplinary, multimodal, and polymorphic treatments will be the most suitable pattern for liver cancer in the future, among which combination of interventional therapy with targeted, immunological treatments and information technology(IT) tools may bring a revolutionary breakthrough in liver cancer treatment.Mengxuan Zuo Jinhua Huang 2018Journal of Interventional Medicine2018,1,2:59

3Application of retrievable Solitaire AB stents in the endovascular treatment of acute ischemic stroke显示文摘Purpose: Retrievable stents are widely used in acute ischemic stroke(AIS); however, the results remain unclear in Chinese patients. This study aimed to explore the usefulness of Solitaire AB stents in AIS. Materials and Methods: Seventy-three AIS patients treated with Solitaire AB stents for thrombectomy of large artery occlusion of anterior circulation in January 2014-June 2015 were retrospectively evaluated. Recanalization was assessed with the Thrombolysis In Cerebral Ischemia(TICI) scale. Clinical outcomes were assessed according to the National Institute of Health Stroke Scale(NIHSS) and the modified Rankin Scale(mRS). Operation-related complications were recorded. The main factors affecting successful recanalization with Solitaire AB were analyzed. Results: The 73 patients enrolled included 39 males and 34 females(median age of 59 [31-78] years); 77 Solitaire AB stents were used. The initial recanalization rate with Solitaire AB as the first thrombectomy method was 53.42%(39/73; recanalization group). Among the 34 patients with failed stent retrieval, 32 underwent other treatments; the final arterial recanalization rate was 89.04%(65/73). Perioperative embolization events and symptomatic intracranial hemorrhage(sICH) occurred in 5 and 8 patients, respectively. The mean NIHSS score was 9.12±3.86 one week after thrombectomy, significantly lower compared with admission values. In 31 patients(42.47%), NIHSS score decreased by >8. Good functional independence(mRS score≤2) was achieved in 39 patients(53.42%) at 90 days; 12 patients(16.44%) died. Compared with the recanalization group, the remaining patients showed lower AF and higher LAA percentages. 4Consensus among Chinese experts on standard interventional therapy for deep venous thrombosis of lower extremity(second edition)显示文摘This is an update on the first edition of the expert consensus. This document discusses the indications and contraindications of interventional treatment methods for deep venous thrombosis such as anticoagulation, catheter-directed thrombolysis, percutaneous mechanical thrombectomy, percutaneous transluminal angioplasty and stent implantation. The operational procedures, considerations, preoperative management, and prevention of complications were also updated, supplemented, and revised. 5Manual aspiration thrombectomy for acute and subacute inferior vena cava thrombosis and lower extremity deep venous thrombosis显示文摘Deep vein thrombosis(DVT), which can lead to pulmonary embolism(PE), is a major contributor to the global disease burden and is the third most common cardiovascular pathology after coronary artery disease and stroke. Venous thromboembolic disease, which encompasses the disease entities of DVT and PE, affects up to 10 million cases every year and represents a serious and potentially life-threatening condition. Standard anticoagulation therapy alone is ineffective at promoting deep venous system thrombus removal. Many patients develop postthrombotic syndrome(PTS) despite being on adequate anticoagulation therapy. Aggressive therapy for rapid thrombus removal is important to prevent the development of PTS. Besides impeding the onset of PTS, rapid clearance of the thrombus is also required in the treatment of phlegmasia cerulea dolens, an uncommon but life-threatening complication of acute DVT that can lead to arterial insufficiency, compartment syndrome, venous gangrene, and limb amputation. Manual aspiration thrombectomy(MAT) can provide rapid and effective therapy that could be compared to the open surgical thrombectomy approach with minimal risk of morbidity, mortality, or recurrence after surgery. Though many devices have been developed to date for pharmacomechanical thrombolysis, the cost of the treatment using these devices is very expensive. MAT is simple to perform, easy to learn, inexpensive, and rapid. 7Image-guided locoregional non-intravascular interventional treatments for hepatocellular carcinoma: Current status显示文摘Hepatocellular carcinoma(HCC) is one of the most deadly and frequent cancers worldwide,although great advancement in the treatment of this malignancy have been made within the past few decades.It continues to be a major health issue due to an increasing incidence and a poor prognosis.The majority of patients have their HCC diagnosed at an intermediate or advanced stage in theUSA or China.Curative therapy such as surgical resection or liver transplantation is not considered anoption of treatment at these stages.Transarterial chemoembolization(TACE),the most widely used locoregional therapeutic approach,used to be the mainstay of treatment for cases with unresectable cancer entities.However,for those patients with hypovascular tumors or impaired liver function reserve,TACE is a suboptimal treatment option.For example,embolization does not result in complete coverage of a hypovascular tumor,and may rather promotes postoperative tumor recurrence,or leave residual tumor,in these TACE-resistance patients.In addition,TACE carries a higher risk of hepatic decompensation in patients with poor liver function or reserve.Non-vascular interventional locoregional therapies for HCC include radiofrequency ablation(RFA),microwave ablation(MWA),high-intensity focused ultrasound(HIFU),laser-induced thermotherapy(LITT),cryosurgical ablation(CSA),irreversible Electroporation(IRE),percutaneous ethanol injection(PEI),and brachytherapy.Recent advancements in these techniques have significantly improved the treatment efficacy of HCC and expanded the population of patients who qualify for treatment.This review embraces the current status of imaging-guided locoregional non-intravascular interventional treatments for HCCs,with a primary focus on the clinical evaluation and assessment of the efficacy of combined therapies using these interventional techniques.Kun Qian Feng Zhang Stephen K.Allison Chuansheng Zheng Xiaoming Yang 2021Journal of Interventional Medicine2021,4,1:11

9Percutaneous vertebroplasty in the treatment of malignant vertebral compression fractures with epidural involvement显示文摘Purpose: To evaluate the safety and the clinical efficacy of percutaneous vertebroplasty(PVP) in treating malignant spinal tumors and malignant vertebral compression fractures with epidural involvement. Materials and methods: 43 patients with spinal metastatic tumors and malignant vertebral compression fractures with epidural involvement were treated using PVP. American Spinal Injury Association(ASIA) impairment scale results at presentation were used to divide patients into 2 groups. Patients in group A had no symptoms of neurological compression(n = 25); and patients in group B had symptoms of neurological compression(n = 28). A 13 G bone puncture needle was placed across the pedicle of the fractured vertebra, and polymethyl methacrylate(PMMA) was injected into the fractured vertebral body under fluoroscopic control. Patients were seen in follow-up at 1, 3, and 6 months after the procedure and every six months thereafter. Results: PVP was technically successful and well-tolerated in all patients. Clinical assessment at the final follow-up found complete pain relief(n = 19) or good pain relief(n = 14) in 33 patients(62.3%, 95% CI: 49%, 76%). ASIA impairment scale assessment at the final follow-up demonstrated symptoms of neurologic compression in 31 patients and no symptoms of neurologic compression in 22 patients. Symptoms of neurologic compression were found in five group A patients and eight group B patients. 10Percutaneous osteoplasty for extraspinal metastases显示文摘As an extension of percutaneous vertebroplasty(PVP), percutaneous osteoplasty(POP) refers broadly to percutaneous bone cement injected into various parts of the body and narrowly to cement injected into extraspinal bone lesions. POP mainly includes such surgeries as percutaneous sacroplasty, percutaneous acetabuloplasty, percutaneous femoral osteoplasty, and percutaneous iliac osteoplasty(Figure 1). Currently, POP is a positive and an effective treatment for extraspinal bone lesions in that it can rapidly relieve pain, effectively prevent pathological fractures, and partially inactivate tumors, with few complications. 12Radiofrequency ablation combined with transcatheter arterial chemoembolization therapy versus surgical resection for Barcelona-Clinic Liver Cancer(BCLC) A hepatocellular carcinoma: a meta-analysis显示文摘Purpose: The objective of our study was to compare the effectiveness of the combination of transcatheter arterial chemoembolization(TACE) and radiofrequency ablation(RFA) with that of surgical resection(SR) in Barcelona-Clinic Liver Cancer(BCLC) A hepatocellular carcinoma. Materials and Methods: PubMed, Medline, Embase, and Cochrane Library were searched for comparisons of the two therapies from January 2006 to December 2017. Overall survival rate(OS), recurrence-free survival rate(RFS), complications, and the average length of hospital stay were compared and analyzed. Review Manager v. 5.2 from the Cochrane Collaboration was used for statistical analyses. Results: Seven case-control studies and one randomized controlled trial were identified, of which 717 were treated with a combination of TACE and RFA and 785 were treated with SR. Meta-analysis data revealed that TACE plus RFA had significantly better effectiveness on 1.0-y OS(OR = 0.50, p =.009). The major complications(ORcomplications = 1.88, p =.02) after the combined therapy were significantly lower than those after SR. There were three studies that reported the average length of hospital stay. The hospital stay for the SR group vs the combined therapy group was 19.8 ± 8.4 d vs 7.4 ± 2.2 d, respectively(p <.0001); 18.7 ± 4.9 d vs 11.5 ± 6.9 d, respectively(p <.0001); and 16.6 ± 6.7 d vs 8.5 ± 4.1 d, respectively(p <.0001). There was no significant difference in 3.0-or 5.0-y OS and 1.0-, 3.0-, or 5.0-y RFS. 13A new choice of stent for transjugular intrahepatic portosystemic shunt creation: Viabahn ePTFE covered stent/bare metal stent combination显示文摘Objectives:To compare the clinical outcomes in terms of structure and function between the insertion of a transjugular intrahepatic portosystemic shunt(TIPS) created with the Viabahn ePTFE covered stent/bare metal stent(BMS) combination and the Fluency ePTFE covered stent/BMS combination.Methods:A total of 101 consecutive patients who received a TIPS from February 2016 to August 2018 in our center were retrospectively analyzed.Sixty-four subjects were enrolled in the Viabahn group and 37 were enrolled in the Fluency group.The geometry characteristics of the TIPS were calculated,and the associated occurrence of shunt dysfunction,survival,overt hepatic encephalopathy,and variceal rebleeding were evaluated.Results:The technical success rate was 100%.After the insertion of the TIPS,the rate of shunt dysfunction during the first 3 months was significantly different between the Viabahn and Fluency groups(1.6% and 13.5%,respectively;p=0.024).Multivariate analysis indicated that the angle of portal venous inflow(α) was the only independent risk factor for shunt dysfunction(hazard ratio=1.060,95% confidence interval=1.009-1.112,p=0.020).In addition,3 months after the TIPS insertion,the a angle distinctly increased from 20.9°±14.3°-26.9°±20.1°(p=0.005) in the Fluency group but did not change significantly in the Viabahn group(from 21.9°±15.1°-22.9°± 17.6°,p=0.798).Conclusions:Shunt dysfunction was related to the a angle owing to the slight effect on the a angle after the implantation of the TIPS.The Viabahn ePTFE covered stent/BMS combination was more stable in structure and promised higher short-term stent patency compared with the Fluency ePTFE covered stent/BMS combination.Jiacheng Liu Jie Meng Chen Zhou Qin Shi Chongtu Yang Jinqiang Ma Manman Chen Bin Xiong 2021Journal of Interventional Medicine2021,4,1:9
14Endovascular treatment of blood blister-like aneurysms in the internal carotid artery using a Willis covered stent显示文摘Background Despite the current availability of flow diverter devices(FDD), problems remains regarding optimal endovascular treatment(EVT) for blood blister-like aneurysms(BBAs) of the internal carotid artery(ICA). Objective To evaluate the safety and efficacy of EVT of BBAs in the ICA with a Willis covered stent. Methods 20 consecutive patients(5 men and 15 women) with ruptured BBAs underwent EVT using a Willis covered stent in two institutions from March 2013 to March 2018. Clinical observations, angiographic characteristics, and procedural and follow-up outcomes were retrospectively evaluated. 15Precise localization of small pulmonary nodules using Pre-VATS with Xper-CT in combination with real-time fluoroscopy-guided coil:report of 15 patients显示文摘Purpose: This study aimed to evaluate the value of precise localization of nodules using pre-video-assisted thoracic surgery(VATS) Xper–CT in combination with real-time fluoroscopy-guided coil in the resection of pulmonary nodules using VATS. Materials and Methods: Precise localization of nodules using Xper-CT in combination with real-time fluoroscopy-guided coil and wedge resection using VATS were conducted on 15 patients with 17 small pulmonary nodules(diameter 0.5–1.5 cm) from April 2015 to January 2016. The value of localization was evaluated in terms of procedure time, type of coils, associated complications of localization, and VATS success rate. Results: The success rate of coil localization was found to be 100% in the primary stage(as shown by the CT scan), and the average procedure time was 30–45 min(35.6 ± 3.05 min). No deaths or major complications occurred. Minor complications included five incidents of pneumothorax(the morbidity was 29.4%, 5/17; no patient required chest tube drainage). The dislocation of coil was found in one patient. The results of pathological examination of 17 small pulmonary nodules revealed 11 primary lung cancers, 1 mesenchymal tumor, 3 nonspecific chronic inflammations, 1 hamartoma, and 1 tuberculosis. Two patients with primary lung cancer underwent lobectomy with mediastinal lymph node dissection. Conclusion: The preoperative precise localization of small pulmonary nodules using Xper-CT-guided coil is an effective and safe technique. It helps in the resection of nodules using VATS. 16Bronchial artery embolization for hemoptysis:A systematic review and meta-analysis显示文摘Objective:To assess the safety and efficacy of bronchial artery embolization(BAE)for hemoptysis.Methods and materials:Databases with articles published in English,including Pubmed,Embase,Web of science and Chochrane library,were comprehensively searched to get accurate,up-to-date and sufficient literature about BAE for hemoptysis until March 2020.The technical success rates,immediate control rates,recurrence rates,mortality rates,and total complication rates(minor and major complication rates)extracted from the articles were pooled to estimate and assess the efficacy and safety of BAE using random-effect and fixed-effect models.Results:21 articles published between 2008 and 2019,which include a total of 2511 patients,were studied to evaluate the safety and efficacy of BAE.The technical success and immediate control rates are 99.9%(95%CI:99%-100%)and 99.5%(95%CI:97.8%-99.2%),respectively.This study showed hemoptysis recurrence in 23.7%(95%CI:18.5%-28.9%)with a mortality rate of 2%(95%CI:0-3%).Additionally,the assessment of complications revealed a total complication rate of 13.4%(95%CI:7.6-19.2%),in which 0.2%(95%CI:0.2-0.4%)were major complications and 10%(95%CI:4.7-9.6%)were minor complications.Conclusion:BAE is an effective,safe,and feasible procedure with a low complication rate for hemoptysis patients.However,recurrence of hemoptysis is still at high risk after BAE due to different underlying diseases.Zhiyuan Zheng Zhiquan Zhuang Minjie Yang Jianjun Luo Wen Zhang Zhiping Yan Xiaolin Wang 2021Journal of Interventional Medicine2021,4,4:7
17The application and efficacy of stent place for Budd-Chiari syndrome显示文摘Objective To evaluate the application value and efficacy on stent place for Budd-Chiari syndrome(BCS). Methods From January 1990 to May 2017, 2228 patients with BCS were admitted to our institution. The mean age was 43.3 years. Stents were placed in inferior vena cava(IVC), hepatic vein(HV), or both after balloon dilation. During follow-up period, the patency of stent was evaluated by ultrasound regularly and the clinic sign was surveyed by letter, telephone or clinic visit. The restenosis of stent were treated with balloon dilatation and thrombolysis to restore the its function. Results IVC type was diagnosed in 1492 cases, HV type in 510 cases, and mixed type in 226 cases. Eighteen patients aborted treatment because of economic reasons, advanced liver cancer, severe scoliosis, or both bilateral iliac veins and total IVC occlusion. Among the other 2210 cases who underwent endovascular therapy, stents were implanted into IVC in 339 cases, HV in 97 cases, mixed type in 64 cases. The rate of restenosis in IVC stent was 11.50%(39/339). After repeat angioplasty, the long-term patency rate reached to 98.12%. The incidence of HV occlusion caused by IVC stent was 12.09%(n = 41). Restenosis occurred in 47 cases(48.45%) after HV stent placement. However, the 5-year patency rate was 91.75%(89/97) after repeat dilatation and stent re-implantation. The incidence of IVC obstruction caused by HV was 3.33%(3 cases). Conclusion IVC stent placement appears to be an effective treatment for the cases of IVC segmental occlusion, and at the same time, the stent has the dual role of compression and fixation of thrombus and support of lumen. The HV and accessory hepatic vein obstruction could happen when the IVC stent crossed these veins ostium. 18Analysis of factors affecting the prognosis of transcatheter arterial chemoembolization for hepatitis B-related hepatocellular carcinoma显示文摘Objectives:The purpose of this study was to investigate the prognostic factors for transcatheter arterial chemoembolization(TACE)for hepatitis B-related hepatocellular carcinoma(HCC).Materials and methods:The variables that may affect overall survival(OS),such as age,gender,AFP,Child Pugh classification,body mass index,HBV-DNA,Hbe Ag,tumor number,tumor diameter,BCLC stage,embolization method,ablation therapy,and targeted therapy,were analyzed by single factor and many factor COX regression.In addition,predictive factors of OS were stratified and a Kaplan-Meier survival curve was drawn.Results:Among the 136 patients,the median follow-up time was 14.5 months(range:2–72 months).HCC patients with the tumor diameter<3 cm had the highest survival rate,followed by patients with a tumor diameter of 3–5 cm;the survival rate of patients with the tumor diameter(greater than 5 cm)was the lowest.Among the BCLC stages,stage A patients had the highest survival rate,followed by stage B and stage C patients,which had the lowest survival rate.The survival rate of Child Pugh grade A patients was higher than those with Child Pugh grade B.Compared with patients who did not undergo ablation treatment,the survival rate of patients with combined ablation treatment was relatively high.The survival rate of patients receiving drug-eluting beads transarterial chemoembolization(DEB-TACE)treatment was higher than those receiving conventional transarterial chemoembolization(c TACE)treatment.Additionally,repeated TACE treatment improved the OS rate of patients.These six factors were related to patient prognosis and the differences were statistically significant(P<0.05).Conclusions:Tumor diameter,BCLC stage,TACE repetition,and TACE combined with ablation were independent prognostic factors of OS.Jiangping Cun Yonghui Xu Weidong Li Xingxiang Zhao 2021Journal of Interventional Medicine2021,4,2:7
19A comparative study of TR Band and a new hemostatic compression device after transradial coronary catheterization显示文摘Objective: Transradial coronary catheterization has proved to be safe and effective in clinical practice. Various hemostatic compressive devices have been used in subsequent procedures. The objective of this study was to compare the efficacy and safety of a new hemostatic compression device and the widely used TR Band. Methods: A total of 118 patients were divided randomly into two groups: TR Band and the new hemostatic compression device. Efficacy of hemostasis, patient comfort, local vascular dysfunction, and radial artery occlusion(RAO) were evaluated and compared between groups. Results: Occurrence of errhysis or hematoma did not significantly differ between groups(13.6% vs. 11.9%, P = 0.782). Fewer patients had moderate to severe pain or moderate to severe numbness in the new hemostatic compression device group(1.7% vs. 22.0%; 1.7% vs. 18.6%, respectively). Pulse loss between distal artery and device was lower in the new hemostatic compression device group(5.1% vs. 22.0%, P = 0.007), and fewer patients experienced obstruction of venous reflux compared with the TR Band group(6.8% vs. 25.4%, P = 0.006). Combined incidence of RAO at discharge was 7.6%, and was lower in the new hemostatic compression device group(1.7% vs. 13.6%, P = 0.015). In contrast to the TR Band, application of the new hemostatic compression device was independently associated with lower incidence of RAO at discharge(odds ratio: 0.062, 95% confidence interval: 0.006–0.675, P = 0.022). Conclusions: Both the new hemostatic compression device and the TR Band can efficiently achieve hemostasis following transradial coronary catheterization. However, fewer patients felt discomfort with application of the new hemostatic compression device. 20Impact of serum levels of lipoprotein lipase, hepatic lipase, and endothelial lipase on the progression of coronary artery disease显示文摘Purpose: The purpose of this study was to investigate the relationship between serum levels of lipoprotein lipase(LPL), hepatic lipase(HL), and endothelial lipase(EL) and the progression of coronary artery disease(CAD).Materials and methods: According to the inclusion criteria, exclusion criteria, diagnostic criteria, angiography results, and the random matching scheme, the enrolled patients were divided into the following two groups: the progression-free group(n ? 47) and the progression group(n ? 15). The baseline characteristics and various biochemical parameters were obtained from the medical records and medical history. Serum LPL, HL, and EL levels were detected by ELISA. The correlation between serum LPL, HL, and EL levels and coronary lesions was statistically analyzed with SPSS software.Results: Significant differences were observed in serum levels of HL and EL between the progression-free group and the progression group(HL, 75.5 ? 39.2 ng/mL vs. 125.1 ? 42.1 ng/mL, P < 0.05;EL, 139.2 ? 59.6 pg/mL vs.175.1 ? 40.1 pg/mL, P < 0.05), while the difference in the LPL level was not significant(P > 0.05). Receiver operating characteristic curve(ROC) analysis showed that the area under the curve(AUC) values of LPL, HL, and EL were 0.506(95% CI: 0.369–0.642, P ? 0.9470), 0.792(95% CI: 0.664–0.888, P < 0.0001), and 0.693(95% CI:0.553–0.811, P ? 0.0095), respectively. Additionally, logistic regression analysis showed that the serum level of HL was an independent risk factor for coronary artery lesion progression.Conclusion: Serum levels of EL and HL, but not the serum level of LPL, were positively correlated with the progression of CAD. The serum level of HL was an independent risk factor for the progression of CAD, while the serum level of EL or LPL was not an independent risk factor for the progression of CAD.
