Chronic myeloid leukemia-from the Philadelphia chromosome to specific target drugs:A literature review显示文摘Chronic myeloid leukemia(CML)is a myeloproliferative neoplasm and was the first neoplastic disease associated with a well-defined genotypic anomaly―the presence of the Philadelphia chromosome. The advances in cytogenetic and molecular assays are of great importance to the diagnosis, prognosis, treatment, and monitoring of CML.The discovery of the breakpoint cluster region(BCR)-Abelson murine leukemia(ABL)1 fusion oncogene has revolutionized the treatment of CML patients by allowing the development of targeted drugs that inhibit the tyrosine kinase activity of the BCR-ABL oncoprotein.Tyrosine kinase inhibitors(known as TKIs)are the standard therapy for CML and greatly increase the survival rates, despite adverse effects and the odds of residual disease after discontinuation of treatment. As therapeutic alternatives, the subsequent TKIs lead to faster and deeper molecular remissions;however, with the emergence of resistance to these drugs, immunotherapy appears as an alternative, which may have a cure potential in these patients.Against this background, this article aims at providing an overview on CML clinical management and a summary on the main targeted drugs available in that context. | Mariana Miranda Sampaio Maria Luísa Cordeiro Santos Hanna Santos Marques Vinícius Lima de Souza Gonçalves Glauber Rocha Lima Araújo Luana Weber Lopes Jonathan Santos Apolonio Camilo Santana Silva Luana Kauany de SáSantos Beatriz Rocha Cuzzuol Quézia Estéfani Silva Guimarães Mariana Novaes Santos Breno Bittencourt de Brito Filipe Antônio França da Silva Márcio Vasconcelos Oliveira Cláudio Lima Souza Fabrício Freire de Melo | 2021 | World Journal of Clinical Oncology2021,12,2: | 3

Litterfall dynamics and soil carbon and nitrogen stocks in the Brazilian palm swamp ecosystems显示文摘Background: This study aimed to determine the litterfall production, accumulation, decomposition rate and nutrient stocks, and to estimate the soil carbon(C) and nitrogen(N) stocks in three palm swamp ecosystems with different land use intensities in the Southeast of Brazil. Methods: Three palm swamp ecosystems with different land use intensities were evaluated: Agua Doce(AD),conserved area;Capivara(CV), area with small agricultural practices;and Buriti Grosso(BG), area with an intensive land use. Produced and accumulated litterfall from the Cerrado areas surrounding the palm swamps were collected from October 2014 to September 2015, and the Ca, Mg, K, and P concentrations were determined in the dry and rainy seasons. Soil samples were collected in the surrounding Cerrado and within the palm swamps until 100 cm soil depth to determine bulk density and soil C and N contents and stocks. Results: Annual litterfall production in the Cerrado surrounding palm swamps was similar in AD(3.58 Mg·ha^-1·year^-1) and CV(3.79 Mg·ha^-1·year^-1), and was lower in BG(2.84 Mg·ha^-1·year^-1), and was more intense during the dry season. Furthermore, litterfall accumulation was higher in CV(7.12 Mg·ha^-1·year^-1) and BG(6.75 Mg·ha^-1·year^-1), culminating in lower decomposition rates. AD showed the highest decomposition rate(0.60) due to its vegetation structure and conservation. The macronutrient contents from the production and accumulated litterfall had a low influence of the land use, decreasing in the following order: Ca > K > Mg > P. Soils in CV and BG palm swamp areas showed higher values of C and N contents, due to the use of land for agricultural crops and cattle raising in an extensive(rangeland) system. The BG palm swamp, showed the highest C(124.03 Mg·ha^-1) and N(10.54 Mg·ha^-1) stocks due to the land use history with different agricultural practices over time. Conclusions: The litterfall dynamics in the Cerrado surrounding palm swamps was more affected by climatic variables than the intensity of land use, but the litter decomposition was more accelerated in the conserved area.Anthropic interventions with soil organic matter(SOM) inputs contributed to an increase in soil C and N stocks in the palm swamp ecosystems. | Gracielle de Brito Sales Taynan Aquilles Marinho Lessa Daniela Aparecida Freitas Maria das Dores Magalhães Veloso Maria Ligia de Souza Silva Luiz Arnaldo Fernandes Leidivan Almeida Frazão | 2020 | Forest Ecosystems2020,7,3: | 1

Prevalence, phenotype and medication for the pediatric inflammatory bowel disease population of a state in Southeastern Brazil显示文摘BACKGROUND Inflammatory bowel disease(IBD)can lead to social and economic impacts worldwide. In Brazil, where its adult prevalence is increasing, the epidemiology of the pediatric population is not well known, although there is a documented increase in pediatric IBD incidence worldwide.Brazil has continental dimensions, and Espírito Santo is a state of southeastern Brazil, the region with the highest demographic densities and is the economically most important in the country.AIM To assess the prevalence, incidence, phenotype and medications in a Southeastern Brazilian pediatric population.METHODS Data were retrieved from the Public Medication-Dispensing System of the Department of Health in Espírito Santo state from documentation required to have access to highly expensive medication from August 1,2012 to July 31,2014.There were 1048 registered patients with IBD of all ages, and of these patients, the cases≤17 years were selected. The data were obtained through the analysis of administrative requests for these medications and included medical reports, endoscopy exams, histopathology and imaging tests, which followed the Clinical Protocols and Therapeutic Guidelines of the Brazilian Government.Only confirmed cases of IBD were included in the study.RESULTS There were 55 pediatric patients/1048 registered patients(5.34%),with Crohn's disease(CD)representing 30/55(55%),ulcerative colitis(UC)24/55(43.6%)and 1 unclassified IBD,a significant difference from adult patients(P=0.004).The prevalence of IBD in pediatric patients was 5.02 cases/100.000 inhabitants;the incidence in 2014 was 1.36 cases/100.000 inhabitants. The mean age at diagnosis was 12.2 years(±4.2).There were 7 children diagnosed up to 6 years old,7 between 7 to 10 years old and 41 between 11 and≤17 years old. There was no difference in the distribution of UC and CD between these age categories(P=0.743).There was no difference in gender distribution in relation to adults.Children and adolescents with UC had a predominance of pancolitis, unlike adults(P=0.001),and used aminosalicylates and immunomodulators for their treatment.Pediatric patients with CD did not present a difference in disease location but had a higher frequency of fistulizing behavior(P=0.03)and perianal disease phenotype(P=0.007)than adult patients.Patients with CD used more immunomodulators and biological therapy.Treatment with biological therapy was more frequently used in pediatric patients than in adults(P<0.001).CONCLUSION Although the data from this study demonstrate that incidence and prevalence rates are low in southeastern Brazil, these data demonstrate the severity of IBD in pediatric patients, with the need for early diagnosis and therapy, avoiding serious damage. | Adalberta Lima Martins Renata de SáBrito Fróes Maria da Penha Zago-Gomes | 2022 | World Journal of Clinical Pediatrics2022,11,4: | 0
Epidemiological and clinical characteristics of COVID-19 in a Brazilian public hospital显示文摘BACKGROUND The coronavirus disease 2019(COVID-19)pandemic has become a major health concern worldwide. In that context, the understanding of epidemiological and clinical features associated with the disease and its severity is crucial for the establishment of strategies aimed at disease control and remedy.AIM To describe epidemiological features, signs, symptoms, and laboratory findings among severely ill COVID-19 patients from an inten-sive care unit in northeastern Brazil as well as to evaluate predictor factors for disease outcomes.METHODS This is a prospective single-center study that evaluated 115 patients admitted to the intensive care unit in a northeastern Brazilian hospital.RESULTS The patients had a median age of 65.60±15.78 years.Dyspnea was the most frequent symptom, affecting 73.9%of the patients, followed by cough(54.7%).Fever was reported in approximately one-third of patients and myalgia in 20.8%of the patients. At least two comorbidities were found in 41.7%of the patients, and hypertension was the most prevalent(57.3%).In addition, having two or more comorbidities was a predictor of mortality, and lower platelet count was positively associated with death.Nausea and vomiting were two symptoms that were predictors of death, and the presence of a cough was a protective factor.CONCLUSION This is the first report of a negative correlation between cough and death in severely ill severe acute respiratory syndrome coronavirus 2-infected individuals. The associations between comorbidities, advanced age,and low platelet count and the outcomes of the infection were similar to the results of previous studies, highlighting the relevance of these features. | Fillipe Dantas Pinheiro Luana Weber Lopes Rafael Santos Dantas Miranda Dórea Glauber Rocha Lima Araújo Filipe Antônio França da Silva Breno Bittencourt de Brito Maria Luísa Cordeiro Santos Geovani Moreno Santos Júnior Maria Teresa Araújo de Lorenzo Barcia Renata de Amorim Marques AndréBezerra Botelho Anna Carolina Saúde Dantas Davi Tanajura Costa Adriano Fernandes Teixeira Cláudio Lima Souza Lucas Miranda Marques Guilherme Barretos Campos Márcio Vasconcelos Oliveira Dulciene Maria de Magalhães Queiroz Fabrício Freire de Melo | 2023 | World Journal of Clinical Cases2023,11,8: | 0
Treatment of Helicobacter pylori infection in children:A systematic review显示文摘BACKGROUND Helicobacter pylori(H.pylori)infection is predominantly acquired in childhood. When indicated, the most accepted treatment for H.pylori eradication in this age group is first-line triple therapy. However, the increasing resistance to clarithromycin and nitroimidazoles has been associated with treatment failure, and thus, alternative treatment regimens have been proposed.AIM To perform a systematic review of randomized controlled trials on treatment regimens for H.pylori infection in children.METHODS We surveyed relevant articles published in English from 2010 to April 2020 in the PubMed and MEDLINE databases. Keywords included“Helicobacter pylori“/”children or childhood“/”treatment or eradication.”The risk of bias was evaluated according to the Cochrane Handbook of Systematic Reviews for Interventions.RESULTS Among the 1144 records identified through the database,20 articles were selected.Four studies compared the eradication rates of H.pylori infection between standard triple therapies, changing only the main antibiotic used.Seven studies evaluated the effectiveness of standard triple therapy with the addition of probiotics. One study investigated the relationship between the effectiveness in the eradication rates of standard triple therapy and vitamin E levels.Six studies analyzed the eradication rates of sequential therapy.CONCLUSION The findings suggest that although standard triple therapy is the most recommended regimen for children by the current guidelines, other therapeutic schemes have shown promising results and may also be recommended for clinical practice in the future. | Filipe Antônio França da Silva Breno Bittencourt de Brito Maria Luísa Cordeiro Santos Hanna Santos Marques Mariana Miranda Sampaio Ronaldo Teixeira da Silva Júnior Jonathan Santos Apolonio Lorena Sousa de Carvalho Camilo Santana Silva Luana Kauany de SáSantos Márcio Vasconcelos Oliveira Gifone Aguiar Rocha Dulciene Maria de Magalhães Queiroz Fabrício Freire de Melo | 2020 | World Journal of Meta-Analysis2020,8,4: | 0
Metabolic and nutritional triggers associated with increased risk of liver complications in SARS-CoV-2显示文摘Obesity,diabetes,cardiovascular and respiratory diseases, cancer and smoking are risk factors for negative outcomes in severe acute respiratory syndrome coronavirus 2(SARS-CoV-2),which can quickly induce severe respiratory failure in 5%of cases.Coronavirus disease-associated liver injury may occur during progression of SARS-CoV-2 in patients with or without pre-existing liver disease, and damage to the liver parenchyma can be caused by infection of hepatocytes.Cirrhosis patients may be particularly vulnerable to SARS-CoV-2 if suffering with cirrhosis-associated immune dysfunction.Furthermore, pharmacotherapies including macrolide or quinolone antibiotics and steroids can also induce liver damage. In this review we addressed nutritional status and nutritional interventions in severe SARS-CoV-2 liver patients. As guidelines for SARS-CoV-2 in intensive care(IC)specifically are not yet available, strategies for management of sepsis and SARS are suggested in SARS-CoV-2.Early enteral nutrition(EN)should be started soon after IC admission, preferably employing iso-osmolar polymeric formula with initial protein content at 0.8 g/kg per day progressively increasing up to 1.3 g/kg per day and enriched with fish oil at 0.1 g/kg per day to 0.2 g/kg per day.Monitoring is necessary to identify signs of intolerance, hemodynamic instability and metabolic disorders, and transition to parenteral nutrition should not be delayed when energy and protein targets cannot be met via EN.Nutrients including vitamins A,C,D,E,B6,B12,folic acid, zinc, selenium andω-3 fatty acids have in isolation or in combination shown beneficial effects upon immune function and inflammation modulation.Cautious and monitored supplementation up to upper limits may be beneficial in management strategies for SARS-CoV-2 liver patients. | Rosangela Passos de Jesus Jozélio Freire de Carvalho Lucivalda Pereira Magalhães de Oliveira Carla de Magalhães Cunha Thaisy Cristina Honorato Santos Alves Sandra Tavares Brito Vieira Virginia Maria Figueiredo Allain Amador Bueno | 2022 | World Journal of Hepatology2022,14,1: | 0
