1Age exerts a continuous effect in the outcomes of Asian breast cancer patients treated with breast-conserving therapy显示文摘Background:Asians are diagnosed with breast cancer at a younger age than Caucasians are.We studied the effect of age on locoregional recurrence and the survival of Asian breast cancer patients treated with breast-conserving therapy.Methods:Medical records of 2492 patients treated with breast-conserving therapy between 1989 and 2012 were reviewed.The Kaplan-Meier method was used to estimate locoregional recurrence,breast cancer-free survival,and breast cancer-specific survival rates.These rates were then compared using log-rank tests.Outcomes and age were modeled by Cox proportional hazards.Fractional polynomials were then used to test for non-linear relationships between age and outcomes.Results:Patients≤40 years old were more likely to have locoregional recurrence than were older patients(Hazard ratio[HR]=2.32,P<0.001).Locoregional recurrence rates decreased year-on-year by 4%for patients with luminal-type breast cancers,compared with 8%for those with triple-negative cancers.Similarly,breast cancer-free survival rates increased year-on-year by 4%versus 8%for luminal-type and triple-negative cancers,respectively.Breast cancer-spe-cific survival rates increased with age by 5%year-on-year.Both breast cancer-free survival and breast cancer-specific survival rates in patients with luminal cancers exhibited a non-linear(“L-shaped”)relationship-where decreasing age at presentation was associated with escalating risks of relapse and death.The influence of age on overall survival was confounded by competing non-cancer deaths in older women,resulting in a“U-shaped”relationship.Conclusions:Young Asian breast cancer patients have a continuous year-on-year increase in rates of disease relapse and cancer deaths compared with older patients with no apparent threshold.Fuh Yong Wong Wei Ying Tham Wen Long Nei Cindy Lim Hui Miao 2018Cancer Communications2018,38,1:9

6Alanine aminotransferase is an inadequate surrogate marker for detecting lamivudine resistance显示文摘AIM: To investigate the accuracy of serum alanine aminotransferase (ALT) in diagnosing lamivudine resistance and factors that contributed to abnormal serum ALT.METHODS: This was a retrospective study of chronic hepatitis B patients on lamivudine therapy who were followed for 3-mo with liver function tests and hepatitis B virus (HBV) DNA measurement. Lamivudine resistance was defined as HBV DNA ≥ 1 log from nadir on at least 2 occasions, confirmed by genotyping. Serum ALT levels in patients with lamivudine resistance were compared to serum ALT levels in those without lamivudine resistance. RESULTS: There were 111 patients with and 117 without lamivudine resistance. The area under the receiver operating characteristic of serum ALT to diagnose lamivudine resistance was 0.645 ± 0.037. Serum ALT > 42.5 U/L gave the best diagnostic accuracy with sensitivity = 61%, specificity = 60%, positive predictive value = 60%, negative predictive value = 61%, positive likelihood ratio = 1.53 and negative likelihood ratio = 0.65 for predicting lamivudine resistance, missing 39% of resistant patients. Using other serum ALT cutoffs, diagnostic accuracy was lower. By multivariate analysis, baseline abnormal serum ALT was associated with abnormal ALT during resistance (OR = 5.98, P = 0.003), and males were associated with serum ALT flares during resistance (OR = 8.9, P = 0.016). 9Prevalence and risk factors of intestinal parasitism among two indigenous subethnic groups in Peninsular Malaysia显示文摘Background:Intestinal parasitic infections(IPIs)among indigenous people have been widely documented in Malaysia,however,the prevalence of these infections remains high.In the past,most studies have focused on specific species of parasites but polyparasitism has received limited attention.In addition,epidemiology studies on indigenous people tend to consider them as a homogenous group,whereas in reality different sub-ethnic groups have different cultural and living practices.Variations in living habits such as personal hygiene practices may predispose different groups to different parasitic infections.To better understand prevalence and risk factors of intestinal parasitism among different sub-ethnic groups,the present study was conducted among two sub-ethnic groups of indigenous people(Temuan and Mah Meri)residing in Selangor state,Malaysia.Methods:A cross-sectional study that focused on two distinct sub-ethnic groups was carried out from February to September 2014.Faecal samples were collected from 186 participants and examined using the formalin-ether sedimentation technique.A molecular approach was adopted to conduct a genetic characterisation of the parasites.Additionally,questionnaires were administered to obtain information on the demographics,socioeconomic backgrounds and behavioural risks relating to the participants,as well as information about their environments.Statistical analyses(i.e.binary and multivariate logistic regression analyses)were performed to measure risk factors.Results:For Temuan communities,trichuriasis(64.2%)was the most common infection found,preceding hookworm infection(34%),ascariasis(7.5%),giardiasis(14.2%)and amoebiasis(7.5%).As for the Mah Meri communities,trichuriasis(77.5%)prevailed over ascariasis(21.3%),hookworm(15%),giardiasis(7.5%)and amoebiasis(3.8%).Significant differences in proportions of trichuriasis,ascariasis and hookworm infections were observed between the Temuan and Mah Meri sub-ethnic groups.Polyparasitism was more common among the Temuan sub-ethnic group(41.5%)compared to the Mah Meri sub-ethnic group(32.5%),with the majority of participants harbouring two parasites concurrently(Temuan:33%,Mah Meri:20%).Trichuris trichiura and Ascaris lumbricoides co-infections were most prevalent(10%)among the Mah Meri communities,while a co-infection of T.trichiura with hookworm(19.8%)was most common among the Temuan communities.Multivariate analyses showed that being unemployed,having a large family and drinking unboiled water were found to be significantly associated with intestinal parasitism.Conclusion:The present study highlights substantial polyparasitism and risk factors for infections in the Temuan and Mah Meri sub-ethnic groups.The high prevalence of IPIs among these two sub-ethnic groups indicates that parasitic infections are important health issues in these communities.Hence,it is imperative to implement sound intervention strategies such as periodic preventive chemotherapy coupled with health education in order to reduce and eradicate these infections.Yuee Teng Chin Yvonne Ai Lian Lim Chun Wie Chong Cindy Shuan Ju Teh Ivan Kok Seng Yap Soo Ching Lee Mian Zi Tee Vinnie Wei Yin Siow Kek Heng Chua 2016Infectious Diseases of Poverty2016,5,1:0
