1Fatty liver disease in severe obese patients:Diagnostic value of abdominal ultrasound显示文摘AIM:To evaluate the sensitivity and specificity of abdominal ultrasound (US) for the diagnosis of hepatic steatosis in severe obese subjects and its relation to histological grade of steatosis. METHODS: A consecutive series of obese patients, who underwent bariatric surgery from October 2004 to May 2005, was selected. Ultrasonography was performed in all patients as part of routine preoperative time and an intraoperative wedge biopsy was obtained at the beginning of the bariatric surgery. The US and histological findings of steatosis were compared, considering histology as the gold standard. RESULTS: The study included 105 patients. The mean age was 37.2 ± 10.6 years and 75.2% were female. The histological prevalence of steatosis was 89.5%. The sensitivity and specificity of US in the diagnosis of hepatic steatosis were, respectively, 64.9% (95% CI: 54.9-74.3) and 90.9% (95% CI: 57.1-99.5). The positive predictive value and negative predictive value were, respectively, 98.4% (95% CI: 90.2-99.9) and 23.3% (95% CI: 12.3-39.0). The presence of steatosis onUS was associated to advanced grades of steatosis on histology (P = 0.016). CONCLUSION: Preoperative abdominal US in our series has not shown to be an accurate method for the diagnosis of hepatic steatosis in severe obese patients. 13Endoscopic retrograde cholangiopancreatography drainage for palliation of malignant hilar biliary obstruction——stent-in-stent or side-by-side?A systematic review and meta-analysis显示文摘BACKGROUND Biliary drainage,either by the stent-in-stent(SIS)or side-by-side(SBS)technique,is often required when treating a malignant hilar biliary obstruction(MHBO).Both methods differ from each other and have distinct advantages.AIM To compare both techniques regarding their efficacy and safety in achieving drainage of MHBO.METHODS A comprehensive search of multiple electronic databases(MEDLINE,Embase,LILACS,BIREME,Cochrane)was conducted and grey literature from their inception until December 2020 with no restrictions regarding the year of publication or language,since there was at least an abstract in English.The included studies compared SIS and SBS techniques through endoscopic retrograde cholangiopancreatography.Outcomes analyzed included technical and clinical success,early and late adverse events(AEs),stent patency,reintervention,and procedure-related mortality.RESULTS Four cohort studies and one randomized controlled trial evaluating a total of 250 patients(127 in the SIS group and 123 in the SBS group)were included in this study.There were no statistically significant differences between the two groups concerning the evaluated outcomes,except for stent patency,which was higher in the SIS compared with the SBS technique[mean difference(d)=33.31;95%confidence interval:9.73 to 56.90,I2=45%,P=0.006].CONCLUSION The SIS method showed superior stent patency when compared to SBS for achieving bilateral drainage in MHBO.Both techniques are equivalent in terms of technical success,clinical success,rates of both early and late AEs,reintervention,and procedure-related mortality.Gabriel Mayo Vieira de Souza Igor Braga Ribeiro Mateus Pereira Funari Diogo Turiani Hourneaux de Moura Maria Vitória Cury Vieira Scatimburgo João Remíde Freitas Júnior Sergio A Sánchez-Luna Renato Baracat Eduardo Turiani Hourneaux de Moura Wanderley Marques Bernardo Eduardo Guimarães Hourneaux de Moura 2021World Journal of Hepatology2021,13,5:1

16Litterfall dynamics and soil carbon and nitrogen stocks in the Brazilian palm swamp ecosystems显示文摘Background: This study aimed to determine the litterfall production, accumulation, decomposition rate and nutrient stocks, and to estimate the soil carbon(C) and nitrogen(N) stocks in three palm swamp ecosystems with different land use intensities in the Southeast of Brazil.Methods: Three palm swamp ecosystems with different land use intensities were evaluated: Agua Doce(AD),conserved area;Capivara(CV), area with small agricultural practices;and Buriti Grosso(BG), area with an intensive land use. Produced and accumulated litterfall from the Cerrado areas surrounding the palm swamps were collected from October 2014 to September 2015, and the Ca, Mg, K, and P concentrations were determined in the dry and rainy seasons. Soil samples were collected in the surrounding Cerrado and within the palm swamps until 100 cm soil depth to determine bulk density and soil C and N contents and stocks.Results: Annual litterfall production in the Cerrado surrounding palm swamps was similar in AD(3.58 Mg·ha^-1·year^-1) and CV(3.79 Mg·ha^-1·year^-1), and was lower in BG(2.84 Mg·ha^-1·year^-1), and was more intense during the dry season. Furthermore, litterfall accumulation was higher in CV(7.12 Mg·ha^-1·year^-1) and BG(6.75 Mg·ha^-1·year^-1), culminating in lower decomposition rates. AD showed the highest decomposition rate(0.60) due to its vegetation structure and conservation. The macronutrient contents from the production and accumulated litterfall had a low influence of the land use, decreasing in the following order: Ca > K > Mg > P. Soils in CV and BG palm swamp areas showed higher values of C and N contents, due to the use of land for agricultural crops and cattle raising in an extensive(rangeland) system. The BG palm swamp, showed the highest C(124.03 Mg·ha^-1) and N(10.54 Mg·ha^-1) stocks due to the land use history with different agricultural practices over time.Conclusions: The litterfall dynamics in the Cerrado surrounding palm swamps was more affected by climatic variables than the intensity of land use, but the litter decomposition was more accelerated in the conserved area.Anthropic interventions with soil organic matter(SOM) inputs contributed to an increase in soil C and N stocks in the palm swamp ecosystems.Gracielle de Brito Sales Taynan Aquilles Marinho Lessa Daniela Aparecida Freitas Maria das Dores Magalhães Veloso Maria Ligia de Souza Silva Luiz Arnaldo Fernandes Leidivan Almeida Frazão 2020Forest Ecosystems2020,7,3:1
