| 1 | Targeting the PI3K/Akt signaling pathway in gastric carcinoma: A reality for personalized medicine?显示文摘Frequent activation of phosphatidylinositol-3 kinases(PI3K)/Akt/m TOR signaling pathway in gastric cancer(GC) is gaining immense popularity with identification of mutations and/or amplifications of PIK3 CA gene or loss of function of PTEN,a tumor suppressor protein,to name a few; both playing a crucial role in regulating this pathway. These aberrations result in dysregulation of this pathway eventually leading to gastric oncogenesis,hence,there is a need for targeted therapy for more effective anticancer treatment. Several inhibitors are currently in either preclinical or clinical stages for treatment of solid tumors like GC. With so many inhibitors under development,further studies on predictive biomarkers are needed to measure the specificity of any therapeutic intervention. Herein,we review the common dysregulation of PI3K/Akt/m TOR pathway in GC and the various types of single or dual pathway inhibitors under development that might have a superior role in GC treatment. | 2 | Effects of 42-year long-term fertilizer management on soil phosphorus availability, fractionation,adsorption–desorption isotherm and plant uptake in flooded tropical rice显示文摘Soil phosphorus(P) fractionation, adsorption, and desorption isotherm, and rice yield and P uptake were investigated in flooded tropical rice(Oryza sativa L.) following 42-year fertilizer and manure application. The treatments included low-input [unfertilized control without N, P, or K(C0N0)], farmyard manure(FYM)(C1N0), NP(C0NP), NPK(C0NPK), FYM + NP(C1NP), and high-input treatment, FYM + NPK(C1NPK). Grain yield was increased significantly by 74%over the control under the combined application of FYM + NPK. However, under low- and high-input treatments, yield as well as P uptake was maintained at constant levels for 35 years.During the same period, high yield levels and P uptake were maintained under the C0 NP, C0 NPK,and C1 NPK treatments. These are unique characteristics of a tropical flooded ecosystem, which is a self-sustaining system for rice production. The Fe–P fraction was highest compared to the Ca–P and Al–P fractions after 42 years of fertilizer application and was significantly higher under FYM + NPK treatment. The P adsorption capacity of soil was highest under the low-input treatment and lowest under long-term balanced fertilization(FYM + NPK). In contrast, P desorption capacity was highest under NPK and lowest in the control treatment. | 3 | Garcinol sensitizes human head and neck carcinoma to cisplatin in a xenograft mouse model despite downregulation of proliferative biomarkers显示文摘OBJECTIVE Platinum compounds such as cisplatin and carboplatin are frequently used as the first-line chemotherapy for the treatment of the head and neck squamous cell carcinoma(HNSCC).In the present study,we investigated whether garcinol,apolyisoprenylated benzophenone can chemosensitize HNSCC to cisplatin.METHODS The effect of garcinol and cisplatin on HNSCC was assessed by MTT,Western blotting,real time PCR,FACS,immunohistochemistry,DNA binding assay and xenograft mouse model.RESULTS We found that garcinol inhibited the viability of a panel of diverse HNSCC cell lines,enhanced the apoptotic effect of cisplatin,suppressed constitutive as well as cisplatin-induced NF-κB activation,and downregulated the expression of various oncogenic gene products(cyclin D1,Bcl-2,survivin and VEGF).In vivo study showed that administration of garcinol alone(0.5 mg·kg-1,ip five times/week)significantly suppressed the growth of the tumor,and this effect was further increased by cisplatin.Both the markers of proliferation index(Ki-67)and microvessel density(CD31)were downregulated in tumor tissues by the combination of cisplatin and garcinol.The pharmacokinetic results of garcinol indicated that good systemic exposure was achievable after ip administration of garcinol at 0.5and 2mg·kg-1 with mean peak concentration(cmax)of 1825.4 and 6635.7nmol·L-1 in the mouse serum,respectively.CONCLUSION Overall,our results suggest that garcinol can indeed potentiate the effects of cisplatin by negative regulation of various inflammatory and proliferative biomarkers. | Feng Li Muthu K. | 4 | Impact of gout on in-hospital outcomes of acute coronary syndrome-related hospitalizations and revascularizations: Insights from the national inpatient sample显示文摘BACKGROUND Previous studies have established a role of gout in predicting risk and prognosis of cardiovascular diseases. However, large-scale data on the impact of gout on inpatient outcomes of acute coronary syndrome (ACS)-related hospitalizations and post-revascularization is inadequate. AIM To evaluate the impact of gout on in-hospital outcomes of ACS hospitalizations, subsequent healthcare burden and predictors of post-revascularization inpatient mortality. METHODS We used the national inpatient sample (2010-2014) to identify the ACS and goutrelated hospitalizations, relevant comorbidities, revascularization and postrevascularization outcomes using the ICD-9 CM codes. A multivariable analysis was performed to evaluate the predictors of post-revascularization in-hospital mortality. RESULTS We identified 3144744 ACS-related hospitalizations, of which 105198 (3.35%) also had gout. The ACS-gout cohort were more often older white males with a higher prevalence of comorbidities. Coronary artery bypass grafting was required more often in the ACS-gout cohort. Post-revascularization complications including cardiac (3.2% vs 2.9%), respiratory (3.5% vs 2.9%), and hemorrhage (3.1% vs 2.7%) were higher whereas all-cause mortality was lower (2.2% vs 3.0%) in the ACSgout cohort (P < 0.001). An older age (OR 15.63, CI: 5.51-44.39), non-elective admissions (OR 2.00, CI: 1.44-2.79), lower household income (OR 1.44, CI: 1.17- 1.78), and comorbid conditions predicted higher mortality in ACS-gout cohort undergoing revascularization (P < 0.001). Odds of post-revascularization inhospital mortality were lower in Hispanics (OR 0.45, CI: 0.31-0.67) and Asians (OR 0.65, CI: 0.45-0.94) as compared to white (P < 0.001). However, postoperative complications significantly raised mortality odds. Mean length of stay, transfer to other facilities, and hospital charges were higher in the ACS-gout cohort. | 6 | Do we really need to repair the pronator quadratus after distal radius plating?显示文摘Purpose:Fractures of distal radius are one of the common orthopaedic injuries.Placing the plate on volar surface requires release of underlying pronator quadratus(PQ)muscle.No consensus is present in the literature about the repair or not of the PQ.The purpose of this study was to evaluate the influence of PQ repair on functional outcome and complications.Methods:Retrospectively 83 patients of distal radius fractures managed with volar plating between 2014 and 2016 were evaluated.Demographic data,operative notes and physical therapy records were retrieved.Patients were divided into group A where PQ repair was done and group B where no repair was done.Functional data such as range of motion(ROM),grip strength,visual analogue scale(VAS)score and disabilities of arm,shoulder and hand(DASH)score at 4 weeks,3 months,6 months and finial follow-up were retrieved.Results:Totally 63 patients(n=29 in group A and n=34 in group B)with the mean age of 51.64 years were examined.Patients were followed up for a mean of 35.2 months(range 27.2-47.1 months)in group A and 38.6 months(range 28.6-51.0 months)in group B.Though functional outcome of the affected limb was not significantly different between two groups after 3 months,PQ repair did affect the recovery at an early stage.Repair group had significantly better ROM(p=0.0383)and VAS score at 4 weeks(p=0.017)while grip strength(p=0.014)was significantly better at 3 months.Conclusions:Repair of PQ may provide pain relief and increased ROM in early postoperative period and hence every attempt should be made to achieve the repair. | Subodh Pathak Rashid Anjum Rakesh Kumar Gautam Pritam Maheshwari Jatin Aggarwal Aryan Sharma Vineet Pruthi | 2019 | Chinese Journal of Traumatology2019,22,6: | 3 |

Therefore, evaluation and inferring the data perfectly to prevent fire related accidental risk in underground coal mining (UMC) system are very necessary. In the present article, we have proposed a novel type-2 fuzzy logic system (T2FLS) for the prediction of fire intensity and its risk assessment for risk reduction in an underground coal mine. Recently, for the observation of underground coal mines, wireless underground sensor network (WUSN) are being concerned frequently. To implement this technique IT2FLS, main functional components are sensor nodes which are installed in coal mines to accumulate different imprecise environmental data like, temperature, relative humidity, different gas concentrations etc. and these are sent to a base station which is connected to the ground observation system through network. In the present context, a WUSN based fire monitoring system is developed using fuzzy logic approach to enhance the consistency in decision making system to improve the risk chances of fire during coal mining. We have taken Mamdani IT2FLS as fuzzy model on coal mine monitoring data to consider real-time decision making (DM). It is predicted from the simulated results that the recommended system is highly acceptable and amenable in the case of fire hazard safety with compared to the wired and off-line monitoring system for UMC.
