| 1 | Retrospective analysis of extra-gastrointestinal stromal tumors显示文摘AIM:To investigate the clinicopathologic features of patients with extra-gastrointestinal stromal tumors(EGISTs)in South Korea.METHODS:A total of 51 patients with an EGIST were identified.The clinicopathologic features,including sex,age,location,tumor size,histology,mitotic rate,immunohistochemical features,genetic status and survival data,were analyzed.RESULTS:The median age was 55 years(range:29-80years),and male:female ratio was 1:1.04.The most common site was in the mesentery(n=15)followed by the retroperitoneum(n=13)and omentum(n=8).The median tumor size was 9.0 cm(range:2.6-30.0cm)and the median mitotic rate was 5.0/50HPF.(1/50-185/50).KIT was analyzed in 16,which revealed 10cases with wild-type KIT and 6 cases with an exon 11mutation.Among 51 patients,31 patients had undergone surgery,and 10 had unresectable disease and had taken palliative imatinib,which resulted in 22.7 mo of progression-free survival.Of the patients who had undergone surgery,18 did not take adjuvant imatinib,and 8 of these were categorized as'high risk'according to the risk criteria.However,the relapse-free survival was not different(P=0.157)between two groups.CONCLUSION:Because the biologic behaviors of GISTs differ according to the location of the tumor,a more stratified strategy is required for managing EGISTs including incorporation of molecular features. | Jun Ho Yi Byeong-Bae Park Jung Hun Kang In Gyu Hwang Dong Bok Shin Sun Jin Sym Hee Kyung Ahn Soon Il Lee Do Hyoung Lim Keon Woo Park Young-Woong Won Sung Hee Lim Se Hoon Park | 2015 | World Journal of Gastroenterology2015，21，6： | 9 |
| 2 | Indeterminate pulmonary nodules in colorectal cancer显示文摘AIM:To investigate the clinicopathologic parameters of pulmonary metastasis in colorectal cancer(CRC) patients after lung operation of indeterminate pulmonary nodules(IPNs).METHODS:From a prospective database of CRCpatients,40 cases that underwent lung operation between November 2008 and December 2012 for suspicious metastatic pulmonary nodules on chest computed tomography(CT)were enrolled.The decision to perform a lung operation was made if the patient met the following criteria:(1)completely resected or resectable primary CRC;(2)completely resectable IPNs;(3)controlled or controllable extrapulmonary metastasis;and(4)adequate general condition and pulmonary function to tolerate pulmonary operation.Lung operation was performed by a thoracic surgeon without CT-guided biopsy for pathologic confirmation.RESULTS:A total of 40 cases of lung resection was performed in 29 patients.Five patients underwent repeated lung resection.The final pathology result showed metastasis from the CRC in 30 cases(75%)and benign pathology in 10 cases(25%).The primary tumor site was the rectum in 26/30(86.6%)cases with pulmonary metastasis,but only 3/10(30%)cases in the benign group had a primary rectal cancer(P=0.001).Positron emission tomography(PET)-CT was performed for 22/30(73.4%)patients in the lung metastasis group and for 6/10(60.0%)patients in the benign group.PET-CT revealed hot uptake of18fluorine 2-fluoro-2-deoxy-D-glucose with all IPNs in both groups.The group with pulmonary metastasis had a higher incidence of primary rectal cancer(P=0.001),a more advanced tumor stage(P=0.011),and more frequent lymphatic invasion of tumor cells(P=0.005).Six cases with previous liver metastasectomy were present in the lung metastasis group.Serum carcinoembryonic antigen levels before lung operation were not elevated in any of the patients.CONCLUSION:The stage and location of the primary tumor and tumor cell infiltration of lymphatics provide useful indicators for deciding on lung resection of IPNs in CRC. | Eun-Joo Jung Su-Ran Kim Chun-Geun Ryu Jin Hee Paik Jeong Geun Yi Dae-Yong Hwang | 2015 | World Journal of Gastroenterology2015，21，10： | 5 |
| 3 | Efficacy and Safety of Tofacitinib in Chinese Patients with Rheumatoid Arthritis显示文摘Background:Tofacitinib is an oral Janus kinase inhibitor for the treatment of rheumatoid arthritis (RA).This study assessed the efficacy and safety of tofacitinib in Chinese patients with RA enrolled in Phase 3and long-term extension (LTE)studies. Methods:ORAL Sync was a 1-year,randomized,placebo-controlled,Phase 3trial.Patients received tofacitinib 5 or 10 mg twice daily (BID)or placebo advanced to tofacitinib 5 or 10 mg BID at 3 or 6 months.All patients remained on >1background conventional synthetic disease-modifying antirheumatic drug.ORAL Sequel is an open-label LTE study (data-cut:March 2015;data collection and analyses were ongoing,and study database was not locked at the time of analysis;study was closed in 2017).Efficacy outcomes:American College of Rheumatology (ACR)20/50/70response rates and Disease Activity Score in 28 joints using erythrocyte sedimentation rate (DAS28-4[ESR]).Patient-and physician-reported outcomes:HealthAssessment Questionnaire-Disability Index (HAQ-DI),Patient and Physician Global Assessment of Arthritis,and pain (visual analog scale).Safety was assessed throughout. Results:ORAL Sync included 218 patients;192 were subsequently enrolled into ORAL Sequel.In ORAL Sync,more patients achieved ACR20(tofacitinib 5 mg BID,67.4%;10 mg BID,70.6%;placebo,34.1%)and DAS28-4(ESR)<2.6(tofacitinib 5 mg BID,7.1%; 10mg BID,13.1%;placebo,2,3%)with tofacitinib versus placebo at Month 6.Mean changes from baseline in HAQ-DI were greater with tofacitinib versus placebo at Month 6.In ORAL Sequel,efficacy was consistent to Month 48.Incidence rates for adverse events of special interest in tofacitinib-treated patients were similar to the global population. | 5 | Emerging and re-emerging infectious diseases: Challenges and opportunities for militaries显示文摘The communal nature of living and training environments, alongside suboptimal hygiene and stressors in the field, place military personnel at higher risk of contracting emerging infectious diseases. Some of these diseases spread quickly within ranks resulting in large outbreaks, and personnel deployed are also often immunologically na?ve to otherwise uncommonly-encountered pathogens. Furthermore, the chance of weaponised biological agents being used in conventional warfare or otherwise remains a very real, albeit often veiled, threat. However, such challenges also provide opportunities for the advancement of preventive and therapeutic military medicine, some of which have been later adopted in civilian settings. Some of these include improved surveillance, new vaccines and drugs, better public health interventions and inter-agency co-operations. | 6 | Hypothyroidism affects astrocyte and microglial morphology in type 2 diabetes显示文摘In the present study,we investigated the effects of hypothyroidism on the morphology of astrocytes and microglia in the hippocampus of Zucker diabetic fatty rats and Zucker lean control rats.To induce hypothyroidism,Zucker lean control and Zucker diabetic fatty rats at 7 weeks of age orally received the vehicle or methimazole,an anti-thyroid drug,treatment for 5 weeks and were sacrificed at 12 weeks of age in all groups for blood chemistry and immunohistochemical staining.In the methimazole-treated Zucker lean control and Zucker diabetic fatty rats,the serum circulating triiodothyronine(T3)and thyroxine(T4)levels were significantly decreased compared to levels observed in the vehicle-treated Zucker lean control or Zucker diabetic fatty rats.This reduction was more prominent in the methimazole-treated Zucker diabetic fatty group.Glial fibrillary acidic protein immunoreactive astrocytes and ionized calcium-binding adapter molecule 1(Iba-1)-immunoreactive microglia in the Zucker lean control and Zucker diabetic fatty group were diffusely detected in the hippocampal CA1 region and dentate gyrus.There were no significant differences in the glial fibrillary acidic protein and Iba-1 immunoreactivity in the CA1 region and dentate gyrus between Zucker lean control and Zucker diabetic fatty groups.However,in the methimazole-treated Zucker lean control and Zucker diabetic fatty groups,the processes of glial fibrillary acidic protein immunoreactive astrocytes and Iba-1 immunoreactive microglia,were significantly decreased in both the CA1region and dentate gyrus compared to that in the vehicle-treated Zucker lean control and Zucker diabetic fatty groups.These results suggest that diabetes has no effect on the morphology of astrocytes and microglia and that hypothyroidism during the onset of diabetes prominently reduces the processes of astrocytes and microglia. | Sung Min Nam Yo Na Kim Dae Young Yoo Sun Shin Yi Jung Hoon Choi In Koo Hwang Je Kyung Seong Yeo Sung Yoon | 2013 | Neural Regeneration Research2013，8，26： | 2 |
| 7 | Weekly docetaxel and gemcitabine in previously treated metastatic esophageal squamous cell carcinoma显示文摘AIM: To assess the efficacy and safety of weekly docetaxel plus a fixed-dose rate(FDR) of gemcitabine in metastatic esophageal squamous cell carcinoma(SCC).METHODS: A multi-center, open-label, prospective phase Ⅱ study was designed.Thirty-three esophageal SCC patients with documented progression after fluoropyrimidine/platinum-based first-line chemotherapy were enrolled and treated with docetaxel 35 mg/m2 and gemcitabine 1000 mg/m2 iv at a FDR(10 mg/m2 per minute) on days 1 and 8.Treatment was repeatedevery twenty-one days until disease progression, unacceptable toxicity, or consent withdrawal.The primary endpoint was response rate(RR), and secondary endpoints were safety, progression-free survival(PFS) and overall survival(OS).RESULTS: Combination of weekly docetaxel and FDR gemcitabine was well tolerated: the most common treatment-related adverse events were anemia(97%), fatigue(64%) and neutropenia(55%).One patient with multiple lung and lymph node metastases died of respiratory failure after receiving four cycles of chemotherapy, and the possibility of drug-induced pneumonitis could not be completely excluded.Disease control(objective response plus stable disease) in the ITT population was achieved in 88% of patients, and the overall RR was 30%(95%CI: 15%-46%).The median PFS and OS were 4.0(95%CI: 3.4-4.6) and 8.8 mo(95%CI: 7.8-9.8 mo), respectively.CONCLUSION: A combination of weekly docetaxel and FDR gemcitabine showed promising antitumor activity and tolerability in previously treated, metastatic esophageal SCC. | Min-Young Lee Ki Sun Jung Hae Su Kim Ji Yun Lee Sung Hee Lim Moonjin Kim Hyun Ae Jung Sung Min Kim Jong Mu Sun Myung-Ju Ahn Jeeyun Lee Se Hoon Park Seong Yoon Yi In Gyu Hwang Sang-Cheol Lee Hee Kyung Ahn Do Hyoung Lim Soon Il Lee Keon Woo Park | 2015 | World Journal of Gastroenterology2015，21，14： | 2 |
