1Submucosal tunneling and endoscopic resection of submucosal tumors at the esophagogastric junction显示文摘AIM: To evaluate the safety and efficacy of submucosal tunneling and endoscopic resection(STER) for treating submucosal tumors(SMTs).METHODS: Between August 2012 and October 2013, 21 patients with SMTs originating from the muscularis propria(MP) layer at the esophagogastric junction were treated by STER of their tumors. Key steps of the procedure include:(1) mucosal incision: a 2-cm longitudinal mucosal incision was made 5 cm proximal to the tumor;(2) submucosal tunneling: a submucosal tunnel was created 5 cm proximal to and 1 to 2 cm distal to the tumor;(3) tumor resection: the SMT was resected under direct endoscopic viewing;(4) hemostasis: while finishing the tumor resection, careful hemostasis of the MP defect and the tunnel was performed; and(5) mucosal closure: the mucosal incision site was closed by using hemostatic clips. During the operation, equipment used included a cap-fitted endoscope, an insulatedtip knife, a hook knife, hemostatic forceps, an injection needle, a snare, an endoclip, and a high-frequency generator. Carbon dioxide(CO2) insufflation was achieved by using a CO2 insufflator.RESULTS: The median age of the patients was 46.2 years(range, 35-59 years), and the majority were male(18 male vs 3 female). Complete resection rate was 100%(21/21). Eighteen lesions were resected en bloc. Mean tumor size was 23 mm(range, 10-40 mm), and mean procedure time was 62.9 min(range, 45-90 min). Pathological diagnosis of these tumors included leiomyoma(15 out of 21) and gastrointestinal stromal tumor(6 out of 21). Full-thickness MP resection was performed in 9 of 21 patients(42.9%), with mediastinal and subcutaneous emphysema occurring in all nine. At the completion of the procedure, all patients received closure of the incision with hemoclips. One patient required percutaneous drainage. The remaining 20patients required no further endoscopic or surgical intervention. There were no incidents of massive or delayed bleeding. The median follow-up period after the procedure was 6 mo(range, 2-14 mo). 6Hematological disorders and pulmonary hypertension显示文摘Pulmonary hypertension(PH),a serious disorder with a high morbidity and mortality rate,is known to occur in a number of unrelated systemic diseases.Several hematological disorders such as sickle cell disease,thalassemia and myeloproliferative diseases develop PH which worsens the prognosis.Associated oxidant injury and vascular inflammation cause endothelial damage and dysfunction.Pulmonary vascular endothelial damage/dysfunction is an early event in PH resulting in the loss of vascular reactivity,activation of proliferative and antiapoptotic pathways leading to vascular remodeling,elevated pulmonary artery pressure,right ventricular hypertrophy and premature death.Hemolysis observed in hematological disorders leads to free hemoglobin which rapidly scavenges nitric oxide(NO),limiting its bioavailability,and leading to endothelial dysfunction.In addition,hemolysis releases arginase into the circulation which converts L-arginine to ornithine,thus bypassing NO production.Furthermore,treatments for hematological disorders such as immunosuppressive therapy,splenectomy,bone marrow transplantation,and radiation have been shown to contribute to the development of PH.Recent studies have shown deregulated iron homeostasis in patients with cardiopulmonary diseases including pulmonary arterial hypertension(PAH).Several studies have reported low iron levels in patients with idiopathic PAH,and iron deficiency is an important risk factor.This article reviews PH associated with hematological disorders and its mechanism:and iron homeostasis and its relevance to PH.Rajamma Mathew Jing Huang Joseph M Wu John T Fallon Michael H Gewitz 2016World Journal of Cardiology2016,8,12:6
7Modeling contaminant transport in homogeneous porous media with fractional advection-dispersion equation显示文摘The newly developed Fractional Advection-Dispersion Equation (FADE), which is FADE was extended and used in this paper for modelling adsorbing contaminant transport by adding an adsorbing term. A parameter estimation method and its corresponding FORTRAN based program named FADEMain were developed on the basis of Nonlinear Least Square Algorithm and the analytical solution for one-dimensional FADE under the conditions of step input and steady state flow. Data sets of adsorbing contaminants Cd and NH4+-N transport in short homogeneous soil columns and conservative solute NaCI transport in a long homogeneous soil column, respectively were used to estimate the transport parameters both by FADEMain and the advection-dispersion equation (ADE) based program CXTFIT2.1. Results indicated that the concentration simulated by FADE agreed well with the measured data. Compared to the ADE model, FADE can provide better simulation for the concentration in the initial lower concentration part and the late higher concentration part of the breakthrough curves for both adsorbing contaminants. The dispersion coefficients for ADE were from 0.13 to 7.06 cm2/min, while the dispersion coefficients for FADE ranged from 0.119 to 3.05 cm1.856/min for NaCI transport in the long homogeneous soil column. We found that the dispersion coefficient of FADE increased with the transport distance, and the relationship between them can be quantified with an exponential function. 10Blood DNA methylation markers in prospectively identifiedhepatocellular carcinoma cases and controls from Taiwan显示文摘AIM: To determine if gene-specific DNA methylation in prospectively collected blood samples is associated with later development of hepatocellular carcinoma(HCC).METHODS: Comparing genome-wide DNA methylation profiles using Illumina Human methylation 450 K arrays, we previously identified a list of loci that were differentially methylated between tumor and adjacent nontumor tissues. To examine if dysregulation of DNAmethylation patterns observed in tumor tissues can be detected in white blood cell(WBC) DNA, we conducted a prospective case-control study nested within a community-based cancer screening cohort in Taiwan with 16 years of follow up. We measured methylation levels in ninety-six loci that were aberrant in DNA methylation in HCC tumor tissues compared to adjacent tissues. Baseline WBC DNA from 159 HCC cases and 312 matched controls were bisulfite treated and assayed by Illumina Bead Array. We used the χ2 test for categorical variables and student's t-test for continuous variables to assess the difference in selected characteristics between cases and controls. To estimate associations with HCC risk, we used conditional logistic regression models stratified on the matching factors to calculate odds ratios(OR) and 95%CI. RESULTS: We found that high methylation level in cg10272601 in WNK2 was associated with increased risk of HCC, with an OR of 1.91(95%CI: 1.27-2.86). High methylation levels in both cg12680131 in TPO and cg22511877 in MYT1 L, however, were associated with decreased risk. The ORs(95%CI) were 0.59(0.39-0.87) and 0.50(0.33-0.77), respectively, for those with methylation levels of cg12680131 and cg22511877 above the median compared with those with levels below the median. These associations were still statistically significant in multivariable conditional logistic regression models after adjusting for hepatitis B virus infection and alcohol consumption.
