| 3 | A national report from China Liver Transplant Registry: steroid avoidance after liver transplantation for hepatocellular carcinoma显示文摘Objective:We aimed to evaluate the efficacy and safety of steroid-free immunosuppression after liver transplantation(LT)for hepatocellular carcinoma(HCC).Methods:We retrospectively analyzed HCC recipients without steroids after LT(SF group,n=368)based on the China Liver Transplant Registry(CLTR)database.These recipients were matched 1:2 with patients using steroids(S group,n=736)for the same period after LT for HCC,according to propensity scores.Results:Multivariate analysis indicates that recipients with younger age[odds ratio(OR),1.053;P=0.011],preoperative hepatitis B virus(HBV)DNA≥1,000 copies/mL(OR,2.597;P=0.004)and beyond Milan criteria(OR,4.255;P<0.001)were identified as the risk factors associated with tumor recurrence in steroid avoidance recipients after LT.The patients fulfilling the Milan criteria in the SF group presented higher overall and tumor-free survival rates than those in the S group(P<0.05).Multivariate analysis revealed that recipient beyond Milan criteria was an independent prognostic factor for overall survival(OR,1.690;P<0.001)and tumor-free survival(OR,2.066;P<0.001).The incidences of new-onset diabetes mellitus(21.20% vs.33.29%,P<0.001),new-onset hypertension(10.05%vs.18.61%,P<0.001)and hyperlipidemia(4.08% vs.7.20%,P=0.042)were significantly lower in the SF group.Conclusions:Steroid-free immunosuppression could be safe and feasible for HBV-related HCC patients in LT.Age,HBV DNA level and Milan criteria maybe risk factors associated with tumor recurrence in steroid avoidance recipients.Recipient beyond Milan criteria was an independent prognostic factor and recipient fulfilling Milan criteria can benefit the most from steroid-free immunosuppression. | Qiang Wei Feng Gao Runzhou Zhuang Qi Ling Qinghong Ke Jian Wu Tian Shen Mangli Zhang Min Zhang Xiao Xu Shusen Zheng | 2017 | Chinese Journal of Cancer Research2017,29,5: | 25 |
| 4 | Intracoronary arterial retrograde thrombolysis with percutaneous coronary intervention: a novel use of thrombolytic to treat acute ST-segment elevation myocardial infarction显示文摘Background Clearance of coronary arterial thrombosis is necessary in patients with acute ST-segment elevation myocardial infarction (STEMI) undergoing urgent percutaneous coronary intervention (PCI). There is currently no highly-recommended method of thrombus removal during interventional procedures. We describe a new method for opening culprit vessels to treat STEMI: intracoronary arterial retrograde thrombolysis (ICART) with PCI. Methods & Results Eight patients underwent ICART. The guidewire was advanced to the distal coronary artery through the occlusion lesion. Then, we inserted a microcatheter into the distal end of the occluded coronary artery over the guidewire. Urokinase (5–10 wu) mixed with contrast agents was slowly injected into the occluded section of the coronary artery through the microcatheter. The intracoronary thrombus gradually dissolved in 3–17 min, and the effect of thrombolysis was visible in real time. Stents were then implanted according to the characteristics of the recanalized culprit lesion to achieve full revascularization. One patient experienced premature ventricular contraction during vascular revascularization, and no malignant arrhythmias were seen in any patient. No reflow or slow flow was not observed post PCI. Thrombolysis in myocardial infarction flow grade and myocardial blush grade post-primary PCI was 3 in all eight patients. No patients experienced bleeding or stroke. Conclusions ICART was accurate and effective for treating intracoronary thrombi in patients with STEMI in this preliminary study. ICART was an effective, feasible, and simple approach to the management of STEMI, and no intraprocedural complications occurred in any of the patients. | 7 | Observation of atmospheric nitrous acid with DOAS in Beijing, China显示文摘Measurements of nitrous acid(HONO) and nitrogen dioxide(NO2) in Beijing City have been performed by means of a developed differential optical absorption spectroscopy(DOAS) system based on photodiode array(PDA), during the autumn of 2004. HONO and NO2 were simultaneously identified by their characteristic absorption bands in the spectral region between 337 nm and 372 nm with high sensibility and time resolution. The concentrations of HONO exhibit obviously diurnal variation with a nocturnal maximum and a daytime minimum. The highest HONO value up to 11.8 ?滋g/m3 was observed during the night of 2/3 September. Possible sources of the observed HONO were discussed. Good correlation to NO2 indicates that NO2 is a main source component. | 8 | S-1-based vs non-S-1-based chemotherapy in advanced gastric cancer: A meta-analysis显示文摘AIM: To assess the efficacy and tolerability of S-1-based vs non-S-1-based chemotherapy in advanced gastric cancer(AGC). METHODS: We extracted reported endpoints, including overall survival(OS), progression-free survival(PFS), time-to-treatment failure(TTF), objective response rate(ORR) and adverse effects, from randomized controlled trials identified in PubMed, the Cochrane library, Science Direct, EMBASE and American Society of Clinical Oncology meetings. Stata software was used to calculate the pooled values.RESULTS: Seven randomized controlled trials involving 2176 patients were included in this meta-analysis. Compared to non-S-1-based regimens, the use of S-1-based regimens were associated with an increase in ORR(RR = 1.300; 95%CI: 1.028-1.645); OS(HR = 0.89; 95%CI: 0.81-0.99; P = 0.025), TTF(HR = 0.83; 95%CI: 0.75-0.92; P = 0.000), and a lower risk of febrile neutropenia(RR = 0.225; P = 0.000) and stomatitis(RR = 0.230; P = 0.032). OS, PFS and TTFwere prolonged, especially in the Asian population. In subgroup analysis, statistically significant increases in ORR(RR = 1.454; P = 0.029), OS(HR = 0.895; P = 0.041) and TTF(HR = 0.832; P = 0.000) were found when S-1-based chemotherapy was compared to 5-fluorouracil(5-FU)-based chemotherapy. The incidence of leukopenia(RR = 0.584; P = 0.002) and stomatitis(RR = 0.230; P = 0.032) was higher in the 5-FU-based arm. S-1-based regimens had no advantage in ORR, OS, PFS, TTF and grade 3 or 4 adverse events over capecitabine-based regimens. METHODS: Using stratified random sampling according to fibrinogen (Fib) levels,145 liver cirrhosis patients due to hepatitis B complicated by coagulation disorder were treated. Of them,70 in research group were treated with TCM by 'nourishing yin,cooling blood and invigorating blood circulation' and Western medicine,75 in control group were treated with conventional Western medicine. The indexes of liver function,coagulation function and bleeding events were observed and compared. RESULTS: The prothrombin time (PT) was shorter and the fibrinogen (Fib) level was higher in the research group than in the control group (Fib = 1.6-2.0 g/L,1.1-1.5 g/L,and ≤ 1.0 g/L). The total bilirubin (TBIL) level was significantly lower in the research group than in the control group,except for the subgroup of FIB ≤ 1.0 g/L. | 12 | Inhibitory Activity of Honeysuckle Extracts against Influenza A Virus In Vitro and In Vivo显示文摘Honeysuckle has been used in the treatment of influenza virus infection for thousands of years in China.However,its main active components and the functional mechanisms remain to be elucidated.Here,four honeysuckle extracts,including acids extract,flavonoids extract,total extract and acids-flavonoids mixture,were prepared to clarify the main active antiviral components.The cytopathic effect reduction assay showed that all the four extracts inhibited the replication of influenza viruses H1N1,H3N2 and the oseltamivir-resistant mutant strain H1N1-H275Y.The acids-flavonoids mixture had the strongest inhibitory effects in vitro with EC_(50) values of 3.8,4.1,and >20 lg/m L against H1N1,H3N2 and H1N1-H275Y,respectively,showing competitive antiviral activity with oseltamivir and ribavirin.Honeysuckle acids extract also showed the most significant antiviral activity in vivo.Oral administration of the acids extract at a dosage of 600 mg/kg/d effectively alleviated viral pneumonia,maintained body weight and improved the survival rate to 30% of the mice infected with a lethal dose of H1N1.The results of time-of-drug addition experiment and neuraminidase (NA) inhibition assay showed that honeysuckle extracts had a broad-spectrum inhibitory effect against influenza virus NAs.The flavonoid extract showed the strongest inhibitory effect on the NA of influenza virus H7N9 with an IC_(50) of 24.7 lg/m L.These results suggested that these extracts might exert their antiviral activity by suppressing the release of influenza viruses.Briefly,our findings demonstrate that acids and flavonoids extracts of honeysuckle are the major antiviral active components,and the acids extract has the potential to be developed into an antiviral agent against influenza virus,especially for oseltamivirresistant viruses. | Mengwei Li Yuxu Wang Jing Jin Jie Dou Qinglong Guo Xue Ke Changlin Zhou Min Guo | 2021 | Virologica Sinica2021,36,3: | 11 |

Therefore, the aim of this study was to use qualitative research methods exploring the strengths, weaknesses, opportunities and threat factors that could facilitate or impede individuals' abstinence in a real-life context from the perspectives of MA abusers. Aim To use qualitative research methods exploring the strengths, weaknesses, opportunities and threat factors relating to individuals' abstinence in a real-life context. Method Semistructured interviews were conducted with 32 MA abusers recruited in Shanghai guided by open-ended questions on narrating the real-life catalysts and inhibitors related to craving, initiation, relapse and abstinence. All data were transcribed verbatim and analysed using a strengths, weaknesses, opportunities and threats (SWOT) analysis. Results The results of the SWOT analysis revealed that contextual factors including peer influence, prevalence and availability of MA, familiar venue of MA use, discrimination, sexual behaviours, alcohol, emotional states and their attitudes towards smoking MA were important factors that contribute to reinitiation and relapse. Surveillance systems, antidrug social workers, vocational skills trainings, moving to another city and family responsibility might serve as counter measures targeti叩 those mentioned weaknesses and threats above. Conclusion This SWOT analysis highlights the complex nature of relapse.
