| 1 | Sclerosing epithelioid fibrosarcoma of the liver infiltrating the inferior vena cava显示文摘Sclerosing epithelioid fibrosarcoma (SEF) is a rare and distinct variant of fibrosarcoma, composed of epithelioid tumor cells arranged in strands, nests, cords, or sheets embedded within a sclerotic collagenous matrix. We report a 39-year-old man with SEF of the liver, which infiltrated the inferior vena cava (IVC). The SEF of the liver was successfully resected, and the infiltrated IVC was also removed together with the liver tumor. Histopathological examination of the tumor showed typical histopathology of SEF. Immunohistochemically, the tumor was positive for vimentin. Recurrence was noted 7 mo after surgery. After chemotherapy, the recurrent tumor was resected surgically, and histopathological examination showed similar findingsto those of the primary tumor. | 2 | Computed tomography attenuation values of ascites are helpful to predict perforation site显示文摘AIM:To evaluate the effect of computed tomography(CT) attenuation values of ascites on gastrointestinal(GI) perforation site prediction.METHODS:The CT attenuation values of the ascites from 51 patients with GI perforations were measured by volume rendering to calculate the mean values.The effect of the CT attenuation values of the ascites on perforation site prediction and postoperative complications was evaluated.RESULTS:Of 24 patients with colorectal perforations,the CT attenuation values of ascites were significantly higher than those in patients with perforations at other sites [22.5 Hounsfield units(HU) vs 16.5 HU,respectively,P = 0.006].Colorectal perforation was significantly associated with postoperative complications(P = 0.038).The prediction rate of colorectal perforation using attenuation values as an auxiliary diagnosis improved by 9.8% compared to that of CT findings alone(92.2% vs 82.4%).CONCLUSION:The CT attenuation values of ascites could facilitate the prediction of perforation sites and postoperative complications in GI perforations,particularly in cases in which the perforation sites are difficult to predict by CT findings alone. | Ryo Seishima Koji Okabayashi Hirotoshi Hasegawa Masashi Tsuruta Hiroki Hoshino Toru Yamada Yuko Kitagawa | 2015 | World Journal of Gastroenterology2015，21，5： | 3 |

| 15 | Recent evidence for subcutaneous drains to prevent surgical site infections after abdominal surgery:A systematic review and metaanalysis显示文摘BACKGROUND Surgical site infections(SSIs)increase mortality,hospital stays,additional medical treatment,and medical costs.Subcutaneous drains prevent SSIs in gynecological and breast surgeries;however,their clinical impact in abdominal surgery remains unclear.AIM To investigate whether subcutaneous drains were beneficial in abdominal surgery using a systematic review and meta-analysis.METHODS The database search used PubMed,MEDLINE,and the Cochrane Library.The following inclusion criteria were set for the systematic review:(1)Randomized controlled trial studies comparing SSIs after abdominal surgery with or without subcutaneous drains;and(2)Studies that described clinical outcomes,such as SSIs,seroma formation,the length of hospital stays,and mortality.RESULTS Eight studies were included in this meta-analysis.The rate of total SSIs was significantly lower in the drained group(54/771,7.0%)than in the control group(89/759,11.7%),particularly in gastrointestinal surgery.Furthermore,the rate of superficial SSIs was slightly lower in the drained group(31/517,6.0%)than in the control group(49/521,9.4%).No significant differences were observed in seroma formation between the groups.Hospital stays were shorter in the drained group than in the control group.CONCLUSION Subcutaneous drains after abdominal surgery prevented SSIs and reduced hospital stays but did not significantly affect seroma formation.The timing of drain removal needs to be reconsidered in future studies. | Tomohiro Ishinuki Hiroji Shinkawa Keita Kouzu Seiichi Shinji Erika Goda Toshio Ohyanagi Masahiro Kobayashi Motomu Kobayashi Katsunori Suzuki Yuichi Kitagawa Chizuru Yamashita Yasuhiko Mohri Junzo Shimizu Motoi Uchino Seiji Haji Masahiro Yoshida Hiroki Ohge Toshihiko Mayumi Toru Mizuguchi | 2023 | World Journal of Gastrointestinal Surgery2023，15，12： | 0 |
