| 4 | Systematic review of dynamization vs exchange nailing for delayed/non-union femoral fractures显示文摘AIM To analyze the literature on efficacy of dynamamization vs exchange nailing in treatment of delayed and nonunion femur fractures.METHODS Ultimately, 31 peer-reviewed articles with 644 exchanged nailing patients and 131 dynamization patients were identified and analyzed. The following key words were inputted in different combinations in order to search the field of publications in its entirety: 'nonunion', 'delayed union', 'ununited', 'femur fracture', 'femoral fracture', 'exchange nailing', 'dynaiz(s)ation', 'secondary nailing', 'dynamic', 'static', and 'nail revision'. The initial search yielded over 150 results, and was refined based on the inclusion criteria: Only studies reporting on humans, non-unions and delayed unions, and the usage of exchange nailing and/or dynamization as a secondary treatment after failed IM nailing. The resulting 66 articles were obtained through online journal access. The results were filtered further based on the exclusion criteria: No articles that failed to report overall union rates, differentiate between success rates of their reported techniques, or articles that analyzed less than 5 patients. RESULTS Exchange nailing lead to fracture union in 84.785% of patients compared to the 66.412% of dynamization with statistically comparable durations until union(5.193 ± 2.310 mo and 4.769 ± 1.986 mo respectively). Dynamically locking exchange nails resulted in an average union time of 5.208 ± 2.475 mo compared to 5.149 ± 2.366 mo(P = 0.8682) in statically locked exchange nails. The overall union rate of the two procedures, statically and dynamically locked exchange nailing yielded union rates of 84.259% and 82.381% respectively. Therefore, there was no significant difference between the different locking methods of exchange nailing for union rate or time to union at a significance value of P < 0.05. The analysis showed exchange nailing to be the more successful choice in the treatment of femoral non-unions in respect to its higher success rate(491/567 EN, 24/57 dynam, P < 0.0001). However, there was no significant difference between the success rates of the two procedures for delayed union fractures(25/27 EN, 45/55 dynam, P = 0.3299). | 6 | Effects of atmospheric circulation on summertime precipitation variability and glacier mass balance over the Tuyuksu Glacier in Tianshan Mountains, Kazakhstan显示文摘The amount and the form of precipitation have significant effects on glacier mass balances in high altitude mountain areas by controlling the accumulation, the ablation and the energy balance of a glacier through impact on the surface albedo. The liquid precipitation has negative effects on glacier accumulation and may increase the ablation of surface ice through the heat input for melting. The timing and the forms of precipitation over glacierized regions depend on the weather processes both locally and regionally. Early studies showed that regional to large-scale atmospheric circulation processes play a key role in affecting the precipitation events over glaciers. This paper analyzed the relationship between the inter-annual variability of the summertime precipitation over the Tuyuksu Glacier and the atmospheric circulation types, which related to various atmospheric circulation types in the Northern Hemisphere. Results indicated that the decrease in the duration of zonal processes and the increase in the meridional northern processes were observed in the last decade. The total summer precipitation associated with these processes also increased along with an increase of summertime solid precipitation. | 9 | The clinical effectiveness of reflectance optical spectroscopy for the in vivo diagnosis of oral lesions显示文摘Optical spectroscopy devices are being developed and tested for the screening and diagnosis of oral precancer and cancer lesions. This study reports a device that uses white light for detection of suspicious lesions and green–amber light at 545 nm that detect tissue vascularity on patients with several suspicious oral lesions. The clinical grading of vascularity was compared to the histological grading of the biopsied lesions using specific biomarkers. Such a device, in the hands of dentists and other health professionals, could greatly increase the number of oral cancerous lesions detected in early phase. The purpose of this study is to correlate the clinical grading of tissue vascularity in several oral suspicious lesions using the IdentafiH system with the histological grading of the biopsied lesions using specific vascular markers. Twenty-one patients with various oral lesions were enrolled in the study. The lesions were visualized using IdentafiH device with white light illumination, followed by visualization of tissue autofluorescence and tissue reflectance. Tissue biopsied was obtained from the all lesions and both histopathological and immunohistochemical studies using a vascular endothelial biomarker(CD34) were performed on these tissue samples. The clinical vascular grading using the green–amber light at 545 nm and the expression pattern and intensity of staining for CD34 in the different biopsies varied depending on lesions, grading ranged from 1 to3. The increase in vascularity was observed in abnormal tissues when compared to normal mucosa, but this increase was not limited to carcinoma only as hyperkeratosis and other oral diseases, such as lichen planus, also showed increase in vascularity. Recently, a platform technique of transfer photolithography was invented to fabricate stable metal microarrays on the surface of a cell-adhesion resistant and mechanically biomimetic poly(ethylene glycol) hydrogel; the linker is the key chemical in such a transfer strategy. This article reports the design and synthesis of a hetero-bifunctional macromonomer linker with a thiol group at one end and an acryloyl group at the other end. The bifunctional linker was characterized by GPC and 1H NMR, and the average number of thiol groups in the bifunctional linker was detected by Ellman's reagent. The regent stability under wet conditions was also confirmed by the model reactants. The material is self healing in such a sense that some external stimulus is needed in the form of heating by induction energy. Steel wool was added to porous asphalt concrete to enhance its electrical conductivity and induction heating was applied to increase the temperature to heal the micro-cracks and repair the bonding between aggregate and binder. The main purpose of this paper is to examine the mechanical properties of this sustainable porous asphalt concrete including indirect tensile strength,work of fracture,particle loss resistance and water sensitivity.
