| 1 | Diagnostic delay and associated factors among patients with pulmonary tuberculosis in Dar es Salaam,Tanzania显示文摘Background:Tanzania is among the 30 countries with the highest tuberculosis(TB)burdens.Because TB has a long infectious period,early diagnosis is not only important for reducing transmission,but also for improving treatment outcomes.We assessed diagnostic delay and associated factors among infectious TB patients.Methods:We interviewed new smear-positive adult pulmonary TB patients enrolled in an ongoing TB cohort study in Dar es Salaam,Tanzania,between November 2013 and June 2015.TB patients were interviewed to collect information on socio-demographics,socio-economic status,health-seeking behaviour,and residential geocodes.We categorized diagnostic delay into≤3 or>3 weeks.We used logistic regression models to identify risk factors for diagnostic delay,presented as crude(OR)and adjusted Odds Ratios(aOR).We also assessed association between geographical distance(incremental increase of 500 meters between household and the nearest pharmacy)with binary outcomes.Results:We analysed 513 patients with a median age of 34 years(interquartile range 27-41);353(69%)were men.Overall,444(87%)reported seeking care from health care providers prior to TB diagnosis,of whom 211(48%)sought care>2 times.Only six(1%)visited traditional healers before TB diagnosis.Diagnostic delay was positively associated with absence of chest pain(aOR=7.97,95%confidence intervals[CI]:3.15-20.19;P<0.001),and presence of hemoptysis(aOR=25.37,95%CI:11.15-57.74;P<0.001)and negatively associated with use of medication prior to TB diagnosis(aOR=0.31,95%CI:0.14-0.71;P=0.01).Age,sex,HIV status,education level,household income,and visiting health care facilities(HCFs)were not associated with diagnostic delay.Patients living far from pharmacies were less likely to visit a HCF(incremental increase of distance versus visit to any facility:OR=0.51,95%CI:0.28-0.96;P=0.037).Conclusions:TB diagnostic delay was common in Dar es Salaam,and was more likely among patients without prior use of medication and presenting with hemoptysis.Geographical distance to HCFs may have an impact on health-seeking behaviour.Increasing community awareness of TB signs and symptoms could further reduce diagnostic delays and interrupt TB transmission. | Khadija Said Jerry Hella Grace Mhalu Mary Chiryankubi Edward Masika Thomas Maroa Francis Mhimbira Neema Kapalata Lukas Fenner | 2017 | Infectious Diseases of Poverty2017,6,1: | 4 |

| 4 | Genetic Evidence of Middle East Respiratory Syndrome Coronavirus(MERS-Cov) and Widespread Seroprevalence among Camels in Kenya显示文摘We describe the first genome isolation of Middle East respiratory syndrome coronavirus(MERS-CoV) in Kenya. This fatal zoonotic pathogen was first described in the Kingdom of Saudi Arabia in 2012. Epidemiological and molecular evidence revealed zoonotic transmission from camels to humans and between humans. Currently, MERS-CoV is classified by the WHO as having high pandemic potential requiring greater surveillance. Previous studies of MERS-CoV in Kenya mainly focused on site-specific and archived camel and human serum samples for antibodies. We conducted active nationwide cross-sectional surveillance of camels and humans in Kenya, targeting both nasal swabs and plasma samples from 1,163 camels and 486 humans collected from January 2016 to June 2018. A total of 792 camel plasma samples were positive by ELISA. Seroprevalence increased with age, and the highest prevalence was observed in adult camels(82.37%, 95%confidence interval(CI) 79.50–84.91). More female camels were significantly seropositive(74.28%, 95% CI 71.14–77.19)than male camels(P \ 0.001)(53.74%, 95% CI 48.48–58.90). Only 11 camel nasal swabs were positive for MERS-CoV by reverse transcription-quantitative PCR. Phylogenetic analysis of whole genome sequences showed that Kenyan MERSCoV clustered within sub-clade C2, which is associated with the African clade, but did not contain signature deletions of orf4 b in African viruses. None of the human plasma screened contained neutralizing antibodies against MERS-CoV. This study confirms the geographically widespread occurrence of MERS-CoV in Kenyan camels. | 11 | Long Term Perinatal Deltamethrin Exposure Alters Electrophysiological Properties of Embryonic Ventricular Cardiomyocyte显示文摘Increased use of pyrethroids and the exposure to pyrethroids for pregnant women and children have raised the concerns over the potential effect of pyrethroids on developmental cardiotoxicity and other abnormalities.The purpose of this study was to investigate whether long tenn peri natal deltamethrin exposure altered embryonic cardiac electrophysiology in mice.Pregnant mice were administered with 0 or 3 mg/kg of deltamethrin by gavage daily from gestational day(gd)10.5 to gd 17.5.Whole cell patch-clamp technique was used in electrophysiological study,and real time RT-PCR was applied to analyze the molecular changes for the electrophysiological properties.Deltamethrin exposure resulted in increased mortality of pregnant mice and decreased viability of embryos.Moreover,deltamethrin slowed the maximum depolarization velocity(Vmax),prolonged the action potential duration(APD)and depolarized the maximuin diastolic potential(MDP)of embryonic cardiomyocytes.Additionally,perinatal deltamethrin exposure decreased the mRNA expression of Na^+channel regulatory subunit Navpl,inward rectifier K^+channel subunit Kir2.1,and delayed rectifier K^+channel subunit MERG while the L-type Ca^2+channel subunit,Cavl.2 expression was increased.On the contraiy,deltamethrin administration did not significantly alter the regulation ofβ-adrenergic or muscarinic receptor on embryonic cardiomyocytes.In conclusion,deltamethrin exposure at perinatal stage significantly alters mRNA expression of embryonic cardiac ion channels and therefore influences embryonic cardiac electrophysiological properties.This highlights the need to understand the persistent effects of pyrethroid exposure on cardiac function during embryonic development due to potential for cardiac arrhythmogenicity. | Hong-yan LUO Jacob Masika Xiu-wen Guan Li NIE Dong-hui AO Yu QI Rui SHI Jurgen Hescheler Ying ZENG | 2019 | Current Medical Science2019,39,1: | 1 |

| 14 | Preparation of ultrahigh surface area porous carbons templated using zeolite 13X for enhanced hydrogen storage显示文摘In this report,the use of zeolite 13X as a template to generate ultrahigh surface area carbons,via a two-step process combining liquid impregnation and chemical vapour deposition is explored.The first step in the nanocasting process involves impregnation of zeolite 13X with furfuryl alcohol and the second step consists of chemical vapour deposition(CVD) of ethylene at 700 1C.Zeolite-like structural ordering was achieved for zeolite templated carbons(ZTCs) prepared at variable heating ramp rates of 5,10 or 15 1C/min.The textural properties of ZTCs prepared at all heating ramp rates were comparable with small variations in which the lowest ramp rate(5 1C/min) generated ZTC with highest surface area and pore volume of 3332 m 2 /g and 1.66 cm 3 /g respectively.The carbon materials achieved a remarkable hydrogen uptake of 7.3 wt% at 20 bar and 77 K which is the highest ever recorded for carbon materials.This report also explores the mechanical stability of the ZTCs via compaction at up to 10t(equivalent to 740 MPa) in which the compacted samples showed minimal modification and retained high hydrogen storage capacity. | Eric Masika Robert Mokaya | 2013 | Progress in Natural Science:Materials International2013,23,3: | 1 |
