| 1 | Effects of polydispersity on the micro–macro behavior of granular assemblies under different deformation paths显示文摘The micromechanical and macromechanical behavior of idealized granular assemblies, made by linearly elastic, frictionless, polydisperse spheres, are studied in a periodic, triaxial box geometry, using the discrete element method. Emphasis is put on the effect of polydispersity under purely isotropic loading and unloading, deviatoric(volume conserving), and uniaxial compression paths.We show that scaled pressure, coordination number and fraction of rattlers behave in a very similar fashion as functions of volume fraction, irrespective of the deformation path applied. Interestingly, they show a systematic dependence on the deformation mode and polydispersity via the respective jamming volume fraction. This confirms that the concept of a single jamming point has to be rephrased to a range of variable jamming points, dependent on microstructure and history of the sample, making the jamming volume fraction a state-variable.This behavior is confirmed when a simplified constitutive model involving structural anisotropy is calibrated using the purely isotropic and deviatoric simulations. The basic model parameters are found to depend on the polydispersity of the sample through the different jamming volume fractions. The predictive power of the calibrated model is checked by comparison with an independent test, namely uniaxial compression. | 3 | Triple tube drainage for “difficult” gastroduodenal perforations:A prospective study显示文摘AIM To prospectively study the outcome of difficult gastroduodenal perforations(GDPs) treated by triple tube drainage(TTD) in order to standardize the procedure.METHODS Patients presenting to a single surgical unit of a tertiary hospital with difficult GDPs(large, unfavourable local and systemic factors) were treated with TTD(gastrostomy, duodenostomy and feeding jejunostomy). Postoperative parameters were observed like time to return of bowel sounds, time to start enteral feeds, time to start oral feeds, daily output of all drains, time to clamping/removal of all drains, time for skin to heal, complications, hospital stay, and, mortality. Descriptive statistics were used. RESULTS Between December 2013 and April 2015, 20 patients undergoing TTD for GDP were included, with mean age of 44.6 ± 19.8 years and male:female ratio of 17:3. Mean pre-operative APACHE Ⅱ scores were 10.85 ± 3.55; most GDPs were prepyloric(9/20; 45%) or proximal duodenal(8/20; 40%) and mean size was 1.83 ± 0.59 cm(largest 2.5 cm). Median times of resumption of enteral feeding, removal of gastrostomy, removal of duodenostomy,removal of feeding jejunostomy and oral feeding were 4 d(4-5 IQR), 13(12-16.5 IQR), 16(16.25-22.25 IQR), 18(16.5-24 IQR) and 12 d(10.75-18.5 IQR) respectively. Median hospital stay was 22 d(19-26 IQR) while mortality was 4/20(20%). CONCLUSION TTD for difficult GDP is feasible, easy in the emergency, and patients recover in two-three weeks. | 6 | Assessment of long-term climate variability and its impact on the decadal growth of horticultural crops in central India显示文摘Background:The impact of climate change on horticultural production is of utmost concern worldwide.One such vulnerable region of horticultural importance is the Madhya Pradesh state in the central part of India.It is crucial to analyse the long-term trend in climatic variations and its effect on horticultural crop production.Therefore,this study focuses on detecting trends in mean annual precipitation and temperature of 115 years for the 15 districts covering all the regions of the state as well as projections of all the Representative Concentration Pathway(RCP)climatic scenarios for 2050 and 2080 of Madhya Pradesh state.The decadal(2010–2020)insight into the area and production of horticultural crops is undertaken which will help forecasting the future growth trend.For a better insight,case study on four horticultural crops is undertaken to assess their decadal growth trend vis-à-vis climate parameters in these 15 districts.Results:The Mann–Kendall test for 1901–2016 and Sen’s slope indicated a non-significant change in long-term trend for precipitation except for increasing change in Khargone and decreasing change in Rewa districts.Signifi-cant increasing trend of average temperature is obtained for Hoshangabad,Jabalpur,Sagar and Rewa districts.The Sen’s slope indicated an increase of 0.005 to 0.009℃for maximum temperature and 0.005 to 0.012℃for minimum temperature annually.The projections of all the RCP climate scenarios for the years 2050 and 2080 indicated non-significant variation in precipitation but an increase in maximum(1.4 to 4.1℃)and minimum(1.45 to 4.65℃)annual temperatures.An increasing trend in area and production for horticultural crops is also observed in central India for the study period.There is yield increase in all the crops selected in case study except for potato,which recorded a decreased yield between the years 2010–2015.Conclusion:The Mann–Kendall test and projections indicate towards climate change with a temperature rise.Though the decadal study indicates an increasing trend in horticultural crops,the districts identified to be affected by climate change need to have a plan to lessen the horticultural loss in the state.This study contributes to understand-ing the future climate change trends and its impact on horticultural crop production to formulate various adaptation strategies. | Gaurav Sharma Amita Sharma Nishant Kumar Sinha Om Prakash Sharma Ashutosh Singh Ajai Kumar Pandey Abhishek Kumar Sudhir Kumar Trivedi Bharti Sao Mukesh Kumar Sahu | 2022 | Ecological Processes2022,11,1: | 0 |

The area of biotechnology interfaces with nanostructures, reconfigures their composition, and alters their characteristics;which influences the dispersion of the particles, the biotransformation they cause, and their potential toxic effect. It is vital to link the idea of the lifecycle of nanostructures to the biological impacts and use methodologies to identify, estimate, and track the gradual bioprocessing of nanostructures in vivo, from a body-wide level to a nanoscopic size. This is necessary because understanding how nanostructures processing, degradation, persistence, and recycling predict potential exposure risks. The safe implementation of nanotechnology-based products in biomedical applications necessitates an extensive understanding of the recycling and transformations of nanomaterials in a living organism. Long-term fate in the body is crucial, as it governs potential environmental risks to human health. Strategies may be used to manage the long-term outcome of nanostructures in an organism since, in addition to composition, their design also affects how long they last and how easily they degrade. The lifespan of nanoparticles, a flexible and biocompatible category of nanostructures that have made it into clinical trials, is the subject of this article. | 10 | Clinical characteristics of COVID-19 patients who underwent tracheostomy and its effect on outcome:A retrospective observational study显示文摘BACKGROUND The exponential rise in Coronavirus disease 2019(COVID-19)cases has resulted in an increased number of patients requiring prolonged ventilatory support and subsequent tracheostomy.With the limited availability of literature regarding the outcomes of COVID-19 patients with tracheostomy,we attempted to study the clinical characteristics and multiple parameters affecting the outcomes in these patients.AIM To determine all-cause mortality following tracheostomy and its association with various risk factors in COVID-19 patients.METHODS This retrospective study included 73 adult COVID-19 patients admitted to the ICU between 1 April,2020 and 30 September,2021 who underwent tracheostomy as a result of acute respiratory failure due to COVID-19.The data collected included demographics(age,sex),comorbidities,type of oxygen support at admission,severity of COVID-19,complications,and other parameters such as admission to tracheostomy,intubation to tracheostomy,ICU stay,hospital stay,and outcome.RESULTS This study included 73 adult patients with an average age of 52±16.67 years,of which 52%were men.The average time for admission to tracheostomy was 18.12±12.98 days while intubation to tracheostomy was 11.97±9 days.The mortality rate was 71.2%and 28.8%of patients were discharged alive.The mean duration of ICU and hospital stay was 25±11 days and 28.21±11.60 days,respectively.Greater age,severe COVID-19,mechanical ventilation,shock and acute kidney injury were associated with poor prognosis;however,early tracheostomy in intubated patients resulted in better outcomes.CONCLUSION Patients with severe COVID-19 requiring mechanical ventilation have a poor prognosis but patients with early tracheostomy may benefit with no added risk.We recommend that the timing of tracheostomy be decided on a case-by-case basis and a well-designed randomised controlled trial should be performed to elucidate the potential benefit of early tracheostomy in such patients. | Yudhyavir Singh Kapil Dev Soni Abhishek Singh Nikita Choudhary Fahina Perveen Richa Aggarwal Nishant Patel Shailendra Kumar Anjan Trikha | 2022 | World Journal of Virology2022,11,6: | 0 |
