| 1 | Late biliary complications in human alveolar echinococcosis are associated with high mortality显示文摘AIM:To evaluate the incidence of late biliary complications in non-resectable alveolar echinococcosis(AE)under long-term chemotherapy with benzimidazoles.METHODS:Retrospective analysis of AE patients with biliary complications occurring more than three years after the diagnosis of AE.We compared characteristics of patients with and without biliary complications,analyzed potential risk factor for biliary complications and performed survival analyses.RESULTS:Ninety four of 148 patients with AE in Zurich had non-resectable AE requiring long-term benzimidazole chemotherapy,of which 26(28%)patients developed late biliary complications.These patients had a median age of 55.5(35.5-65)years at diagnosis of AE and developed biliary complications after 15(8.25-19)years of chemotherapy.The most common biliary complications during long-term chemotherapy were late-onset cholangitis(n=14),sclerosing cholangitis-like lesions(n=8),hepatolithiasis(n=5),affection of the common bile duct(n=7)and secondary biliary cirrhosis(n=7).Thirteen of the 26 patients had undergone surgery(including 12 resections)before chemotherapy.Previous surgery was a risk factor for late biliary complications in linear regression analysis(P=0.012).CONCLUSION:Late biliary complications can be observed in nearly one third of patients with non-resectable AE,with previous surgery being a potential risk factor.After the occurrence of late biliary complications,the median survival is only 3 years,suggesting that late biliary complications indicate a poor prognostic outcome. | Pascal Frei Benjamin Misselwitz Meher K Prakash Alain M Schoepfer Bettina M Prinz Vavricka Beat Müllhaupt Michael Fried Kuno Lehmann Rudolf W Ammann Stephan R Vavricka | 2014 | World Journal of Gastroenterology2014，20，19： | 5 |
| 2 | Pancreatic Castleman disease treated with laparoscopic distal pancreatectomy显示文摘BACKGROUND: Castleman disease is an uncommon lympho-proliferative disorder most frequently occurring in the mediastinum. Abdominal forms are less frequent, with pancreatic localization of the disease in particular being extremely rare. Only seventeen cases have been described in the world literature. METHOD: This report describes an interesting and unusual case of pancreatic Castleman disease treated with laparoscopic resection. RESULTS: A 48-year-old woman presented with epigastric pain. CT scan showed a well-encapsulated mass on the ventral border of the pancreas. Endosonography with fine needle aspiration biopsy was performed. Biopsy showed lymphoid elements and structures of a normal lymph node. The patient was treated with laparoscopic distal pancreatectomy. The pancreas was transected with a Ligasure device and the pancreatic stump was secured with a manual suture. One year after surgery the patient was complaint-free and showed no signs of recurrence of the disease. CONCLUSIONS: Laparoscopic distal pancreatectomy is a feasible and safe method for the treatment of lesions in the body and tail of the pancreas. | 3 | Sensitivity analysis of parameters and contrasting performance of ground filtering algorithms with UAV photogrammetry-based and LiDAR point clouds显示文摘Most ground filtering algorithms are primarily designed for airborne LiDAR point cloud processing and their successful use in identifying ground points from photogrammetric point clouds remains questionable.We compared six ground filtering algorithms implemented in Metashape,ArcGIS,CloudCompare,LAStools,and PDAL.We used UAV photogrammetry-based(acquired under leaf-off conditions)and airborne LiDAR(leaf-on)point clouds of the same area to:(i)compare accuracy of generated DTMs;(ii)evaluate the effect of vegetation density and terrain slope on filtering accuracy;and(iii)assess which algorithm parameters have the greatest effect on the filtering accuracy.Our results show that the performance of filtering algorithms was affected by the point cloud type,terrain slope and vegetation cover.The results were generally better for LiDAR(RMSE 0.13–0.19 m)than for photogrammetric(RMSE 0.19–0.23 m)point clouds.The behavior in varying vegetation and terrain conditions was consistent for LiDAR point clouds.However,when applied on photogrammetric point clouds,the algorithms’behavior was inconsistent,especially in areas of steep slope(except for the Progressive Triangulated Irregular Network in LAStools).Parameters related to the selection of the initial minimum elevation ground points were the most influential in all algorithms and point clouds. | Petr Klápště Michal Fogl Vojtěch Barták Kateřina Gdulová Rudolf Urban Vítězslav Moudrý | 2020 | International Journal of Digital Earth2020，13，12： | 2 |
