1Effects and safety of natriuretic peptides as treatment of cirrhotic ascites:A systematic review and meta-analysis显示文摘BACKGROUND Natriuretic peptides are involved in the cascade of pathophysiological events occurring in liver cirrhosis,counterbalancing vasoconstriction and anti-natriuretic factors.The effects of natriuretic peptides as treatment of cirrhotic ascites have been investigated only in small studies,and definitive results are lacking.AIM To examine the effects and safety of natriuretic peptides in cirrhosis patients with ascites.METHODS We searched MEDLINE,Web of Science,Scopus,Cochrane Library and Embase for all available studies applying intravenous administration of any natriuretic peptide to patients suffering from cirrhotic ascites.Inclusion was not limited by treatment duration or dose,or by follow-up duration.Both randomised controlled trials and non-randomised studies were eligible for inclusion.The primary outcome was change in renal sodium excretion.Secondary outcomes included safety measures and changes in renal water excretion,plasma aldosterone concentration,and plasma renin activity.RESULTS Twenty-two studies were included.Atrial natriuretic peptide(ANP)was the only intensively studied treatment.Sodium excretion increased in response to continuous ANP infusion and was more pronounced when infusion rates of>30 ng/kg/min were administered compared with≤30 ng/kg/min(P<0.01).Moreover,natriuresis was significantly higher in study subgroups with mild/moderate ascites compared with moderate/severe and refractory ascites(P<0.01).ANP infusions increased renal water excretion,although without reaching a statistically significant dose-response gradient.Plasma aldosterone concentration and plasma renin activity were significantly lower at baseline in study subgroups achieving a negative sodium balance in response to an ANP administration compared with treatment non-responders(P<0.01).Blood pressure decreases occurred less frequently when ANP doses≤30 ng/kg/min were applied.The quality of evidence for a natriuretic response to ANP was low,mainly due to small sample sizes and considerable between-study heterogeneity.Data were sparse for the other natriuretic peptides;B-type natriuretic peptide and urodilatin.CONCLUSION Intravenous ANP infusions increase sodium excretion in patients with cirrhotic ascites.Continuous infusion rates>30 ng/kg/min are the most effective.However,safety increases with infusion rates≤30 ng/kg/min.Rasmus Hvidbjerg Gantzel Mikkel Breinholt Kjær Peter Jepsen Niels Kristian Aagaard Hugh Watson Lise Lotte Gluud Henning Grønbæk 2022World Journal of Hepatology2022,14,4:1

16In vivo cardiac pacemaker function of differentiated human mesenchymal stem cells from adipose tissue transplanted into porcine hearts显示文摘BACKGROUND Mesenchymal stem cells(MSC)modified by gene transfer to express cardiac pacemaker channels such as HCN2 or HCN4 were shown to elicit pacemaker function after intracardiac transplantation in experimental animal models.Human MSC derived from adipose tissue(haMSC)differentiate into cells with pacemaker properties in vitro,but little is known about their behavior after intracardiac transplantation.AIM To investigate whether haMSC elicit biological pacemaker function in vivo after transplantation into pig hearts.METHODS haMSC under native conditions(nhaMSC)or after pre-conditioning by medium differentiation(dhaMSC)(n=6 pigs each,5×106 cells/animal)were injected into the porcine left ventricular free wall.Animals receiving PBS injection served as controls(n=6).Four weeks later,total atrioventricular(AV)-block was induced by radiofrequency catheter ablation,and electronic pacemaker devices were implanted for backup stimulation and heart rate monitoring.Ventricular rate and rhythm of pigs were evaluated during a follow-up of 15 d post ablation by 12-lead-ECG with heart rate assessment,24-h continuous rate monitoring recorded by electronic pacemaker,assessment of escape recovery time,and pharmacological challenge to address catecholaminergic rate response.Finally,hearts were analyzed by histological and immunohistochemical investigations.RESULTS In vivo transplantation of dhaMSC into the left ventricular free wall of pigs elicited spontaneous and regular rhythms that were pace-mapped to ventricular injection sites(mean heart rate 72.2±3.6 bpm;n=6)after experimental total AV block.Ventricular rhythms were stably detected over a 15-d period and were sensitive to catecholaminergic stimulation(mean maximum heart rate 131.0±6.2 bpm;n=6;P<0.001).Pigs,which received nhaMSC or PBS presented significantly lower ventricular rates(mean heart rates 47.2±2.5 bpm and 37.4±3.2 bpm,respectively;n=6 each;P<0.001)and exhibited little sensitivity towards catecholaminergic stimulation(mean maximum heart rates 76.4±3.1 bpm and 60.5±3.1 bpm,respectively;n=6 each;P<0.05).Histological and immunohistochemical evaluation of hearts treated with dhaMSC revealed local clusters of transplanted cells at the injection sites that lacked macrophage or lymphocyte infiltrations or tumor formation.Intense fluorescence signals at these sites indicated membrane expression of HCN4 and other pacemaker-specific proteins involved in cardiac automaticity and impulse propagation.CONCLUSION dhaMSC transplanted into pig left ventricles sustainably induced rate-responsive ventricular pacemaker activity after in vivo engraftment for four weeks.The data suggest that pre-conditioned MSC may further differentiate along a pacemakerrelated lineage after myocardial integration and may establish superior pacemaker properties in vivo.Fabrice F Darche Rasmus Rivinius Ann-Kathrin Rahm Eva Köllensperger Uwe Leimer Günter Germann Miriam Reiss Michael Koenen Hugo A Katus Dierk Thomas Patrick A Schweizer 2020World Journal of Stem Cells2020,12,10:0

18Highly Increased Levels of Inter-α-inhibitor Heavy Chain 4(ITIH4)in Autoimmune Cholestatic Liver Diseases显示文摘Background and Aims:There is an unmet need for new bi-omarkers to improve diagnostics and prognostics in primary biliary cholangitis(PBC)and primary sclerosing cholangitis(PSC).Inter-α-inhibitor heavy chain 4(ITIH4)is an abun-dant,liver-produced protein,and its synthesis may be al-tered in liver diseases.We investigated whether ITIH4 plas-ma concentrations were affected in PBC and PSC patients.Methods:We developed an immunoassay specific for ITIH4 and determined ITIH4 plasma concentrations in 66 PBC,126 PSC,92 autoimmune hepatitis(AIH),67 chronic hepatitis C(CHC),33 alcoholic hepatitis(AH)patients and 138 healthy controls(HCs).Hepatic ITIH4 expression was investigated by immunohistochemistry in PBC.Results:The mean plas-ma concentration of ITIH4 was almost doubled in PBC[409μg/mL(95%CI:388-431)]and 35%higher in PSC[308μg/mL,(95%CI:296-319)]compared with HCs[226μg/mL(95%CI:221-231);p<0.001].In PBC patients,ITIH4 correlated with IgM(rho=0.49,p<0.001).Responders to ursodeoxycholic acid treatment(UDCA)had lower levels of ITIH4 than incomplete responders[395μg/mL(95%CI:364-425)]vs.460μg/mL(95%CI:421-498);p=0.02].Four weeks of UDCA treatment had no effect(p=0.19).In-creased ITIH4 immunohistochemical staining was seen in a liver biopsy from a PBC patient.ITIH4 levels in AIH[224μg/mL(95%CI:208-241)]and HCs were similar(p=0.8).ITIH4 levels were lower in AH[199μg/mL(95%CI:175-223)]and CHC[202μg/mL(192-212)]patients than in HCs(p<0.05).Conclusions:The plasma concentration of ITIH4 was highly elevated in patients with PBC and PSC,suggesting that ITIH4 should be further investigated as a biomarker in cholestatic liver disease.Tea Lund Laursen Lars Bossen Rasmus Pihl Anne Troldborg Thomas Damgaard Sandahl Annette Gudmann Hansen Trine Folserass Mette Vesterhus Henning Grønbæk Steffen Thiel 2022Journal of Clinical and Translational Hepatology2022,10,5:0
