2Extrahepatic complications to cirrhosis and portal hypertension: Haemodynamic and homeostatic aspects显示文摘In addition to complications relating to the liver, patients with cirrhosis and portal hypertension develop extrahepatic functional disturbances of multiple organ systems. This can be considered a multiple organ failure that involves the heart, lungs, kidneys, the immune systems, and other organ systems. Progressive fibrosis of the liver and subsequent metabolic impairment leads to a systemic and splanchnic arteriolar vasodilatation. This affects both the haemodynamic and functional homeostasis of many organs and largely determines the course of the disease. With the progression of the disease, the circulation becomes hyperdynamic with cardiac, pulmonary as well as renal consequences for dysfunction and reduced survival. Infections and a changed cardiac function known as cirrhotic cardiomyopathy may be involved in further aggravation of other complications such as renal failure precipitatingthe hepatorenal syndrome.Patients with end-stage liver disease and related complications as for example the hepatopulmonary syndrome can only radically be treated by liver transplantation.As a bridge to this treatment,knowledge on the mechanisms of the pathophysiology of complications is essential for the choice of vasoactive drugs,antibiotics,drugs with specific effects on fibrogenesis and inflammation,and drugs that target specific receptors.S?ren M?ller Jens H Henriksen Flemming Bendtsen 2014World Journal of Gastroenterology2014,20,42:14

4Steroid hyperglycemia: Prevalence, early detection and therapeutic recommendations: A narrative review显示文摘Steroids are drugs that have been used extensively in a variety of conditions. Although widely prescribed for their anti-inflammatory and immunosuppressive properties, glucocorticoids have several side effects, being hyperglycemia one of the most common and representative. In the present review, we discuss the main epidemiologic characteristics associated with steroid use, with emphasis on the identification of high risk populations. Additionally we present the pathophysiology of corticosteroid induced hyperglycemia as well as the pharmacokinetics and pharmacodynamics associated with steroid use. We propose a treatment strategy based on previous reports and the understanding of the mechanism of action of both, the different types of glucocorticoids and the treatment options, in both the ambulatory and the hospital setting. 5Multivariate statistical analysis of clinicopathologic factors influencing survival of patients with bile duct carcinoma显示文摘AIM: To evaluate the influence of various clinicopathologicfactors on survival of patients with bile duct carcinoma aftercurative resection.METHODS: A retrospective analysis was made for 86 casesof bile duct carcinoma treated from January 1981 toSeptember 1995. Fifteen clinicopathologic factors possiblyinfluencing survival were selected. Independent variableswere first analyzed by univariate methods. Survival forvariable was estimated by the method of Kaplan and Meier.The variables that were statistically significant by univarlateanalysis were included in a multivariate analysis, which wereconfirmed using the Cox stepwise proportion hazard modelwith the help of SPSS 10.0 for Windows software.RESULTS: The overall cumulative survival rate was 72.6 %at 1 year, 32.4 % at 3 years, and 18.7 % at 5 years. Theresults of univariate analysis showed that the majorsignificant prognostic factors influencing survival of thesepatients were histological type of lesion, lymph nodemetastasis, pancreatic invasion, duodenal invasion, pedneuralinvasion, macroscopic vessel involvement, resected surgicalmargin and depth of cancer invasion (P=0.02, 0.02, 0.004,0.005, 0.01, 0.43, 0.03 and 0.04). Age, sex, location of tumor,size of tumor, macroscopic type of lesions, hepaticmetastasis, and hepatic invasion were not significantlyassociated with prognosis (P>0.05). Pancreatic invasion,perineural invasion and lymph node metastases were thethree most important prognostic factors by multivariateanalysis using the Cox proportional hazards model.CONCLUSION: Pancreatic invasion, pedneural invasion andlymph node metastases are the most important prognosticfactors for bile duct carcinoma after curative resection.He, P Shi, JS Chen, WK Wang, ZR Ren, H Li, H 2002World Journal of Gastroenterology2002,8,5:10
6Experimental small bowel preservation using Polysol: A new alternative to University of Wisconsin solution, Celsior and histidine-tryptophan-ketoglutarate solution?显示文摘AIM: To evaluate the potential of Polysol, a newly developed preservation solution, in cold storage of small bowel grafts, compared with the current standards, University of Wisconsin solution (UW), Celsior and histidine-tryptophan-ketoglutarate solution (HTK). METHODS: Male Wistar rats were used as donors. Small bowels were retrieved, flushed and then stored in the respective 4 solutions for 18 h at 4℃. Functional integrity of the grafts was evaluated by isolated reperfusion with oxygenated Krebs-Henseleit buffer at 37℃ for 30 min in all 4 groups. RESULTS: Polysol preservation exhibited the highest tissue ATP concentration and the lowest release of LDH. Malondialdehyde, an index for tissue lipid peroxidation, was also the lowest in Polysol. Tissue oxygen consumption was significantly higher in Polysol than in the others. Of interest, UW-storage promoted 10-fold higher apoptosis than in the others. Moreover, electron microscopy revealed that the mucosal villi/micro-villi formation and the cell organelles, including mitochondria, were both significantly better preserved in Polysol, while deleterious alterations were apparent in the others, most notably in UW. Although Celsior and HTK exhibited the better trend of results than UW in some parameters, but could not reach the over-all superiority to UW.CONCLUSION: Cold storage using Polysol resulted in significantly better integrity and function of small bowel grafts than UW. 8Cellulose structure and lignin distribution in normal and compression wood of the Maidenhair tree(Ginkgo biloba L.)显示文摘We studied in detail the mean micro fibril angle and the width of cellulose crystals from the pith to the bark of a 15-year-old Maidenhair tree(Ginkgo biloba L.). The orientation of cellulose micro fibrils with respect to the cell axis and the width and length of cellulose crystallites were determined using Xray diffraction. Raman microscopy was used to compare the lignin distribution in the cell wall of normal/opposite and compression wood, which was found near the pith. Ginkgo biloba showed a relatively large mean micro fibril angle,varying between 19° and 39° in the S2 layer, and the average width of cellulose crystallites was 3.1–3.2 nm. Mild compression wood without any intercellular spaces or helical cavities was observed near the pith. Slit-like bordered pit openings and a heavily lignified S2 L layer con firmed the presence of compression wood. Ginkgo biloba showed typical features present in the juvenile wood of conifers. The micro fibril angle remained large over the 14 annual rings. The entire stem disc,with a diameter of 18 cm, was considered to consist of juvenile wood. 9Nutrition in chronic pancreatitis显示文摘The pancreas is a major player in nutrient digestion.In chronic pancreatitis both exocrine and endocrine insufficiency may develop leading to malnutrition over time.Maldigestion is often a late complication of chronic pancreatic and depends on the severity of the underlying disease.The severity of malnutrition is correlated with two major factors:(1)malabsorption and depletion of nutrients(e.g.,alcoholism and pain)causes impaired nutritional status;and(2)increased metabolic activity due to the severity of the disease.Nutritional deficiencies negatively affect outcome if they are not treated.Nutritional assessment and the clinical severity of the disease are important for planning any nutritional intervention.Good nutritional practice includes screening to identify patients at risk,followed by a thoroughly nutritional assessment and nutrition plan for risk patients.Treatment should be multidisciplinary and the mainstay of treatment is abstinence from alcohol,pain treatment,dietary modifications and pancreatic enzyme supplementation.To achieve energy-end protein requirements,oral supplementation might be beneficial.Enteral nutrition may be used when patients do not have sufficient calorie intake as in pylero-duodenalstenosis,inflammation or prior to surgery and can be necessary if weight loss continues.Parenteral nutrition is very seldom used in patients with chronic pancreatitis and should only be used in case of GI-tract obstruction or as a supplement to enteral nutrition.Henrik Hφjgaard Rasmussen φivind Irtun Sφren Schou Olesen Asbjφrn Mohr Drewes Mette Holst 2013World Journal of Gastroenterology2013,19,42:4

12Emission from quantum-dot high-β microcavities: transition from spontaneous emission to lasing and the effects of superradiant emitter coupling显示文摘Measured and calculated results are presented for the emission properties of a new class of emitters operating in the cavity quantum electrodynamics regime.The structures are based on high-finesse GaAs/AlAs micropillar cavities,each with an active medium consisting of a layer of InGaAs quantum dots(QDs)and the distinguishing feature of having a substantial fraction of spontaneous emission channeled into one cavity mode(highβ-factor).This paper demonstrates that the usual criterion for lasing with a conventional(lowβ-factor)cavity,that is,a sharp non-linearity in the input–output curve accompanied by noticeable linewidth narrowing,has to be reinforced by the equal-time second-order photon autocorrelation function to confirm lasing.The paper also shows that the equal-time second-order photon autocorrelation function is useful for recognizing superradiance,a manifestation of the correlations possible in high-βmicrocavities operating with QDs.In terms of consolidating the collected data and identifying the physics underlying laser action,both theory and experiment suggest a sole dependence on intracavity photon number.Evidence for this assertion comes from all our measured and calculated data on emission coherence and fluctuation,for devices ranging from light-emitting diodes(LEDs)and cavity-enhanced LEDs to lasers,lying on the same two curves:one for linewidth narrowing versus intracavity photon number and the other for g(2)(0)versus intracavity photon number.Sören Kreinberg Weng W Chow Janik Wolters Christian Schneider Christopher Gies Frank Jahnke Sven Höfling Martin Kamp Stephan Reitzenstein 2017Light(Science & Applications)2017,6,1:2
