| 1 | 游走脾:手术之謎显示文摘Wandering spleen,also referred to as‘ptotic spleen’,is a rare clinical condition characterized by splenicmigration form its normal left hypochondrial position to any other abdominal or pelvic position.Among themultifactorial etiologies proposed,laxity of the spleen’s primary supporting ligaments is themost agreed-upon hypothesis.We present one rare case of wandering spleen in an 11-year-old girl who presented with recurrent abdominal pain with no localizing features.Her abdominal examination revealed an intra-abdominal left iliac fossa lump with restrictedmobility,which was confirmed as a wandering spleen by abdominal sonography and contrast-enhanced computed tomography.Intraoperatively,an infarcted spleen was encountered with tortuous,elongated,torsional splenic pedicle and a single dense adhesive band with descending colon.Splenectomy was offered to the patient.Post-operatively,the patient is healthy and symptom free at one-year follow-up.The rare clinical diagnosis of this condition,particularly in the paediatric age-group,makes it an enigma for the surgical world. | Ashok Kumar Puranik Rohit Mehra Sushila Chauhan Rahul Pandey | 2017 | Gastroenterology Report2017,5,3: | 2 |

| 4 | Chromophobe renal cell carcinoma: Novel molecular insights and clinicopathologic updates显示文摘Chromophobe renal cell carcinoma(ChRCC)is the third most common renal cell carcinoma(RCC)subtype,which predominantly occurs in sporadic setting.ChRCCs are considered to originate from the intercalated cell of distal tubules with two main morphological variants,classic and eosinophilic.Most ChRCCs carry a favorable clinical outcome.Histology alone is limited in predicting the behavior of ChRCCs that do not have overtly aggressive morphologic findings such as necrosis and sarcomatoid features.Along with positive CD117 expression,classic ChRCCs generally express diffuse and uniform CK7,while eosinophilic variant demonstrates more heterogeneous CK7 expression(rare or patchy).Multiple losses of chromosomes 1,2,6,10,13,17,and 21 are considered to be the genetic hallmarks of classic and eosinophilic ChRCCs,while chromosomal gains are known to be associated with sarcomatoid ChRCCs.TP53 and PTEN are the two most frequently mutated genes in ChRCCs.The major challenge in the differential diagnosis of ChRCCs includes considerations around the eosinophilic variant(of ChRCCs),where it may share overlapping features with oncocytoma or other recent emergent oncocytic tumors.Most eosinophilic ChRCCs share expression of the recently described biomarkers,LINC01187 and FOXI1,with classic ChRCCs,however,a subset of eosinophilic-like ChRCCs with lower biomarker expression have been demonstrated to harbor MTOR gene mutations.Overall,the morphologic features of ChRCCs and genetic profile with combinations of chromosomal losses and gains suggest this tumor entity to represent a distinct,yet heterogeneous group of renal neoplasms. | Reza Alaghehbandan G.Przybycin Christopher Virginie Verkarre Rohit Mehra | 2022 | Asian Journal of Urology2022,9,1: | 1 |

| 10 | 右侧十二指肠旁疝:两例报告和文献复习显示文摘Paraduodenal hernia(PDH),a rare congenital anomaly,is a type of internal hernia which occurs due to a defect in the reduction and rotation of the midgut.On anatomical and embryological basis,PDH can be broadly divided into right-and Left PDH.Right PDH is rarer than its counterpart.We present two cases of Right PDH.The patientsy presented with a history of recurrent intestinal obstruction since childhood,which was managed conservatively,without a definitive diagnosis.Once they presented to us,a detailed clinical history and a barium meal follow-through clinched the diagnosis of PDH.Intra-operative findings correlated well with the clinical diagnosis.The jejunal loops had herniated through the fossa of Waldeyer.Reduction of hernia contents and excision of the hernia sac was carried out.Post-operatively,the patients are healthy and symptom-free at 4 and 3 years follow-up,respectively.The rarity of this condition and the need for early diagnosis,to prevent the high risk of bowel obstruction and strangulation,makes PDH one of the difficult challenges for the clinicians. | Rohit Mehra Aswini Kumar Pujahari | 2016 | Gastroenterology Report2016,4,2: | 0 |
