1Prophylactic lateral pelvic lymph node dissection in stage Ⅳ low rectal cancer显示文摘AIM To assess the clinical significance of prophylactic lateral pelvic lymph node dissection (LPLND) in stage Ⅳ low rectal cancer.METHODS We selected 71 consecutive stage Ⅳ low rectal cancer patients who underwent primary tumor resection,and enrolled 50 of these 71 patients without clinical LPLN metastasis.The patients had distant metastasis such as liver,lung,peritoneum,and paraaortic LN.Clinical LPLN metastasis was defined as LN with a maximum diameter of 10 mm or more on preoperative pelvic computed tomography scan.All patients underwent primary tumor resection,27 patients underwent total mesorectal excision(TME) with LPLND(LPLND group),and 23 patients underwent only TME(TME group).Bilateral LPLND was performed simultaneously with primary tumor resection in LPLND group.R0 resection of both primary and metastatic sites was achieved in 20 of 50 patients.We evaluated possible prognostic factors for 5-year overall survival (OS),and compared 5-year cumulative local recurrence between the LPLND and TME groups.RESULTS For OS,univariate analyses revealed no significant benefit in the LPLND compared with the TME group (28.7% vs 17.0%,P = 0.523); multivariate analysis revealed that R0 resection was an independent prognostic factor.Regarding cumulative local recurrence,the LPLND group showed no significant benefit compared with TME group (21.4% vs 14.8%,P = 0.833).CONCLUSION Prophylactic LPLND shows no oncological benefits in patients with Stage Ⅳ low rectal cancer without clinical LPLN metastasis.Hiroshi Tamura Yoshifumi Shimada Hitoshi Kameyama Ryoma Yagi Yosuke Tajima Takuma Okamura Mae Nakano Masato Nakano Masayuki Nagahashi Jun Sakata Takashi Kobayashi Shin-ichi Kosugi Hitoshi Nogami Satoshi Maruyama Yasumasa Takii Toshifumi Wakai 2017World Journal of Clinical Oncology2017,8,5:4
2Health Emergency and Disaster Risk Management (Health-EDRM):Developing the Research Field within the Sendai Framework Paradigm显示文摘The intersection of health and disaster risk reduction(DRR) has emerged in recent years as a field of critical inquiry.Health is recognized as an outcome and a goal of DRR,and the integration of both fields is essential to ensure the implementation of the Sendai Framework for Disaster Risk Reduction 2015–2030.Health Emergency and Disaster Risk Management(Health-EDRM) has emerged as an umbrella field that encompasses emergency and disaster medicine,DRR,humanitarian response,community health resilience,and health systems resilience.In September 2016,an international group of experts met in Hong Kong to assess the current status and potential of the Health-EDRM research field,a research area that these scholars characterized as underdeveloped and fragmented.Key challenges identified include research overlap,lack ofstrategic research agenda,absence of consensus regarding terminology,and limited coordination between stakeholders.The Sendai Framework provides a useful paradigm within which to shape the research field's strategic development.The WHO Thematic Platform for Health-EDRM Research Group was established to coordinate activities,promote information-sharing,develop partnerships,and provide technical advice to strengthen the Health-EDRM research field.This group will promote the generation of robust and scientific health research to support the meaningful implementation of the Sendai Framework.Sharon Tsoon Ting Lo Emily Ying Yang Chan Gloria Kwong Wai Chan Virginia Murray Jonathan Abrahams Ali Ardalan Ryoma Kayano Johnny Chung Wai Yau 2017International Journal of Disaster Risk Science2017,8,2:3

6Daunorubicin can eliminate iPS-derived cancer stem cells via ICAD/CAD-independent DNA fragmentation显示文摘Aim:To identify a drug that can effectively eliminate these cancer stem cells(CSCs)and determine its mode of action.Methods:CSCs were obtained from mouse induced pluripotent stem cells(miPSCs)using cancer cell-conditioned media.Drug screening was performed on these cells or after transplantation into mice.Apoptosis was analyzed by flow cytometry and western blotting.Results:Drug screening studies showed that daunorubicin,a topoisomerase II inhibitor,is specifically cytotoxic to miPS-CSCs.Daunorubicin-induced apoptosis was found to be associated with p53 accumulation,activation of the caspase cascade,and oligonucleosomal DNA fragmentation.Treatment with the caspase inhibitor abolished daunorubicin-induced DNA fragmentation and was therefore considered to act downstream of caspase activation.This was also suppressed by treatment with a Ca2+-specific chelator,which suggested that CAD endonuclease does not contribute.Moreover,no obvious ICAD reduction/degradation was detected.Conclusion:Daunorubicin effectively eliminated CSCs,which are dependent on the p53/caspase signaling cascade.The current findings provided the basis for further studies on CSC-targeted drugs for the development of cancer treatment strategies.Akimasa Seno Akifumi Mizutani Kazuki Aizawa Ryoma Onoue Junko Masuda Naotaka Ochi Saki Taniguchi Tatsuyuki Sota Yuki Hiramoto Taisuke Michiue Neha Nair Masaharu Seno 2019Cancer Drug Resistance2019,2,2:1

10Health Emergency and Disaster Risk Management: Five Years into Implementation of the Sendai Framework显示文摘The Sendai Framework for Disaster Risk Reduction 2015–2030 recognizes health at the heart of disaster risk management(DRM)at the global policy level.Five years on,it has catalyzed the rapid development of the field of Health Emergency and Disaster Risk Management(Health EDRM)by providing a mandate for building partnerships as well as enhancing scientific research.Key milestones achieved include publication of the World Health Organization’s Health EDRM Framework,development of the WHO Thematic Platform for Health EDRM and the WHO Health EDRM Research Network,and further application of health information principles to DRM.Furthermore,health actors at all levels have continued to engage in the Sendai Framework processes and have had a key role in its implementation and proposed monitoring.There have been significant gains made through the partnership of health and DRM,but the relationship has not been without its challenges.Many national,regional,and global initiatives continue to operate with a lack of consistency and of linkages to respond to the Sendai Framework’s call for embedding health resilience in DRM,and conversely,embedding DRM in health resilience.Overcoming this hurdle is important,and doing so will be a key marker of success of the next 10 years of partnership under the Sendai Framework.Natalie Wright Lucy Fagan Jostacio MLapitan Ryoma Kayano Jonathan Abrahams Qudsia Huda Virginia Murray 2020International Journal of Disaster Risk Science2020,11,2:1
