1右美托咪定自控镇静在眼科斜视手术中的应用效果显示文摘目的探讨右美托咪定用于眼科斜视手术自控镇静的效果及安全性。方法选取2016年10月~2017年10月于我院眼科行斜视手术的40例患者作为研究对象,年龄16~35岁,ASAⅠ或Ⅱ级,将其随机分为对照组(C组)和自控镇静组(PCS组),每组各20例。C组患者恒速泵注(10 ml/h)生理盐水。PCS组患者局麻前静脉泵注负荷量右美托咪定0.2μg/kg,10 min注完,局麻后给予维持量右美托咪定0.1μg/(kg·h),单次量1.0 ml/次(4μg/ml),锁定时间60 s。比较两组患者的泵注负荷量前(T0)、负荷量结束时(T1)、手术开始时(T2)、术中5 min(T3)、术中10 min(T4)、术中20 min(T5)、术中30 min(T6)、术毕(T7)时的MAP、HR、Sp O_2、Ramsay镇静评分,患者的合作度、满意度及不良反应发生情况,并观察PCS组患者的按压次数。结果 PCS组患者T2~T7的MAP均低于T0,且均低于C组,差异有统计学意义(P<0.05)。PCS组患者T1~T7的HR均明显低于T0,且均低于C组,差异有统计学意义(P<0.05)。C组患者T1~T7的MAP、HR与T0比较,差异无统计学意义(P>0.05)。两组患者T0、T1、T7的Ramsay镇静评分比较,差异无统计学意义(P>0.05)。PCS组患者T2~T7的Ramsay镇静评分均高于T0,差异有统计学意义(P<0.05)。C组患者T1~T6的Ramsay镇静评分与T0比较,差异无统计学意义(P>0.05),C组患者T7的Ramsay镇静评分高于T0,差异有统计学意义(P<0.05),PCS组患者T2~T6的Ramsay镇静评分均高于C组,差异有统计学意义(P<0.05)。PCS组患者的满意度及合作度均优于C组,差异有统计学意义(P<0.05)。PCS组患者术中牵拉不适、躁动和呻吟的发生率显著低于C组,差异有统计学意义(P<0.05),两组患者眼心反射的发生率比较,差异无统计学意义(P>0.05)。PCS组患者给予负荷量后,有效按压次数为(10.7±5.9)次。结论右美托咪定自控镇静可安全有效地应用于眼科斜视手术,能充分体现个体化用药,患者满意度明显增高。高新明 曹贺 邓芬芬 Karthikeyan Arcot Sadagopan Dennis Lam 尉洋 2018中国当代医药2018,25,24:4
