| 5 | 糖尿病视网膜病变和黄斑囊样水肿患者曲安奈德眼内注射前、后玻璃体内血管内皮生长因子和基质衍生因子-1的水平显示文摘背景:能导致糖尿病视网膜病变(DR)视力丧失的弥漫性黄斑囊样水肿(DME)和(或)异常新生血管(NV)可能受生长因子和化学激活剂的调节。化学激活素基质衍生因子-1(SDF-1)是一种血管内皮生长因子(VEGF)表达的有效刺激剂,是NV的重要效应物。并且是与发生DME有关血管通透性的关键诱导物。循环中的内皮前体细胞对SDF-1应答迁徙参与NV。目的:调查不同程度的的DR和DME患者。在眼内注射曲安奈德(triamcinolone acetonide)前后,玻璃体中SDF-1和VEGF之间的关系。用于治疗难治的DME。方法:在这一前瞻性的研究中。包括36例患者并观察了6个月。在紧接着注射曲安奈德前和注射1个月后获得的标本,用酶联免疫吸附试验检测玻璃体中VEGF和SDF-1的水平。结果:增生性DR患者的VEGF和SDF-1与非增生DR患者相比显著高(P〈0.01)。DME患者与非DME患者相比较。SDF-1水平显著增加。VEGF与SDF-1的水平和疾病的严重程度相关(r^2=0.88)。结论:应用曲安奈德出现了激动人心的结果,VEGF和SDF-1降到了几乎检测不出来的水平。消除了DME并使活动性的NV退化。我们的结果支持SDF-1和VEGF在DR有害视力后果的发病机制中的作用,并提示注射曲安奈德后,可能由于抑制了VEGF和SDF-1,使得DME消退和(或)NV开始纤维化的退化。 | H.
