1Long-term outcome and prognostic factors of patients with hilar cholangiocarcinoma显示文摘AIM: To evaluate the long-term outcome and prognostic factors of patients with hilar cholangiocarinoma. METHODS: Ninety-six consecutive patients underwent treatment for malignant hilar bile duct tumors during 1995–2005. Of the 96 patients, 20 were initially treated with surgery (n = 2 R0 / n = 18 R1). In non-operated patients, data analysis was performed retrospectively. RESULTS: Among the 96 patients, 76 were treated with endoscopic transpapillary (ERC, n = 45) and/or percutaneous transhepatic biliary drainage (PTBD, n = 31). The mean survival time of these 76 patients undergoing palliative endoscopic and/or percutaneous drainage was 359 ± 296 d. The mean survival time of patients with initial bilirubin levels > 10 mg/dL was significantly lower (P < 0.001) than patients with bilirubin levels < 10 mg/dL. The mean survival time of patients with Bismuth stage Ⅱ (n = 8), Ⅲ (n = 28) and Ⅳ (n = 40) was 496 ± 300 d, 441 ± 385 d and 274 ± 218 d, respectively. Thus, patients with advanced Bismuth stage showed a reduced mean survival time, but the difference was not significant. The type of biliary drainage had no significant beneficial effect on the mean survival time (ERC vs PTBD, P = 0.806). CONCLUSION: Initial bilirubin level is a significant prognostic factor for survival of patients. In contrast, age, tumor stage according to the Bismuth-Corlette classification, and types of intervention are not significant prognostic parameters for survival. 2Depiction of secondary metabolites and antifungal activity of Bacillus velezensis DTU001显示文摘For a safe and sustainable environment,effective microbes as biocontrol agents are in high demand.We have isolated a new Bacillus velezensis strain DTU001,investigated its antifungal spectrum,sequenced its genome,and uncovered the production of lipopeptides in HPLC-HRMS analysis.To test the antifungal efficacy,extracts of B.velezensis DTU001 was tested against a range of twenty human or plant pathogenic fungi.We demonstrate that inhibitory potential of B.velezensis DTU001 against selected fungi is superior in comparison to single lipopeptide,either iturin or fengycin.The isolate showed analogous biofilm formation to other closely related Bacilli.To further support the biocontrol properties of the isolate,coculture with Candida albicans demonstrated that B.velezensis DTU001 exhibited excellent antiproliferation effect against C.albicans.In summary,the described isolate is a potential antifungal agent with a broad antifungal spectrum that might assist our aims to avoid hazardous pathogenic fungi and provide alternative to toxicity caused by chemicals.Sagarika Devi Heiko T.Kiesewalter Renato Kovacs Jens Christian Frisvad Tilmann Weber Thomas Ostenfeld Larsen Akos T.Kovacs Ling Ding 2019Synthetic and Systems Biotechnology2019,4,3:4

4Smart Grid Wide-Area Transmission System Visualization显示文摘The installation of vast quantities of additional new sensing and communication equipment, in conjunction with building the computing infrastructure to store and manage data gathered by this equipment, has been the fi rst step in the creation of what is generically referred to as the 'smart grid' for the electric transmission system. With this enormous capital investment in equipment having been made, attention is now focused on developing methods to analyze and visualize this large data set. The most direct use of this large set of new data will be in data visualization. This paper presents a survey of some visualization techniques that have been deployed by the electric power industry for visualizing data over the past several years. These techniques include pie charts, animation, contouring, time-varying graphs, geographic-based displays, image blending, and data aggregation techniques. 5Should antibiotics be administered before arthroscopic knee surgery? A systematic review of the literature显示文摘AIM To explore the current evidence surrounding the administration of prophylactic antibiotics for arthroscopic knee surgery. METHODS Databases were searched from inception through May of 2018 for studies examining prophylactic antibiotic use and efficacy in knee arthroscopy. Studies with patient data were further assessed for types of arthroscopic procedures performed,number of patients in the study,use of antibiotics,and outcomes with the intention of performing a pooled analysis. Data pertaining to 'deep tissue infection' or 'septic arthritis' were included in our analysis. Reported data on superficial infection were not included in our data analysis. For the pooled analysis,a relative risk ratio was calculated and χ~2 tests were used to assess for statistical significance between rates of infection amongst the various patient groups. Post hoc power analyses were performed to compute the statistical power obtained from our sample sizes. Number needed to treat analyses were performed for statistically significant differences by dividing 1 by the difference between the infection rates of the antibiotic and no antibiotic groups. An alpha value of 0.05 was used for our analysis. Study heterogeneity was assessed by Cochrane's Q test as well as calculation of the I^2 value.RESULTS A total of 49682 patients who underwent knee ar-throscopy for a diverse set of procedures across 19 studies met inclusion critera for pooled analysis. For those not undergoing graft procedures,there were 27 cases of post-operative septic arthritis in 34487 patients(0.08%) who received prophylactic antibiotics and 16 cases in 10911(0.15%) who received none [risk ratio(RR) = 0.53,95% confidence interval(CI): 0.29-0.99,P = 0.05]. A sub-group analysis in which bony procedures were excluded was performed which found no significant difference in infection rates between patients that received prophylactic antibiotics and patients that did not(P > 0.05). All anterior cruciate ligament reconstruction studies used prophylactic antibiotics,but two studies investigating the effect of soaking the graft in vancomycin in addition to standard intravenous(IV) prophylaxis were combined for analysis. There were 19 cases in 1095 patients(1.74%) who received IV antibioitics alone and no infections in 2034 patients who received IV antibiotics and had a vancomycin soaked graft(RR = 0.01,95%CI: 0.001-0.229,P < 0.01).CONCLUSION Prophylactic antibiotics are effective in preventing septic arthritis following simple knee arthroscopy. 11Drying conditions in Switzerland-indication from a 35-year Landsat time-series analysis of vegetation water content estimates to support SDGs显示文摘Exacerbated by climate change,Europe has experienced series of hot and dry summer since the beginning of the 21st century.The importance of land conditions became an international concern with a dedicated sustainable development goal(SDG),the SDG 15.It calls for developing and finding innovative solu-tions to follow and evaluate impacts of changing land condi-tions induced by various driving forces.In Switzerland,drought risk will significantly increase in the coming decades with severe consequences on agriculture,energy production and vegeta-tion.In this paper,we used a 35-year satellite-derived annual and seasonal times-series of normalized difference water index(NDWI)to follow vegetation water content evolution at different spatial and temporal scales across Switzerland and related them to temperature and precipitation to investigate possible responses of changing climatic conditions.Results indicate that there is a small and slow drying tendency at the country scale with a NDWI mean decreasing slope of−0.22%/year for the 23%significant pixels across Switzerland.This tendency is mostly visible below 2000 m above sea level(m.a.s.l.)and in all biogeographical regions.The Southern Alps regions appear to be more responsive to changing drying conditions with a significant and slight negative NDWI trend(−0.39%/year)over the last 35 years.Moreover,NDWI values are mostly a func-tion of temperature at elevations below the tree line rather than precipitation.Findings suggest that multi-annual and seasonal NDWI can be a valuable indicator to monitor vegetation water content at different scales,but other components such as land cover type and evapotranspiration should be considered to better characterize NDWI variability.Satellite Earth Observations data can provide valuable complementary obser-vations for national statistics on the ecological state of vegeta-tion to support SDG 15 to monitor land affected by drying conditions.Charlotte Poussin Alexandrine Massot Christian Ginzler Dominique Weber Bruno Chatenoux Pierre Lacroix Thomas Piller Liliane Nguyen Gregory Giuliani 2021Big Earth Data2021,5,4:1

18测定血清S100蛋白用于诊断克-雅病:前瞻性病例对照研究显示文摘目的:分析克-雅病(Creutzfeldt-Jakob dis-ease,CJD)患者及对照组血清中脑特异性 S100蛋白浓度。设计:前瞻性病例对照研究。研究单位:国家 CJD 监测站。研究对象:224例疑为 CJD 的转诊患者及35例无痴呆的对照病例。主要结果测定:CJD 患者、其他疾病引起的痴呆患者及对照组血清 S100浓度。结果:224例疑为 CJD 的患者中,65例经病理证实确诊;6例有基因突变而肯定诊断;43例很可能患 CJD;36例可能患 CJD;74例患有其他疾病。108例为肯定或很可能 CJD 患者,其血清 S100浓度中位数为395 pg/ml(SD 387 pg/ml)。这明显高于患其他疾病的74例患者(S100浓度中位数为109pg/ml,SDl77pg/ml;P=0.0001)。以213pg/ml为界值,S100测定用于 CJD 诊断的敏感性为77.8%(95%的可信区间为66.8%~85.2%),特异性为81.1%(70.3%~89.3%)。在 Kaplan-Meier 曲线上,不同浓度 S100患者的存活时间差异有显著性(P=0.023)。结论:S100血清浓度测定是一种有价值的工具,在 CJD 的鉴别诊断中,较测定脑脊液更有价值。尚需更多的研究以确定连续测定是否有助于提高此病的诊断准确性。Markus Otto Jens Wiltfang Ekkehard Schtz Ingo Zerr Anke Otto Annette Pfahlberg Olaf Gefeller Manfred Uhr Armin Giese Thomas Weber Hans A Kretzschmar Sigrid Poser 张星虎 1998英国医学杂志中文版1998,0,1:1
