1Gut microbiota,fatty liver disease,and hepatocellular carcinoma显示文摘Intestinal bacteria contribute to the pathogenesis of non-alcoholic fatty liver disease(NAFLD).Recently developed microbial profiling techniques are beginning to shed light on the nature of the changes in the gut microbiota that accompany NAFLD and non-alcoholic steatohepatitis(NASH).In this review,we summarize the role of gut microbiota in the development of NAFLD,NASH,and hepatocellular carcinoma(HCC).We highlight the mechanisms by which gut microbiota contribute to NAFLD/NASH,including through alterations in gut epithelial permeability,choline metabolism,endogenous alcohol production,release of inflammatory cytokines,regulation of hepatic Toll-like receptor(TLR),and bile acid metabolism.In addition,we analyze possible mechanisms for enhanced hepatic carcinogenesis,including alterations in bile acid metabolism,release of inflammatory cytokines,and expression of TLR-4.Finally,we describe therapeutic approaches for NAFLD/NASH and preventive strategies for HCC involving modulation of the intestinal microbiota or affected host pathways.Although recent studies have provided useful information,large-scale prospective studies are required to better characterize the intestinal microbiota and metabolome,in order to demonstrate a causative role for changes in the gut microbiota in the etiology of NAFLD/NASH,to identify new therapeutic strategies for NAFLD/NASH,and to develop more effective methods of preventing HCC.Huikuan Chu Brandon Williams Bernd Schnabl 2018Liver Research2018,2,1:16
2Clinical classification of symptomatic heterotopic pancreas of the stomach and duodenum:A case series and systematic literature review显示文摘BACKGROUND Heterotopic pancreas(HP)is an aberrant anatomic malformation that occurs most commonly in the upper gastrointestinal tract.While the majority of heterotopic pancreatic lesions are asymptomatic,many manifest severe clinical symptoms which require surgical or endoscopic intervention.Understanding of the clinical manifestations and symptoms of HP is limited due to the lack of large volume studies in the literature.The purpose of this study is to review symptomatic cases at a single center and compare these to a systematic review of the literature in order to characterize common clinical manifestations and treatment of this disease.AIM To classify the common clinical manifestations of heterotopic pancreas.METHODS A retrospective review was conducted of pathologic samples containing heterotopic pancreas from 2000-2018.Review was limited to HP of the upper gastrointestinal tract due to the frequency of presentation in this location.Symptomatic patients were identified from review of the medical records and clinical symptoms were tabulated.These were compared to a systematic review of the literature utilizing Pub Med and Embase searches for papers pertaining to heterotopic pancreas.Publications describing symptomatic presentation of HP were selected for review.Information including demographics,symptoms,presentation and treatment were compiled and analyzed.RESULTS Twenty-nine patient were identified with HP at a single center,with six of these identified has having clinical symptoms.Clinical manifestations included,gastrointestinal bleeding,gastric ulceration with/without perforation,pancreatitis,and gastric outlet obstruction.Systemic review of the literature yielded 232 publications detailing symptomatic cases with only 20 studies describing ten or more patients.Single and multi-patient studies were combined to form a cohort of 934 symptomatic patients.The majority of patients presented with abdominal pain(67%)combined with one of the following clinical categories:(1)Dyspepsia,(n=445,48%);(2)Pancreatitis(n=260,28%);(3)Gastrointestinal bleeding(n=80,9%);and(4)Gastric outlet obstruction(n=80,9%).The majority of cases(n=832,90%)underwent surgical or endoscopic resection with 85%reporting resolution or improvement in their symptoms.CONCLUSION Heterotopic pancreas can cause significant clinical symptoms in the upper gastrointestinal tract.Better understanding and classification of this disease may result in more accurate identification and treatment of this malformation.Michael T Le Compte Brandon Mason Keenan J Robbins Motoyo Yano Deyali Chatterjee Ryan C Fields Steven M Strasberg William G Hawkins 2022World Journal of Gastroenterology2022,28,14:3
3Factors associated with refractory pain in emergency patients admitted to emergency general surgery显示文摘BACKGROUND: Oligoanalgesia in emergency departments (EDs) is multifactorial. A previousstudy reported that emergency providers did not adequately manage patients with severe paindespite objective findings for surgical pathologies. Our study aims to investigate clinical andlaboratory factors, in addition to providers’ interventions, that might have been associated witholigoanalgesia in a group of ED patients with moderate and severe pains due to surgical pathologies.METHODS: We conducted a retrospective study of adult patients who were transferred directlyfrom referring EDs to the emergency general surgery (EGS) service at a quaternary academic centerbetween January 2014 and December 2016. Patients who were intubated, did not have adequaterecords, or had mild pain were excluded. The primary outcome was refractory pain, which wasdefi ned as pain reduction <2 units on the 0–10 pain scale between triage and ED departure.RESULTS: We analyzed 200 patients, and 58 (29%) had refractory pain. Patients with refractory painhad signifi cantly higher disease severity, serum lactate (3.4±2.0 mg/dL vs. 1.4±0.9 mg/dL, P=0.001), and lessfrequent pain medication administration (median [interquartile range], 3 [3–5] vs. 4 [3–7], P=0.001), whencompared to patients with no refractory pain. 18Mutations of the brpR and brpS genes affect biofilm formation in Staphylococcus aureus显示文摘BACKGROUND In the United States,Staphylococcus aureus(S.aureus)kills tens of thousands of individuals each year and the formation of a biofilm contributes to lethality.Biofilm-associated infections are hard to treat once the biofilm has formed.A new stilbene drug,labeled SK-03-92,was shown to kill S.aureus and affected transcription of two genes tied to a putative two-component system(TCS)we have named brpR(biofilm regulating protein regulator)and brpS(biofilm regulating protein sensor).AIM To determine if BrpR and BrpS regulate biofilm formation,brpR and brpS mutants were assessed using biofilm assays compared to wild-type S.aureus.METHODS A combination of biofilm and quantitative real-time-polymerase chain reaction assays were used.In addition,bioinformatic software tools were also utilized.RESULTS Significantly more biofilm was created in the brpR and brpS mutants vs wild-type cells.Quantitative real-time polymerase chain reactions showed the brpS mutant had differences in transcription of biofilm associated genes that were eight-fold higher for srtA,two-fold lower for lrgA,and 1.6-fold higher for cidA compared to wild-type.Bioinformatic analysis demonstrated that the S.aureus brpR/brpS TCS had homology to streptococcal late-stage competence proteins involved in celldeath,increased biofilm production,and the development of persister cells.CONCLUSION Our study suggests that brpR/brpS is a TCS that may repress S.aureus biofilm production and be linked to late-stage competence in S.aureus.Allison Zank Lillian Schulte Xavier Brandon Lauren Carstensen Amy Wescott William R Schwan 2022World Journal of Clinical Infectious Diseases2022,12,1:0
19水动力效率最高的鳗鱼推进器(英文)显示文摘It is hypothesized that steady anguilliform swimming motion of aquatic animals is purely reactive such that no net vortex wake is left downstream. This is versus carangiform and tunniform swimming of fish, where vortex streams are shed from tail, fins, and body. But there the animal movements are such to produce partial vortex cancellation downstream in maximizing propulsive efficiency. In anguilliform swimming characteristic of the eel family, it is argued that the swimming motions are configured by the animal such that vortex shedding does not occur at all. However, the propulsive thrust in this case is higher order in the motion amplitude, so that relatively large coils are needed to produce relatively small thrust; the speeds of anguilliform swimmers are less than the carangiform and tunniform, which develop first order thrusts via lifting processes. Results of experimentation on live lamprey are compared to theoretical prediction which assumes the no-wake hypothesis. Two-dimensional analysis is first performed to set the concept. This is followed by three-dimensional analysis using slender-body theory. Slender-body theory has been applied by others in studying anguilliform swimming, as it is ideally suited to the geometry of the lamprey and other eel-like animals. The agreement between this new approach based on the hypothesis of wakeless swimming and the experiments is remarkably good in spite of the physical complexities.William S. Vorus Brandon M.
