| 1 | Do inhaled corticosteroids increase the risk of Pneumocystis pneumonia in people with lung cancer?显示文摘Pneumocystis pneumonia(PCP) is a life-threatening infection in immunocompromised patients. It is relatively uncommon in patients with lung cancer. We report a case of PCP in a 59-year-old man with a past medical history of chronic obstructive pulmonary disease treated with formoterol and a moderate daily dose of inhaled budesonide. He had also advanced stage non-small lung cancer treated with concurrent chemo-radiation with a cisplatin-etoposide containing regimen. The diagnosis of PCP was suspected based on the context of rapidly increasing dyspnea, lymphopenia and the imaging findings. Polymerase chain reaction testing on an induced sputum specimen was positive for Pneumocystis jirovecii. The patient was treated with oral trimethoprim-sulfamethoxazole and systemic corticotherapy and had showed clinical and radiological improvement. Six months after the PCP diagnosis, he developed a malignant pleural effusion and expired on hospice care. Through this case, we remind the importance of screening for PCP in lung cancer patients under chemotherapeutic regimens and with increasing dyspnea. In addition, we alert to the fact that long-term inhaled corticosteroids may be a risk factor for PCP in patients with lung cancer. | 2 | Systematic design method for PI controller with Virtual Resistor-based Active Damping of LCL filter显示文摘The Virtual Resistor based Active Damping(VR-AD) is widely employed in converters connected to the grid via LCL filters in order to mitigate the inherent resonance of the filters. Nevertheless, in digitally controlled systems, the PWM and the calculating delays modify the system characteristics in terms of frequency and phase, thus destabilizing the system and degrading the VR-AD performances, mainly in low switching frequencies. Moreover, the stability of the system is greatly affected under weak grid operation characterized by large grid impedance variation. This paper solves these problems by proposing a systematic, robust and optimized design procedure of voltage oriented PI control(VOC) with VRAD. The considered design procedure ensures robust control(sufficient stability margins) and high quality of grid current(reduced steady-state error and minimized THD value) despite the negative impact of digital time delay, grid impedance variation and filter parameters change. | 3 | Caries-related factors and bacterial composition of supragingival plaques in caries free and caries active Algerian adults显示文摘Objective: To compare oral hygiene practices, education and social background, food intake and oral malodor of Algerian adults suffering from dental caries with normal controls, and to determine and compare the bacterial composition of the supragingival plaques from the above-mentioned groups.Methods: Participants completed a questionnaire and were clinically examined for dental caries using decayed, missing and filled teeth index according to the criteria laid down by the World Health Organization. Supragingival plaque samples were collected from 50 caries-free adults(CF) and 50 caries-active adults(CA). Standard procedures of culture and identification of aerobic and anaerobic bacteria were used. Data were analyzed using Chi-square test.Results: A total of 117 bacterial strains were isolated from supragingival plaques in CF group subjects, 76(64.96%) of them belonged to 9 aerobic genera, and 41(35.04%) to 9anaerobic genera(P < 0.05). While in the second group, 199 strains were isolated, 119(59.80%) of the strains belonged to 10 aerobic genera and 80(40.20%) to 10 anaerobic bacteria(P < 0.05). Streptococcus mutans, Enterococcus faecium, Aerococcus viridans,Actinomyces meyeri, Lactobacillus acidophilus and Eubacterium limosum showed a significantly higher prevalence in the CA group(P < 0.05). The findings revealed that CA group had a high sugar intake(80%).
