| 1 | Pancreatic guidewire placement for achieving selective biliary cannulation during endoscopic retrograde cholangio-pancreatography显示文摘AIM: To investigate the frequency and risk factors for acute pancreatitis after pancreatic guidewire placement (P-GW) in achieving cannulation of the bile duct during endoscopic retrograde cholangio-pancreatography (ERCP). METHODS: P-GW was performed in 113 patients in whom cannulation of the bile duct was difficult. The success rate of biliary cannulation, the frequency and risk factors of post-ERCP pancreatitis, and the frequency of spontaneous migration of the pancreatic duct stent were investigated. RESULTS: Selective biliary cannulation with P-GW was achieved in 73% of the patients. Post-ERCP pancreatitis occurred in 12% (14 patients: mild, 13; moderate, 1). Prophylactic pancreatic stenting was attempted in 59% of the patients. Of the 64 patients who successfully underwent stent placement, three developed mild pancreatitis (4.7%). Of the 49 patients without stent placement, 11 developed pancreatitis (22%: mild, 10; moderate, 1). Of the five patients in whom stent placement was unsuccessful, two developed mild pancreatitis. Univariate and multivariate analyses revealed no pancreatic stenting to be the only significant risk factor for pancreatitis. Spontaneous migration of the stent was observed within two weeks in 92% of the patients who had undergone pancreatic duct stenting.CONCLUSION: P-GW is useful for achieving selective biliary cannulation. | 2 | A novel animal model for in vivo study of liver cancer metastasis显示文摘AIM:To establish an animal model with human hepatocyte-repopulated liver for the study of liver cancer metastasis.METHODS:Cell transplantation into mouse livers was conducted using alpha-fetoprotein(AFP)-producing hu-man gastric cancer cells(h-GCCs) and h-hepatocytes as donor cells in a transgenic mouse line expressing urokinase-type plasminogen activator(uPA) driven by the albumin enhancer/promoter crossed with a severe combined immunodeficient(SCID) mouse line(uPA/SCID mice).Host mice were divided into two groups(A and B).Group A mice were transplanted with h-GCCs alone,and group B mice were transplanted with h-GCCs and h-hepatocytes together.The replacement index(RI),which is the ratio of transplanted h-GCCs and h-hepatocytes that occupy the examined area of a histological section,was estimated by measuring h-AFP and h-albumin concentrations in sera,respectively,as well as by immunohistochemical analyses of h-AFP and human cytokeratin 18 in histological sections.RESULTS:The h-GCCs successfully engrafted,repopulated,and colonized the livers of mice in group A(RI = 22.0% ± 2.6%).These mice had moderately differentiated adenocarcinomatous lesions with disrupted glandular structures,which is a characteristics feature of gastric cancers.The serum h-AFP level reached 211.0 ± 142.2 g/mL(range,7.1-324.2 g/mL).In group B mice,the h-GCCs and h-hepatocytes independently engrafted,repopulated the host liver,and developed colonies(RI = 12.0% ± 6.8% and 66.0% ± 12.3%,respectively).h-GCC colonies also showed typical adenocarcinomatous glandular structures around the h-hepatocyte-colonies.These mice survived for the full 56 day-study and did not exhibit any metastasis of h-GCCs in the extrahepatic regions during the observational period.The mice with an h-hepatocyte-repopulated liver possessed metastasized h-GCCs and therefore could be a useful humanized liver animal model for studying liver cancer metastasis in vivo.CONCLUSION:A novel animal model of human liver cancer metastasis was established using the uPA/SCID mouse line.This model could be useful for in vivo testing of anti-cancer drugs and for studying the mechanisms of human liver cancer metastasis. | Shinsuke Fujiwara Hikaru Fujioka Chise Tateno Ken Taniguchi Masahiro Ito Hiroshi Ohishi Rie Utoh Hiromi Ishibashi Takashi Kanematsu Katsutoshi Yoshizato | 2012 | World Journal of Gastroenterology2012，18，29： | 6 |
| 3 | Searching for a treatment for Alport syndrome using mouse models显示文摘Alport syndrome(AS) is a hereditary nephritis caused by mutations in COL4A3,COL4A4 or COL4A5 encoding the type IV collagen α3,α4,and α5 chains,which are major components of the glomerular basement membrane.About 20 years have passed since COL4A3,COL4A4,and COL4A5 were identified and the first Alport mouse model was developed using a knockout approach.The phenotype of Alport mice is similar to that of Alport patients,including characteristic thickening and splitting of the glomerular basement membrane.Alport mice have been widely used to study the pathogenesis of AS and to develop effective therapies.In this review,the newer therapies for AS,such as pharmacological interventions,genetic approaches and stem cell therapies,are discussed.Although some stem cel therapies have been demonstrated to slow the rena disease progression in Alport mice,these therapies demand continual refinement as research advances.In terms of the pharmacological drugs,angiotensin-converting enzyme inhibitors have been shown to be effective in Alport mice.Novel therapies that can provide a better outcome or lead to a cure are still awaited. | Kan Katayama Shinsuke Nomura Karl Tryggvason Masaaki Ito | 2014 | World Journal of Nephrology2014，3，4： | 5 |
| 4 | Efficacy of a newly developed dilator for endoscopic ultrasound-guided biliary drainage显示文摘AIM To evaluate the efficacy of a newly developed dilator for endoscopic ultrasound(EUS)-guided drainage(ES Dilator).METHODS Fourteen consecutive patients who had undergone EUSguided choledochoduodenostomy(EUS-CDS) with the ES Dilator were identified from a prospectively maintained database and enrolled in the study group.Fourteen other patients who had undergone EUS-CDS without the dilator just prior to its introduction were analyzed as the control group.A historical cohort study was carried out comparing the two groups.The main outcome measurement was the procedure time.The technical success rate and early AE rate were also compared between the two groups.RESULTS There were no significant differences in age,sex and etiology of biliary obstruction.The utilization rate of a plastic stent was higher in the control group(36% vs 0%).The technical success rate was 100% in both groups.The mean procedure time was significantly shorter in the study group than in the control group(27 ± 7 min vs 44 ± 26 min,P = 0.026).Additionally,there were no patients who required more than 40 min for the procedure in the study group.Early adverse events occurred in 29%(4/14) of the control group whereas none in the study group.The adverse events in all 4 patients was bile peritonitis,including pan-peritonitis in one patient.All patientsrecovered with conservative treatment by medication.CONCLUSION The newly developed dilator was found to be useful for shortening procedure time and would prevent adverse events related to bile leakage in EUS-CDS. | Yoshihide Kanno Kei Ito Shinsuke Koshita Takahisa Ogawa Kaori Masu Yoshiharu Masaki Yutaka Noda | 2017 | World Journal of Gastrointestinal Endoscopy2017，9，7： | 2 |

| 7 | Pneumatosis intestinalis with obstructing intussusception:A case report and literature review显示文摘Pneumatosis intestinalis(PI) often represents a benign condition that should not be considered as an argument for surgery. We report a patient with PI and obstructing intussusception who underwent urgent colectomy and review the literatures regarding PI with intussusception. A 20-year-old man presented at our hospital with a 3-d intermittent lower abdominal pain history. He underwent steroid therapy for membranoproliferative glomerulonephritis for 4 years. Computed tomography revealed ascending colon intussusception with air within the wall. Intraoperative colonoscopy revealed numerous soft polypoid masses with normal overlying mucosa and right hemicolectomy was performed. Histological examination of colonic wall sections revealed large cysts in the submucosal layer. The pathological diagnosis was PI. Nine cases of intussusception associated with primary PI have been reported.
