| 1 | 美国国家卒中协会短暂性脑缺血发作处理指南显示文摘目的:短暂性脑缺血发作(TIA)是常见和重要的卒中先兆。其处理方法多样,大多数发表的指南在近年来均未被更新。我们试图制定一个全面的、无偏倚的、循证的TIA处理指南。方法:根据一种客观标准(可以预测在该研究领域中执业医师对专家提名的文献测量学方法)挑选出15名专家组成员。通过系统回顾检索到过去发表的指南,由专家对其中的推荐意见的质量进行独立评价。选择出质量最高的推荐意见,然后让专家组采用改良Delphi法通过多次问卷调查的方式进行修订,从而对新的修改达成共识。给专家们提供近期临床研究的系统评价,要求专家根据新的证据判断措辞修改的合理性并且根据证据等级和质量对最终的推荐意见进行评定。不允许专家在预期有可能存在任何利益冲突的专题方面提出推荐意见。结果:通过系统回顾检索到257个指南,其中有13篇文献包括的137条推荐意见符合所有纳入标准。需要6次重复的问卷调查对53条最终推荐意见的措辞达成共识。最终的推荐意见涉及TIA的初步处理、评价、内科治疗、外科治疗和危险因素控制。结论:对TIA患者医疗诊治的最终推荐意见强调了紧急评价和治疗的重要性。这种用于制定本指南的新方法是可行的,考虑到了快速更新并能减少偏倚。 | S. | 2 | Sperm retrieval success and testicular histopathology in idiopathic nonobstructive azoospermia显示文摘Prior studies have investigated sperm retrieval rates in men with non obstructive azoospermia(NOA)secondary to specific etiologies,yet most cases of NOA are idiopathic.We compared sperm retrieval rates and testicular histopathology in idiopathic NOA(iNOA)and nonidiopathic NOA(niNOA).We performed a retrospective review of men with NOA who underwent microdissection testicular sperm extraction(microTESE)between 2000 and 2016.Men with no history of malignancy or cryptorchidism and negative genetic evaluation were considered idiopathic.Multivariable regression determined the association between idiopathic etiology and primary outcomes of sperm retrieval and active spermatogenesis on histopathology.Among 224 men,86(38.4%)were idiopathic,75(33.5%)were nonidiopathic,and 63(28.1%)did not undergo genetic testing.Median age and serum testosterone were higher among iNOA or no testing versus niNOA.Median follicle-stimulating hormone(FSH)was lower among iNOA or no testing versus niNOA.A higher proportion of iNOA or no testing versus niNOA had a clinical varicocele.Sperm retrieval rates were similar between iNOA,niNOA,and no testing(41.8%vs 48.0%vs 55.6%,respectively;P=0.255).Active spermatogenesis was seen in a higher proportion of iNOA or no testing versus niNOA(31.4%and 27.0%vs 16.0%,P=0.073).On multivariable analysis,iNOA was not associated with sperm retrieval or spermatogenesis(P=0.430 and P=0.078,respectively).Rates of sperm retrieval and spermatogenesis on testis pathology were similar in men with iNOA and niNOA.These data will be useful to clinicians in preoperative counseling for men with NOA and negative genetic evaluation. | Arighno Das Joshua A Halpern Annie L Darves-Bornoz Mehul Patel James Wren Mary Kate Keeter Robert E Brannigan | 2020 | Asian Journal of Andrology2020，22，6： | 3 |

| 5 | Low prevalence of epilepsy and onchocerciasis after more than 20 years of ivermectin treatment in the Imo River Basin in Nigeria显示文摘Background:High epilepsy prevalence and incidence have been reported in areas with high onchocerciasis transmission.Recent findings suggest that proper community-directed treatment with ivermectin(CDTI)is potentially able to prevent onchocerciasis-associated epilepsy(OAE).We assessed the epilepsy prevalence and onchocerciasis transmission in two Nigerian villages following more than 20 years of CDTI.Methods:A cross-sectional door-to-door survey was performed in two villages in the Imo River Basin reported to be mesoendomic for onchocerciasis(Umuoparaodu and Umuezeala).Individuals were screened for epilepsy using a validated 5-item questionnaire.Persons suspected to have epilepsy were examined by a neurologist or a physician with training in epilepsy for confirmation.Onchocerciasis was investigated via skin snip microscopy and rapid diagnostic tests for Ov16 antibodies.Results were compared with previous findings from the Imo river basin.Results:A total of 843 individuals from 257 households in the two villages were encountered.We detected four persons with epilepsy(PWE)giving a crude epilepsy prevalence of 0.5%.This finding differs from observations reported 14 years ago which showed an epilepsy prevalence of 2.8%in the neighbouring village of Umulolo(P=0.0001),and 1.2%from 13 villages in the Imo river basin(P=0.07).The seroprevalence of Ov16 antibodies was found to be 0%.Only 4.6%of skin snips were positive compared to 26.8%in previous surveys(P<0.0001).Ivermectin mass distribution coverage in the study sites in 2017 was 79.7%.Conclusions:A low epilepsy and onchocerciasis prevalence was observed following more than 20 years of CDTI in the Imo River Basin.Absence of Ov16 antibodies indicates minimal transmission of onchocerciasis.These results contrast with observations from areas of high onchocerciasis transmission,where epilepsy prevalence and incidence remain high.Findings from this study suggest that sustained efforts could eventually achieve elimination of onchocerciasis in these villages. | Joseph N.F.Siewe Chinyere N.Ukaga Ernest O.Nwazor Murphy O.Nwoke Modebelu C.Nwokeji Blessing C.Onuoha Simon O.Nwanjor Joel Okeke Kate Osahor Lilian Chimechefulam Ann I.Ogomaka Augustine A.Amaechi Chika I.Ezenwa Monika N.Ezike Chidimma I.kpeama Ogechi Nwachukwu Austine I.Eriama-Joseph Berthram E.B.Nwoke Robert Colebunders | 2019 | Infectious Diseases of Poverty2019，8，1： | 2 |
| 6 | Differentiation of malignant and benign proximal bile duct strictures: The diagnostic dilemma显示文摘AIM: To identify the criteria for the differentiation of hilar cholangiocarcinoma (HCCA) from benign strictures. METHODS: A total of 68 patients underwent resection of lesions suspicious for HCCA between 1998 and 2006. The results of laboratory investigations, imaging studies and brush cytology were collected. These fi ndings were analyzed to obtain the fi nal diagnosis. RESULTS: Histological examination of the resected specimens confirmed HCCA in 58 patients (85%, group Ⅰ) whereas 10 patients (15%, groupⅡ) were diagnosed to have benign strictures. The most common presenting symptom was obstructive jaundice in 77% patients (79% group Ⅰvs 60% group Ⅱ P = 0.23). Laboratory findings showed greater elevation of transaminase levels in group Ⅰcompared to group Ⅱ The various imaging modalities showed vascular involvement exclusively in the malignant group (36%,P < 0.05). Brush cytology was positive for malignant cells in only 50% patients in group Ⅰ whereas none in group Ⅱ showed malignant cells. CONCLUSION: Despite improvements in imaging techniques, 10 patients (15%) with a presumptive diagnosis of HCCA were ultimately found to have benign strictures. Except for vascular involvement which was associated significantly with malignancy, there were no conclusive features of malignancy on regular imaging modalities. | 18 | Visual and refractive outcomes following implantation of a new trifocal intraocular lens显示文摘Background:Independence from all optical aids,and freedom from unwanted symptoms,following cataract and lens surgery remains the ultimate goal of both patient and surgeon.The development of trifocal IOL technology provides an ever-increasing range of options.The purpose of our study is to understand the predictability,safety and efficacy of a new trifocal intraocular lens(IOL)following cataract or refractive lens exchange(RLE)surgery.Methods:This was a retrospective consecutive case series of patients undergoing cataract extraction or RLE followed by implantation of the Alcon IQ Panoptix IOL.Pre and postoperative refractive and visual parameters were recorded and evaluated.As the cohort followed a normal distribution,standard parametric tests were used.Paired t-test was used to compare the difference between target and postoperative refractive errors.The incidence of intraoperative and postoperative complications was also reported.Results:The IOL was implanted in 66 eyes of 33 patients.Mean postoperative spherical equivalent(SE)refraction was-0.08±0.25 dioptres(D).This was not significantly different from the target refraction(p=0.841).Sixty-five percent of patients were within±0.25 D of the target SE refraction with 100%within±0.50 D of intended correction.Mean postoperative uncorrected distance visual acuity(UDVA)was 0.01±0.10 LogMAR.All patients achieved an unaided distance acuity of 20/40 or better postoperatively.Binocularly,100%saw 0.20 LogMAR or better at near without correction and 88.9%achieved this level for uncorrected intermediate visual acuity.No intraoperative complications were noted.Five patients complained of moderate haloes in the early postoperative period.Conclusion:The AcrySof IQ Panoptix IOL provides functional uncorrected visual acuity at distance,intermediate and near positions.Our results remain equivalent with existing trifocal IOL outcomes and provide surgeons with a further IOL alternative for the patient motivated to obtain true spectacle independence.Surgeons should consider individual reading and working requirements when counselling patients preoperatively to optimise postoperative patient satisfaction. | Michael Lawless Chris Hodge Joe Reich Lewis Levitz Uday K.Bhatt Colm McAlinden Kate Roberts Timothy V.Roberts | 2017 | Eye and Vision2017，4，1： | 1 |
