1Etiology and therapeutic management of neonatal jaundice in Iran:a systematic review and meta-analysis显示文摘Background Jaundice is a life-threatening disorder in the neonates.In the present study,we aimed to assess systematically available evidence on causes and management of jaundice in Iranian newborn patients.Methods We searched the databases of PubMed,Web of Sciences,Scopus and Google Scholar for English articles published since inception until May 2019.A search was also done for Persian articles in Magiran and Scientific Information Database.Studies were evaluated based on predefined criteria by two reviewers.Data analysis was performed by STATA software.Results A total of 33 articles were finally included.The overall pooled prevalence of causes of jaundice among Iranian neonates was as follows:ABO blood groups incompatibility,16.9%[95%confidence interval(CI)10.9-22.8];Rh blood group incompatibility,4%(95%CI 2.5-5.5);ABO and Rh blood groups incompatibility,3.6%(95%CI 0-7.7);glucose6-phosphate dehydrogenase(G6PD)deficiency,6.3%(95%CI 5.1-7.5);infection,6.6%(95%CI 5.2-8.1);hypothyroidism,4.2%(95%CI 0.1-8.3);infant of diabetic mother:2.3%(95%CI 0.1-4.5);unknown,50.7%(95%CI 33.4-68);cephalohematoma,0.6%(95%CI 0.3-0.9).Regarding treatment of icterus,seven and eight articles were found on phototherapy and exchange transfusion,respectively.In five studies,all patients underwent phototherapy,but rate of exchange transfusion use was between 6.6%and 50.9%.Conclusions According to the results,unknown factors were the most common causes of icterus in Iranian neonates,followed by ABO blood groups incompatibility,infections and G6PD deficiency.By the way,phototherapy and exchange transfusion were found as therapeutic choices of neonatal jaundice.Yadollah Zahed Pasha Shaghayegh Alizadeh-Tabari Ermia Zahed Pasha Mohammad Zamani 2020World Journal of Pediatrics2020,16,5:23
2Resistance of Helicobacter pylori to furazolidone and levofloxacin: A viewpoint显示文摘In their review, Arslan et al[1] did not describe the status of Helicobacter pylori(H. pylori) treatment with furazolidone and the resistance to this antibiotic. We have presented different surveys showing the resistance of H. pylori to furazolidone from Asia and South America. The resistance rates varied but were mostly low(< 5%). There are not enough data on its efficacy and resistance in the United States and Europe. H. pylori mutations occurring in the oor D gene, including A041 G, A122 G, C349A(G), A78 G, A112 G, A335 G, C156 T and C165 T, and in the por D gene, including G353 A, A356 G, C357 T, C347 T, C347 G and C346 A, have been indicated to be possibly related to the observed resistance. Additionally, to complete Arslan et al 's statement regarding levofloxacin resistance, it should be noted that compound mutations of N87 A, A88 N and V65 I at codon Asn-87 were recently observed in the gyr A gene for the first time. 3Solvent extraction-separation of La(III),Eu(III) and Er(III) ions from aqueous chloride medium using carbamoyl-carboxylic acid extractants显示文摘N,N-dibutyldiglycol amic acid(HLI) and N,N-dioctyldiglycol amic acid(HLII) were synthesized and characterized by conventional spectroscopic methods. These molecules were examined as extractants for extraction-separation of La(III), Eu(III) and Er(III), as representative ions of light, middle and heavy rare earths, from aqueous chloride solutions. The analysis of the extraction equilibria revealed that the extracted species of lanthanum and europium ions by both of the extractants had a 1:3 metal to ligand ratio. It was suggested that erbium ions were extracted into the organic phase via the formation of Er(LI or II)2Cl complexes. The effect of the organic diluents on the extraction-separation efficiency of the studied rare earths by HLI and HLII was investigated by comparing the results obtained in dichloromethane and carbon tetrachloride. Regardless to the diluent used, the order of selectivity presented by the investigated extractants was Er(III)>Eu(III)>La(III). It is noteworthy that, a significant enhancement in separation of the studied rare earths by the extractants was achieved in their competitive extraction experiments with respect to that obtained in single component extraction experiments. 4Prognostic value of cortisol and thyroid function tests in poisoned patients admitted to toxicology ICU显示文摘BACKGROUND: Prognostic value of cortisol and thyroid function tests(TFTs) has previously been evaluated in medical ICUs. We aimed to evaluate prognostic efficacy of cortisol and TFTs in critically ill poisoned patients admitted to toxicology intensive care unit(ICU).METHODS: In a prospective study of consecutively enrolled subjects admitted to the toxicology ICU, lab analyses included TFTs(total T3 and T4 as well as TSH) and cortisol levels drawn between 8 am–10 am during period of the first 24 hours post-ingestion/exposure. Simplified Acute Physiology Score Ⅱ(SAPS Ⅱ) and Acute Physiology and Chronic Health Evaluation Ⅱ(APACHE Ⅱ) were recorded. All scores were compared to detect the best prognostic factor. Type of poisoning was also included.RESULTS: In 200 patients evaluated, 129 were male and mean age was 31 years. In general, SAPS Ⅱ, T4, and cortisol could prognosticate death. After regression analysis, only cortisol had such efficacy(P=0.04; OR=1.06; 95%CI=1.05–1.08; cut-off=42 μg/d L; sensitivity=70%; specificity=82%). Between aluminium phosphide(ALP)-and non ALP-poisoned patients, level of consciousness, mean arterial pressure, and cortisol level could prognosticate death in ALP poisoning(all Ps<0.001 in both uni and multivariate analyses). Median(interquartile range; IQR) GCS was 7(6, 10) and 15(8, 15) in non-ALP and ALP-poisoned patients(P<0.003). SAPS Ⅱ and APACHE Ⅱ could not prognosticate death at all.CONCLUSION: Cortisol best prognosticated outcomes for subjects admitted to the toxicology ICU. 5Deactivation studies of bifunctional Fe-HZSM5 catalyst in Fischer-Tropsch process显示文摘A physical mixture of alkali-promoted iron catalyst with binder based on Fischer-Tropsch synthesis and an acidic co-catalyst (HZSM5) for syngas conversion to hydrocarbons was studied in a fixed bed micro reactor. Deactivation data were obtained during the synthesis over a 1400 h period. The deactivation studies on iron catalyst showed that this trend followed the phase transformation Fe2.2C ( ε′) → Fe5C2 (χ) → Fe3C (θ), and the final predominant phase of the catalyst was Fe3C (θ). Deactivation of zeolite component in bifunctional catalyst may be caused by coking over the zeolitic component, dealumination of zeolite crystals, and migration of alkali promoters from iron catalyst under synthesis conditions. 16Effects of Zataria oxymel on obesity, insulin resistance and lipid profile:A randomized, controlled, triple-blind trial显示文摘Background: Obesity is a major public health problem and its occurrence is markedly increasing in developed and developing countries. However, few studies have investigated the use of natural products to treat obesity. The effects of taking a combination of oxymel and Zataria multiflora Boiss.(ZM), herein referred to as Zataria oxymel(ZO), on obesity, lipid profile and insulin resistance have not yet been studied.Objective: This study evaluates the effects of oxymel and ZO on obesity, lipid profile and insulin resistance.Design, setting, participants and interventions: In this randomized, controlled, triple-blind trial, overweight patients were randomly divided into three groups and received doses of study compounds twice per day for twelve weeks. Group A received 0.75 g ZM in 10 mL oxymel in each treatment;group B received 1.5 g ZM in 10 m L of oxymel in each treatment and group C(control) only received 10 mL of oxymel in each treatment.Main outcome measures: Anthropometric parameters, including body mass index(BMI), waist circumference and hip circumference, were measured at the time of registration. Blood tests were carried out at the beginning and once again at end of the study. Blood parameters included fasting blood sugar(FBS), insulin levels, serum lipid profile(total cholesterol, triglyceride, low-density lipoprotein cholesterol and highdensity lipoprotein cholesterol) and liver enzymes(aspartate transaminase and alanine transaminase).Serum creatinine was also measured at the beginning of the project and in monthly intervals for three months. The homeostasis model assessment index was calculated as fasting insulin(μIU/mL)×FBS(mg/dL)/405.Results: The results showed that patients receiving ZO experienced significant reduction in waist circumference in groups A, B and C, respectively(P < 0.001) but no significant change in BMI. Group A also experienced reduction in hip circumference(P = 0.01). Groups B and C had reduction in the homeostatic model assessment of insulin resistance(P = 0.05 and P = 0.01, respectively), with no significant reduction in FBS.No effect on lipid profile, liver enzymes or serum creatinine was observed in the three groups.Conclusion: In this study, treatment with ZO and oxymel reduced insulin resistance, and waist and hip circumferences in overweight patients. 19Applied Linguistics with Mixed Leader Optimizer Based English Text Summarization Model显示文摘The term‘executed linguistics’corresponds to an interdisciplinary domain in which the solutions are identified and provided for real-time language-related problems.The exponential generation of text data on the Internet must be leveraged to gain knowledgeable insights.The extraction of meaningful insights from text data is crucial since it can provide value-added solutions for business organizations and end-users.The Automatic Text Summarization(ATS)process reduces the primary size of the text without losing any basic components of the data.The current study introduces an Applied Linguistics-based English Text Summarization using a Mixed Leader-Based Optimizer with Deep Learning(ALTS-MLODL)model.The presented ALTS-MLODL technique aims to summarize the text documents in the English language.To accomplish this objective,the proposed ALTS-MLODL technique pre-processes the input documents and primarily extracts a set of features.Next,the MLO algorithm is used for the effectual selection of the extracted features.For the text summarization process,the Cascaded Recurrent Neural Network(CRNN)model is exploited whereas the Whale Optimization Algorithm(WOA)is used as a hyperparameter optimizer.The exploitation of the MLO-based feature selection and the WOA-based hyper-parameter tuning enhanced the summarization results.To validate the perfor-mance of the ALTS-MLODL technique,numerous simulation analyses were conducted.The experimental results signify the superiority of the proposed ALTS-MLODL technique over other approaches.Hala J.Alshahrani Khaled Tarmissi Ayman Yafoz Abdullah Mohamed Manar Ahmed Hamza Ishfaq Yaseen Abu Sarwar Zamani Mohammad Mahzari 2023Intelligent Automation & Soft Computing2023,,6:0
