| 1 | PCSK9 inhibitors: A new era of lipid lowering therapy显示文摘Hyperlipidemia is a well-established risk factor for developing cardiovascular disease(CVD). The recent American College of Cardiology and American Heart Association guidelines on lipid management emphasize treatment of individuals at increased risk for developing CVD events with 3-hydroxy-3-methylglutaryl coenzyme A reductase inhibitors(statins) at doses proven to reduce CVD events. However, there are limited options for patients who are either intolerant to statin therapy, develop CVD despite being on maximally tolerated statin therapy, or have severe hypercholesterolemia. Recently the Food and Drug Administration approved two novel medications for low-density lipoprotein(LDL)-cholesterol reduction: Evolocumab and Alirocumab. These agents target and inactivate proprotein convertase subtilsinkexin type 9(PCSK9), a hepatic protease that attaches and internalizes LDL receptors into lysosomes hence promoting their destruction. By preventing LDL receptor destruction, LDL-C levels can be lowered 50%-60% above that achieved by statin therapy alone. | 2 | Anti-arthritis activity of roots of Hemidesmus indicus R.Br.(Anantmul)in rats显示文摘Objective:To investigate the protective effects of hydroalcoholic and its fractions from roots of Hemidesmus indicus on arthritis in in vitro models of rodents.Methods:Preliminary phytochemical analysis and thin-layer chromoatography were performed to analyze constituents of hydroalcoholic extract and its three fraction namely ethyl acetate fraction,chloroform fraction and residual fraction of mot of Hemidesmus indicus.Arthritis rats models were established by Complete Freund’s Adjuvant.The parameters including paw edema,body weight,arthritic index,erythrocyte sedimentation rate,serum rheumatoid factor,serum C-reactive protein,serum nitrite level,and histopathology of synovial joints were observed.Methotrexate was taken as positive control.Results:Rats treated with hydroalcoholic extract(450 mg/kg,p.o.),ethyl acetate (75 mg/kg,p.o.),chloroform(60 mg/kg,p.o) and residual fractions(270 mg/kg,p.o.),showed significant decrease in physical and biochemical parameters compared with arthritic model rats. Hydroalcoholic extract and its ethyl acetate fraction of Hemidesmus indicus showed significantly higher anti-arthritic activity than chloroform and residual fraction.Histopathological analysis demonstrated that both of hydroalcoholic extract and its ethyl acetate fraction had comparable anti-arthritic activity with methotrexates.Conclusions:The present study suggests that Hemidesmus indicus has protective activity against arthritis and the activity might be attributed to presence of terpenoid in hydroalcoholic extract,as well as in ethyl acetate fraction. | Alka Mehta Neeraj K Sethiya Chetan Mehta GB Shah | 2012 | Asian Pacific Journal of Tropical Medicine2012,5,2: | 4 |
| 3 | Erythropoietin therapy after out-of-hospital cardiac arrest:A systematic review and meta-analysis显示文摘AIM To assess safety and efficacy of early erythropoietin(Epo) administration in patients with out-of-hospital cardiac arrest(OHCA).METHODS A systematic literature search was performed using PubM ed,MEDLINE,EMBASE,EBSCO,CINAHL,Web of Science and Cochrane databases,of all studies published from the inception through October 10,2016.Inclusion criteria included:(1) Adult humans with OHCA and successful sustained return of spontaneous circulation;and(2) studies including mortality/brain death,acute thrombotic events as their end points.Primary efficacyoutcome was 'brain death or Cerebral Performance Category(CPC) score of 5'.Secondary outcomes were 'CPC score 1,and 2-4','overall thrombotic events' and 'acute coronary stent thrombosis'.RESULTS We analyzed a total of 606 participants(n = 276 received Epo and n = 330 with standard of care alone) who experienced OHCA enrolled in 3 clinical trials.No significant difference was observed between the Epo and no Epo group in brain death or CPC score 5(OR = 0.77;95%CI:0.42-1.39),CPC score 1(OR = 1.16,95%CI:0.82-1.64),and CPC score 2-4(OR = 0.77,95%CI:0.44-1.36).Epo group was associated with increased thrombotic complications(OR = 2.41,95%CI:1.26-4.62) and acute coronary stent thrombosis(OR = 8.16,95%CI:1.39-47.99).No publication bias was observed.CONCLUSION Our study demonstrates no improvement in neurological outcomes and increased incidence of thrombotic events and acute coronary stent thrombosis in OHCA patients who were treated with Epo in addition to standard therapy. | Rahul Chaudhary Jalaj Garg Parasuram Krishnamoorthy Kevin Bliden Neeraj Shah Nayan Agarwal Rahul Gupta Abhishek Sharma Karl B Kern Nainesh C Patel Paul Gurbel | 2017 | World Journal of Cardiology2017,9,12: | 2 |

| 7 | Safety & efficacy of single subconjunctival triamcinolone 5 mg depot vs topical loteprednol post cataract surgery: less drop cataract surgery显示文摘AIM: To do a randomized prospective interventional study for comparing the effects of a single subconjunctival triamcinolone acetonide(SCTA) injection to tapering topical loteprednol in patients undergoing phacoemulsification surgery under topical anesthesia. METHODS: A total of 400 patients were randomized into 2 groups; Group A(200 patients) received 5 mg SCTA at the end of surgery and topical ketorolac tromethamine(0.5%) with ofloxacin(0.3%) combination for 3 wk. Group B(200 patients) received tapering topical loteprednol etabonate(0.5%) along with ofloxacin(0.3%) and ketorolac tromethamine(0.5%) for 3 wk. Outcomes evaluated were intraocular pressure(IOP), anterior chamber cells/flare and macular oedema postoperatively at 1, 6 and 12 wk.RESULTS: Baseline parameters were almost similar in both the groups. No statistical difference was seen between the preoperative and postoperative IOP values for Group A(P=0.82) and Group B(P=0.61) and postoperative IOP values in between both groups(P=0.14) at 1 wk. Incidence of cells/flare postoperative was statistically not significant(P=0.82) in both groups at all follow up visits. Postoperative macular oedema was not observed at any follow up visit. | 8 | Medical malpractice litigation involving otolaryngology residents and fellows:A case-based 30-year review显示文摘BACKGROUND Errors,misdiagnoses,and complications can occur while trainees are involved in patient care.Analysis of such events could reveal areas for improvement by residency and fellowship programs.AIM To examine lawsuits tried at the state and federal level involving otolaryngology trainees.METHODS The LexisNexis database,an online legal research database containing state and federal case records from across the United States,was retrospectively reviewed for malpractice cases involving otolaryngology residents or fellows from January 1,1990 to December 31,2020.Case data collected:Plaintiff/trainee/defendant characteristics,allegations,medical outcomes,and legal outcomes.RESULTS Over the study period,20 malpractice lawsuits involving otolaryngology trainees were identified.Plaintiffs raised numerous allegations including procedural error(n=12,25.5%),incorrect diagnosis and/or treatment(n=8,17.0%),and lack of knowledge of trainee involvement(n=6,12.8%).Nine cases(45%)had verdicts in favor of the plaintiff,whereas 5 cases(25%)had verdicts in favor of the defense.Six cases(30%)ended in a settlement.Awards to plaintiffs were heterogenous,with a median of$617,500(range$32K-17M)for settled cases and verdicts favoring plaintiffs.CONCLUSION The findings enclosed herein represent the first published analysis of trainee involvement in otolaryngology malpractice cases held at the state/federal level.Otolaryngology trainees can be involved in lawsuits for both procedural and nonprocedural events.This study highlights the importance of education specifically in the domains of procedural errors,informed consent,proper diagnosis/management,and clear communication within patient care teams.Training programs should incorporate these study findings into effective simulation courses and didactic sessions.Educating trainees about common pitfalls holds the promise of decreasing healthcare systems costs,reducing trainee burnout,and,most importantly,benefiting patients. | Neeraj V Suresh Viraj N Shah Christian G Fritz Jessica R Griff Shreni Shah Arjun Watane Ravi S Parikh Elizabeth A Nicolli | 2022 | World Journal of Otorhinolaryngology2022,9,1: | 0 |
| 9 | Effect of westernization on oral health among college students of Udaipur City,India显示文摘Background:There is overwhelming evidence that periodontal disease and dental caries affect the majority of populations and that western culture and lifestyle may have a profound influence on oral health,especially in adults.The present study was performed to determine the effect of westernization on the oral health of college students of Udaipur City,Rajasthan.Methods:A descriptive cross-sectional study was conducted among students attending various professional and non-professional bachelor's degree colleges of Udaipur City,Rajasthan,India,from March 2013 to May 2013.Eight hundred students were selected based on a two-stage random sampling procedure.Westernization was assessed by a self-administered structured questionnaire.Periodontal status,dental caries status and malocclusion were assessed according to World Health Organization(WHO) criteria(1997).Statistical analysis was performed using Chi-square and Multivariate logistic regression.The confidence level and level of significance were set at 95% and 5%,respectively.Results:The present study suggested that adverse habits,listening to English music and preferring English food had a significant association with dental caries and periodontal diseases.Malocclusion also showed a significant relationship with consuming English food for snacks and desserts.Multivariate analysis revealed a significantly greater odds ratio(OR) for periodontal disease and dental caries among those who preferred English food for lunch.Conclusion:Based on the results of the present study,there is an association between westernization and oral health. | Piyush Pujara Neeraj Sharma Rujul Jayeshkumar Parikh Maitri Shah Shachi Parikh Vivek Vadera Manpreet Kaur Isha Makkar Mayur Parmar Pratik Rupakar Shrikant Patel | 2016 | Journal of Medical Colleges of PLA(China)2016,31,4: | 0 |
