1Non-invasive diagnosis of gastric mucosal atrophy in an asymptomatic population with high prevalence of gastric cancer显示文摘AIM: To validate a non-invasive method to detect gastric mucosal atrophy in a Chilean population with high prevalence of gastric cancer and a poor survival rate. METHODS: We first determined the optimal cut-off level of serum pepsinogen (PG)-1, PG-1/PG-2 ratio and 17-gastrin in 31 voluntary symptomatic patients (mean age: 66.1 years), of them 61% had histologically confirmed gastric atrophy. Then, in a population-based sample of 536 healthy individuals (209 residents in counties with higher relative risk and 327 residents in counties with lower relative risk for gastric cancer), we measured serum anti-H pylori antibodies, PG and 17-gastrin and estimated their risk of gastric cancer. RESULTS: We found that serum PG-1 < 61.5 μg/L, PG-1/PG-2 ratio < 2.2 and 17-gastrin > 13.3 pmol/L had a high specificity (91%-100%) and a fair sensitivity (56%-78%) to detect corpus-predominant atrophy. Based on low serum PG-1 and PG-1/PG-2 ratio together as diagnostic criteria, 12.5% of the asymptomatic subjects had corpus-predominant atrophy (0% of those under 25 years and 20.2% over 65 years old). The frequency of gastric atrophy was similar (12% vs 13%) but H pylori infection rate was slightly higher (77% vs 71%) in the high-risk compared to the low-risk counties. Based on their estimated gastric cancer risk, individuals were classified as: low-risk group (no H pylori infection and no atrophy; n = 115; 21.4%); moderate-risk group(H pylori infection but no atrophy; n = 354, 66.0%); and high-risk group (gastric atrophy, with or without H pylori infection; n = 67, 12.5%). The high-risk group was significantly older (mean age: 61.9 ± 13.3 years), more frequently men and less educated as compared with the low-risk group. CONCLUSION: We propose to concentrate on an upper gastrointestinal endoscopy for detection of early gastric cancer in the high-risk group. 2The GWmodel R package:further topics for exploring spatial heterogeneity using geographically weighted models显示文摘In this study,we present a collection of local models,termed geographically weighted(GW)models,which can be found within the GWmodel R package.A GW model suits situations when spatial data are poorly described by the global form,and for some regions the localized fit provides a better description.The approach uses a moving window weighting technique,where a collection of local models are estimated at target locations.Commonly,model parameters or outputs are mapped so that the nature of spatial heterogeneity can be explored and assessed.In particular,we present case studies using:(i)GW summary statistics and a GW principal components analysis;(ii)advanced GW regression fits and diagnostics;(iii)associated Monte Carlo significance tests for non-stationarity;(iv)a GW discriminant analysis;and(v)enhanced kernel bandwidth selection procedures.General Election data-sets from the Republic of Ireland and US are used for demonstration.This study is designed to complement a companion GWmodel study,which focuses on basic and robust GW models.Binbin LU Paul HARRIS Martin CHARLTON Chris BRUNSDON 2014Geo-Spatial Information Science2014,17,2:9
3Final anatomic and visual outcomes appear independent of duration of silicone oil intraocular tamponade in complex retinal detachment surgery显示文摘AIM: To report anatomic and visual outcomes following silicone oil removal in a cohort of patients with complex retinal detachment, to determine association between duration of tamponade and outcomes and to compare patients with oil removed and those with oil in situ in terms of demographic, surgical and visual factors. METHODS: We reported a four years retrospective case series of 143 patients with complex retinal detachments who underwent intraocular silicone oil tamponade. Analysis between anatomic and visual outcomes, baseline demographics, duration of tamponade and number of surgical procedures were carried out using Fisher's exact test and unpaired two-tailed t-test. RESULTS: One hundred and six patients(76.2%) had undergone silicone oil removal at the time of review with 96 patients(90.6%) showing retinal reattachment following oil removal. Duration of tamponade was not associated with final reattachment rate or with a deterioration in best corrected visual acuity(BCVA). Patients with oil removed had a significantly better baseline and final BCVA compared to those under oil tamponade(P=0.0001, <0.0001 respectively). CONCLUSION: Anatomic and visual outcomes in this cohort are in keeping with those reported in the literature. 6Pulmonary complications of transcatheter arterial chemoembolization for hepatocellular carcinoma显示文摘Transcatheter arterial chemoembolization(TACE) is an effective palliative intervention that is widely accepted for the management of hepatocellular carcinoma(HCC). Post-TACE pulmonary complications resulting in acute lung injury(ALI) or acute respiratory distress syndrome(ARDS) are rare events. Pulmonary complications after TACE are thought to be related to chemical injury subsequent to the migration of the infused ethiodized oil or chemotherapeutic agent to the lung vasculature, facilitated by arteriovenous(AV) shunts within the hyper-vascular HCC. We review herein the literature on pulmonary complications related to TACE for HCC. Post-TACE pulmonary complications have included pulmonary oil embolism, interstitial pneumonitis, chemical pneumonitis, ALI, ARDS, lipoid pneumonia, acute eosinophilic and neutrophilic pneumonia, bilious pleuritis, pulmonary abscess, pulmonary tumor embolism, and possibly pulmonary metastasis with HCC. The risk factors associated with post-TACE pulmonary complications identified in the literature include large hyper-vascular HCC with AV shunts, large-volume Lipiodol infusion, and embolization via the right inferior phrenic artery. However, the absence of known risk factors is not a guarantee against serious complications. 7Extramural vascular invasion and response to neoadjuvant chemoradiotherapy in rectal cancer: influence of the CpG island methylator phenotype显示文摘AIM To identify whether Cp G island methylator phenotype(CIMP) is predictive of response to neoadjuvant chemoradiotherapy(NACRT) and outcomes in rectal cancer.METHODS Patients undergoing NACRT and surgical resection for rectal cancer in a tertiary referral centre between 2002-2011 were identified. Pre-treatment tumour biopsies were analysed for CIMP status(high, intermediate or low) using methylation specific PCR. KRAS and BRAF status were also determined using pyrosequencing analysis. Clinical information was extracted from case records and cancer services databases. Response to radiotherapy was measured by tumour regression scores determined uponhistological examination of the resected specimen. The relationship between these molecular features, response to NACRT and oncological outcomes were analysed.RESULTS There were 160 patients analysed with a median followup time of 46.4 mo. Twenty-one(13%) patients demonstrated high levels of CIMP methylation(CIMP-H) and this was significantly associated with increased risk of extramural vascular invasion(EMVI) compared with CIMP-L [8/21(38%) vs 15/99(15%), P = 0.028]. CIMP status was not related to tumour regression after radiotherapy or survival, however EMVI was significantly associated with adverse survival(P < 0.001). Intermediate CIMP status was significantly associated with KRAS mutation(P = 0.01). There were 14(9%) patients with a pathological complete response(pC R) compared to 116(73%) patients having no or minimal regression after neoadjuvant chemoradiotherapy. Those patients with pC R had median survival of 106 mo compared to 65.8 mo with minimal regression, although this was not statistically significant(P = 0.26). Binary logistic regression analysis of the relationship between EMVI and other prognostic features revealed, EMVI positivity was associated with poor overall survival, advanced 'T' stage and CIMP-H but not nodal status, age, sex, KRAS mutation status and presence of local or systemic recurrence.CONCLUSION We report a novel association of pre-treatment characterisation of CIMP-H with EMVI status which has prognostic implications and is not readily detectable on pre-treatment histological examination.Jeremy Stuart Williamson Huw Geraint Jones Namor Williams Anthony Paul Griffiths Gareth Jenkins John Beynon Dean Anthony Harris 2017World Journal of Gastrointestinal Oncology2017,9,5:4

11Management of Helicobacter pylori infection in Latin America: A Delphi technique-based consensus显示文摘AIM: To optimize diagnosis and treatment guidelines for this geographic region, a panel of gastroenterologists, epidemiologists, and basic scientists carried out a structured evaluation of available literature.METHODS: Relevant questions were distributed among the experts, who generated draft statements for consideration by the entire panel. A modified three-round Delphi technique method was used to reach consensus. Critical input was also obtained from representatives of the concerned medical community. The quality of the evidence and level of recommendation supporting each statement was graded according to United States Preventive Services Task Force criteria.RESULTS: A group of ten experts was established. The survey included 15 open-ended questions that were distributed among the experts, who assessed the articles associated with each question. The levels of agreement achieved by the panel were 50% in the first round, 73.3% in the second round and 100% in the third round. 16A fast and accurate model for the creation of explosion fragments with improved fragment shape and dimensions显示文摘Explosion models based on Finite Element Analysis(FEA)can be used to simulate how a warhead fragments.However their execution times are extensive.Active protection systems need to make very fast predictions,before a fast attacking weapon hits the target.Fast execution times are also needed in real time simulations where the impact of many different computer models is being assessed.Hence,FEA explosion models are not appropriate for these real-time systems.The research presented in this paper delivers a fast simulation model based on Mott’s equation that calculates the number and weight of fragments created by an explosion.In addition,the size and shape of fragments,unavailable in Mott’s equation,are calculated using photographic evidence and a distribution of a fragment’s length to its width.The model also identifies the origin of fragments on the warhead’s casing.The results are verified against experimental data and a fast execution time is achieved using uncomplicated simulation steps.The developed model then can be made available for real-time simulation and fast computation.David Felix Ian Colwill Paul Harris 2022Defence Technology(防务技术)2022,18,2:2
