1Liver transplantation for hepatitis B virus: Decreasing indication and changing trends显示文摘AIM: To evaluate the indication and outcome of hepatitis B virus(HBV)-related liver transplantation(LT) in the era of newer antiviral agents.METHODS: We collected data on all patients who underwent transplantation at King Faisal Specialist Hospital and Research Center.These data included demographic,perioperative and long-term postoperative follow-up data including viral serological markers,HBV DNA,and repeated liver imaging.Between January 1990 and January 2012,133 patients(106 males and 27 females) underwent LT for HBV-related cirrhosis at our center.All patients were followed up frequently during the first year following transplantation,according to our standard protocol; follow-up visits occurred every 3-6 mo thereafter.Breakthrough infection was definedas re-emergence of HBV-DNA or hepatitis B surface antigen(HBs Ag) while on treatment.Five patients transplanted prior to 1992 did not receive immediate posttransplant anti-HBV prophylaxis; all other patients were treated with HBIG and at least one nucleos(t)ide analog.RESULTS: One hundred and thirty-three patients underwent LT for HBV and were followed for a median of 82 mo(range: 1-274).The rates of post-LT survival and HBV recurrence during the follow-up period were 89% and 11%,respectively.The following factors were associated with disease recurrence: younger age(44.3 ± 16.2 years vs 51.4 ± 9.9 years,P = 0.02),positive pretransplant hepatitis B e antigen(HBe Ag)(60% vs 14%,P < 0.0001),detectable pretransplant HBV DNA(60% vs 27%,P = 0.03),positive posttransplant HBs Ag(80% vs 4%,P < 0.0001) and positive posttransplant HBe Ag(27% vs 1%,P < 0.0001).Forty-four(33%) patients had hepatocellular carcinoma(HCC).In the first(pre-2007) group,HBV was the second leading indication for LT after hepatitis C virus infection.A total of 64 transplants were performed,including 46(72%) for decompensated HBV cirrhosis,12(19%) for decompensated cirrhosis complicated by HCC and 6(10%) for compensated cirrhosis complicated by HCC.In the second group,nonalcoholic steatohepatitis surpassed HBV as the second leading indication for LT.A total of 69 HBV related transplants were performed,including 43(62%) for decompensated HBV cirrhosis,7(10%) for decompensated cirrhosis complicated by HCC and 19(27.5%) for compensated cirrhosis complicated by HCC.There was a significant(P = 0.007) increase in the number of transplants for compensated cirrhosis complicated by HCC.CONCLUSION: The use of potent anti-HBV agents has led to a changing trend in the indications for LT.HBV is currently the third leading indication for LT in this hyperendemic area.Waleed Al-hamoudi Hussien Elsiesy Abdulrahman Bendahmash Nasser Al-masri Safiyya Ali Naglaa Allam Mohammed Al Sofayan Hamad Al Bahili Mohammed Al Sebayel Dieter Broering Sammy Saab Faisal Abaalkhail 2015World Journal of Gastroenterology2015,21,26:4

3A study of in vitro antibacterial activity of lanthanides complexes with a tetradentate Schiff base ligand显示文摘Objective:To establish the antibacterial activity of lanthanides complexes with a tetradentate Schiff base ligand L.Methods:(N,N'-bis(l-naphthaldimine)-o-phenylenediamine)was prepared from the condensation of 2—hydroxy-1-naphthaldehyde with o-phenylenediamine in a molar ratio of 2:1.The antimicrobial activity of the resultant Ln(Ⅲ)complexes was investigated using agar well diffusion and micro-broth dilution techniques;the latter was used to establish the minimum inhibitory concentrations for each compound investigated.Results:Most of Ln(Ⅲ)complexes were found to exhibit antibacterial activities against a number of pathogenic bacteria with MICs ranging between 1.95-250.00μg/mL.Staphylococcus aureus was the most susceptible bacterial species to[LaL(NO_3)_2(H_2O)](NO_3)complex while Shigella dysenteriae and Escherichia coli required a relatively higher MIC(250μg/mL).Trie complexes La(Ⅲ)and Pr(Ⅲ)were effective inhibitors against Staphylococcus aureus,whereas Sm(Ⅲ)complex was effective against Serratia marcescens.On the other hand,Gd(Ⅲ),La(Ⅲ)and Nd(Ⅲ)were found to be more potent inhibitors against Pseudompnas aeruginosa than two of commonly used antibiotics.The remaining Ln(Ⅲ)complexes showed no remarkable activity as compared to the two standard drugs used.Conclusions:Tetradentate Schiff base ligand L and its complexes could be a potential antibacterial compounds after further investigation.Waleed Mahmoud Al Momani Ziyad Ahmed Taha Abdulaziz Mahmoud Ajlouni Qasem Mohammad Abu Shaqra Muaz Al Zouby 2013Asian Pacific Journal of Tropical Biomedicine2013,3,5:3

5Inter-ethnic marriages and severity of coronary artery disease:A multicenter study of Arabian Gulf States显示文摘AIM To assess the association of inter-ethnic vs intra-ethnic marriage with severity of coronary artery disease(CAD) in men undergoing angiography.METHODS We conducted a prospective multicenter,multi-ethnic,cross sectional observational study at five hospitals in Saudi Arabia and the United Arab Emirates,in which we used logistic regression analysis with and without adjustment for baseline differences. RESULTS Data were collected for 1068 enrolled patients undergoing coronary angiography for clinical indications during the period of April 1^(st),2013 to March 30^(th),2014. Ethnicities of spouses were available only for male patients. Of those enrolled,687 were married men and constituted the cohort for the present analysis. Intra-ethnic marriages were reported in 70% and inter-ethnic marriages in 30%. After adjusting for baseline differences,interethnic marriage was associated with lower odds of having significant CAD [adjusted odds ratio 0.52(95%CI:0.33,0.81)] or multi-vessel disease(MVD) [adjusted odds ratio 0.57(95%CI:0.37,0.86)]. The adjusted association with left main disease showed a similar trend,but was not statistically significant [adjusted odds ratio 0.74(95%CI:0.41,1.32)]. 7Synthesis, characterization, biological activities and luminescent properties of lanthanide complexes with [2-thiophenecarboxylic acid, 2-(2-pyridinylmethylene)hydrazide] Schiff bases ligand显示文摘A Schiff base L [2-thiophenecarboxylic acid, 2-(2-pyridinylmethylene)hydrazide] with its lanthanide metal complexes was synthesized. These complexes were characterized by elemental analysis, molar conductivity measurements, spectral analysis(NMR, FT-IR, and UV-Vis), luminescence and thermal gravimetric analysis. The Schiff base ligand was a tridentate chelate and coordinates to the central lanthanide ion with 1:2 metal:ligand ratio. The conductivity data showed a 1:1 electrolytic nature with a general formula [LnL_2(NO_3)_2]NO_3. The luminescence emission properties for Sm, Tb, and Eu complexes were observed and showed that the ligand L could absorb and transfer energy to Sm(III), Tb(III) and Eu(III) ions. The complexes possessed a good antibacterial activity against different bacterial strains. 10在生活施主肝移植的施主的 Day-of-surgery 拒绝显示文摘 AIM: To study diagnostic laparoscopy as a tool for excluding donors on the day of surgery in living donor liver transplantation (LDLT).METHODS: This study analyzed prospectively collected data from all potential donors for LDLT. All of the donors were subjected to a three-step donor evaluation protocol at our institution. Step one consisted of a clinical and social evaluation, including a liver profile, hepatitis markers, a renal profile, a complete blood count, and an abdominal ultrasound with Doppler. Step two involved tests to exclude liver diseases and to evaluate the donor’s serological status. This step also included a radiological evaluation of the biliary anatomy and liver vascular anatomy using magnetic resonance cholan-giopanc reatography and a computed tomography (CT) angiogram, respectively. A CT volumetric study was used to calculate the volume of the liver parenchyma. Step three included an ultrasound-guided liver biopsy. Between November 2002 and May 2009, sixty-nine potential living donors were assessed by open exploration prior to harvesting the planned part of the liver. Between the end of May 2009 and October 2010, 30 potential living donors were assessed laparoscopically to determine whether to proceed with the abdominal incision to harvest part of the liver for donation. RESULTS: Ninety-nine living donor liver transplants were attempted at our center between November 2002 and October 2010. Twelve of these procedures were aborted on the day of surgery (12.1%) due to donor findings, and eighty-seven were completed (87.9%). These 87 liver transplants were divided into the following groups: Group A, which included 65 transplants that were performed between November 2002 and May 2009, and Group B, which included 22 transplants that were performed between the end of May 2009 and October 2010. The demographic data for the two groups of donors were found to match; moreover, no significant difference was observed between the two groups of donors with respect to hospital stay, nar-cotic and non-narcotic analgesia requirements or the incidence of complications. Regarding the recipients, our study clearly revealed that there was no significant difference in either the incidence of different complications or the incidence of retransplantation between the two groups. Day-of-surgery donor assessment for LDLT procedures at our center has passed through two eras,open and laparoscopic. In the first era, sixty-nine LDLT procedures were attempted between November 2002 and May 2009. Upon open exploration of the donors on the day of surgery, sixty-five donors were found to have livers with a grossly normal appearance. Four donors out of 69 (5.7%) were rejected on the day of surgery because their livers were grossly fatty and pale. In the laparoscopic era, thirty LDLT procedures were attempted between the end of May 2009 and October 2010. After the laparoscopic assessment on the day of surgery, twenty-two transplantation procedures were completed (73.4%), and eight were aborted (26.6%). Our data showed that the levels of steatosis in the rejected donors were in the acceptable range. Moreover, the results of the liver biopsies of rejected donors were comparable between the group A and group B donors. The laparoscopic assessment of donors presents many advantages relative to the assessment of donors through open exploration; in particular, the laparo-scopic assessment causes less pain, requires a shorter hospital stay and leads to far superior cosmetic results. 18Multidrug resistance Acinetobacter species at the intensive care unit,Aseer Central Hospital,Saudi Arabia:A one year analysis显示文摘Objective:To identify and to determine the antimicrobial susceptibility of Acinetobacter baumannii(A.baumannii) clinical isolates from ICU at Aseer Central Hospital.Methods:The study was conducted in the Intensive Care Unit,Aseer Central Hospital,Saudi Arabia over 13 months period(2014-2015).Acinetobacter species(n= 105) were isolated from various clinical samples.Isolates were identified using selected phenotypic criteria and confirmed using the Vitek 2 automated system.This system was used to determine the susceptibilities of 21 antimicrobial agents.Patients,isolates and drug data were analyzed using the SPSS statistical software package to determine some epidemiological and microbiological patterns.Results:Of the 105 stains,A.baumannii accounted for 49(46.67%),A.baumannii complex,19(18.09%),A.baumannii/haemolyticus 32(30.47),Acinetobacter haemolyticus 4(3.81%),Acinetobater lwoffii 1(0.95%) and unidentified Acinetobater species 2(1.3%).Of the 105 Acinetobacter strains,103(98.1%) were found multidrug resistant(MDR).A.baumannii strain were 100% sensitive to colistin and 74.5% to trimethoprim + sulfamethoxazole.The remaining 19 antimicrobial agents revealed low or no sensitivities:amikacin 16.3%; ampicillin 7.7%; ceftazidime 7.3%.Distribution of similar sensitivities was shown by other Acinetobacter species.Mean number of isolates from males and females indicates no statistical variation(P=0.867) whereas age groups showed significant differences(P= 0.008) as it is clear from the high percentage of infected individuals more than 60 years followed by those aged 20 to 29 years old(19.05%).Upper respiratory tract(30.48%),lower respiratory tract(47.65%) and subcutaneous tissue(9.5%) were the main sources of Acinetobacter spp.but mean numbers of isolates from these specimens indicate no discrepancy between specimens(P=0.731).Conclusions:Acinetobacterspecies including A.baumannii were found MDR(98.1%) according to the current Acinetobacter spp.antimicrobial categorization.Approximately half of these strains were A.baumannii.All Acinetobacter species were 100% sensitive to colistin and to some extent to trimethoprim + sulfamethoxazole(74.5%).ICU-acquired pneumonia among patients over 60 years of age who spend prolong times at artificial ventilations made up the majority of the cases.Ali Al Bshabshe Martin R.P.Joseph Ali Al Hussein Waleed Haimour Mohamed E.Hamid 2016Asian Pacific Journal of Tropical Medicine2016,9,9:1

20In vitro inhibitory analysis of consensus siRNAs against NS3 gene of hepatitis C virus 1a genotype显示文摘Objective: To explore inhibitory effects of genome-specific, chemically synthesized siRNAs(small interference RNA) against NS3 gene of hepatitis C virus(HCV) 1a genotype in stable Huh-7(human hepatoma) cells as well as against viral replication in serum-inoculated Huh-7 cells. Methods: Stable Huh-7 cells persistently expressing NS3 gene were produced under antibiotic gentamycin(G418) selection. The cell clones resistant to 1 000 μg antibiotic concentration(G418) were picked as stable cell clones. The NS3 gene expression in stable cell clone was confirmed by RT-PCR and Western blotting. siRNA cell cytotoxicity was determined by MTT cell proliferation assay. Stable cell lines were transfected with sequence specific siRNAs and their inhibitory effects were determined by RT-PCR, real-time PCR and Western blotting. The viral replication inhibition by siRNAs in serum inoculated Huh-7 cells was determined by real-time PCR. Results: RT-PCR and Western blot analysis confirmed NS3 gene and protein expression in stable cell lines on day 10, 20 and 30 post transfection. MTT cell proliferation assay revealed that at most concentrated dose tested(50 nmol/L), siRNA had no cytotoxic effects on Huh-7 cells and cell proliferation remained unaffected. As demonstrated by the siRNA time-dependent inhibitory analysis, siRNA NS3-is44 showed maximum inhibition of NS3 gene in stable Huh-7 cell clones at 24(80%, P=0.013) and 48 h(75%, P=0.002) post transfection. The impact of siRNAs on virus replication in serum inoculated Huh-7 cells also demonstrated significant decrease in viral copy number, where siRNA NS3-is44 exhibited 70%(P<0.05) viral RNA reduction as compared to NS3-is33, which showed a 64%(P<0.05) decrease in viral copy number. siRNA synergism(NS3-is33 + NS3-is44) decreased viral load by 84%(P<0.05) as compared to individual inhibition by each siRNA(i.e., 64%–70%(P<0.05) in serum-inoculated cells. Synthetic siRNAs mixture(NS5Bis88 + NS3-is33) targeting different region of HCV genome(NS5B and NS3) also decreased HCV viral load by 85%(P< 0.05) as compared to siRNA inhibitory effects alone(70% and 64% respectively, P<0.05). Conclusions: siRNAs directed against NS3 gene significantly decreased m RNA and protein expression in stable cell clones. Viral replication was also vividly decreased in serum infected Huh-7 cells.
