| 2 | Small for size syndrome difficult dilemma: Lessons from 10 years single centre experience in living donor liver transplantation显示文摘AIM To analyze the incidence, risk factors, prevention, treatment and outcome of small for size syndrome(SFSS) after living donor liver transplantation(LDLT). METHODS Through-out more than 10 years: During the period from April 2003 to the end of 2013, 174 adult-to-adults LDLT(A-ALDLT) had been performed at National Liver Institute, Menoufiya University, Shibin Elkoom, Egypt. We collected the data of those patients to do this cohort study that is a single-institution retrospective analysis of a prospectively collected database analyzing the incidence, risk factors, prevention, treatment and outcome of SFSS in a period started from the end of 2013 to the end of 2015. The median period of follow-up reached 40.50 m, range(0-144 m). RESULTS SFSS was diagnosed in 20(11.5%) of our recipients. While extra-small graft [small for size graft(SFSG)], portal hypertension, steatosis and left lobe graft were significant predictors of SFSS in univariate analysis(P = 0.00, 0.04, 0.03, and 0.00 respectively); graft size was the only independent predictor of SFSS on multivariate analysis(P = 0.03). On the other hand, there was lower incidence of SFSS in patients with SFSG who underwent splenectomy [4/10(40%) SFSS vs 3/7(42.9%) no SFSS] but without statistical significance, However, there was none significant lower incidence of the syndrome in patients with right lobe(RL) graft when drainage of the right anterior and/or posterior liver sectors by middle hepatic vein, V5, V8, and/or right inferior vein was done [4/10(28.6%) SFSS vs 52/152(34.2%) no SFSS]. The 6-mo, 1-, 3-, 5-, 7-and 10-year survival in patients with SFSS were 30%, 30%, 25%, 25%, 25% and 25% respectively, while, the 6-mo, 1-, 3-, 5-, 7-and 10-year survival in patients without SFSS were 70.1%, 65.6%, 61.7%, 61%, 59.7%, and 59.7% respectively, with statistical significant difference(P = 0.00). CONCLUSION SFSG is the independent and main factor for occurrence of SFSS after A-ALDLT leading to poor outcome. However, the management of this catastrophe depends upon its prevention(i.e., selecting graft with proper size, splenectomy to decrease portal venous inflow, and improving hepatic vein outflow by reconstructing large draining veins of the graft). | Hany Shoreem Emad Hamdy Gad Hosam Soliman Osama Hegazy Sherif Saleh Hazem Zakaria Eslam Ayoub Yasmin Kamel Kalid Abouelella Tarek Ibrahim Ibrahim Marawan | 2017 | World Journal of Hepatology2017,9,21: | 3 |
| 3 | Preserving apple(Malus domestica var.Anna)fruit bioactive substances using olive wastes extract-chitosan film coating显示文摘Apple fruits have components of therapeutic nature.Such components,to a great extent, decline or decompose during post-harvest that negatively affect fruit shelf-life.Chitosan fruit-filming has proved useful in maintaining these compounds. This study aims, therefore,at enhancing Chitosan coating-film performance by mixing with some olive wastes extracts of leaf and pomace extracts.Apple fruits were sprayed with six different coating formulas including chitosan-water wax coating,in addition to the uncoated fruits. Then, the total phenolic, flavonoids, antioxidants, pigments, weight loss, decay area, and microstructure were assayed. The bioactive substances drastically changed in uncoated rather than coated fruits.Conversely, weight loss and decay area significantly increased in uncoated fruits.Amazingly, the addition of olive leaf extract to chitosan coating films meaningfully reduced the gradual decline in total phenolic, flavonoids and antioxidants.Olive pomace extract recorded the lowest influencing on anthocyanins during storage at 4±1C for 35 d.Also, both olive leaf and pomace extracts enhanced the coating distribution, due to no pores were observed in the fruits’surfaces.Decidedly, incorporation of olive leaf extracts with 2%into chitosan coating solution was the best formula comparable with the others. Thus, olive wastes extracts, incorporated into chitosan fruit coatings;relatively improve the nutritional quality of apple fruits during post-harvest. | Ibrahim Khalifa Hassan Barakat Hamdy A.El-Mansy Soliman A.Soliman | 2017 | Information Processing in Agriculture2017,4,1: | 2 |

| 20 | High prevalence of chronic viral hepatitis B and C in Minnesota Somalis contributes to rising hepatocellular carcinoma incidence显示文摘BACKGROUND Chronic hepatitis B virus(HBV)and hepatitis C virus(HCV)infections are known risk factors for liver disease, cirrhosis and hepatocellular carcinoma(HCC).There is substantial global variation in HBV and HCV prevalence resulting in variations in cirrhosis and HCC.We previously reported high prevalence of HBV and HCV infections in Somali immigrants seen at an academic medical center in Minnesota.AIM To determine the prevalence of chronic viral hepatitis in Somali immigrants in Minnesota through a community-based screening program.METHODS We conducted a prospective community-based participatory research study in the Somali community in Minnesota in partnership with community advisory boards, community clinics and local mosques between November 2010 and December 2015(data was analyzed in 2020).Serum was tested for hepatitis B surface antigen, hepatitis B core antibody, hepatitis B surface antibody and anti-HCV antibody.RESULTS Of 779 participants,15.4%tested positive for chronic HBV infection,50.2%for prior exposure to HBV and 7.6%for chronic HCV infection.Calculated age-adjusted frequencies in males and females for chronic HBV were 12.5%and 11.6%;for prior exposure to HBV were 44.8%and 41.3%;and for chronic HCV were 6.7%and 5.7%,respectively.Seven participants developed incident HCC during follow up.CONCLUSION Chronic HBV and HCV are major risk factors for liver disease and HCC among Somali immigrants, with prevalence of both infections substantially higher than in the general United States population.Community-based screening is essential for identifying and providing health education and linkage to care for diagnosed patients. | Essa A Mohamed Nasra H Giama Abubaker O Abdalla Hassan M Shaleh Abdul M Oseini Hamdi A Ali Fowsiyo Ahmed Wesam Taha Hager Ahmed Mohammed Jessica Cvinar Ibrahim A Waaeys Hawa Ali Loretta K Allotey Abdiwahab O Ali Safra A Mohamed William S Harmsen Eimad M Ahmmad Numra A Bajwa Mohamud D Afgarshe Abdirashid M Shire Joyce E Balls-Berry Lewis R Roberts | 2022 | World Journal of Gastroenterology2022,28,35: | 0 |
