1Cerebral ischemia and neuroregeneration显示文摘Cerebral ischemia is one of the leading causes of morbidity and mortality worldwide. Although stroke(a form of cerebral ischemia)-related costs are expected to reach 240.67 billion dollars by 2030, options for treatment against cerebral ischemia/stroke are limited. All therapies except anti-thrombolytics(i.e., tissue plasminogen activator) and hypothermia have failed to reduce neuronal injury, neurological deficits, and mortality rates following cerebral ischemia, which suggests that development of novel therapies again st stroke/cerebral ischemia are urgently needed. 3Acaricidal activity of Derris floribunda essential oil and its main constituent显示文摘Objective: To evaluate the acaricidal activity of the essential oil obtained from roots of Derris floribunda(D. floribunda)(Miq.) Benth, and its main constituent nerolidol against the Mexican mite Tetranychus mexicanus(T. mexicanus)(Mc Gregor).Methods: The essential oil from the roots of D. floribunda collected in the Amazon region(Brazil) was obtained by hydrodistillation. Its chemical composition was determined by GC–MS analysis. The acaricidal activities of this essential oil and nerolidol,were evaluated by recording the number of dead females(mortality) and eggs(fertility).Results: The essential oil showed sesquiterpenes as major volatile components. Nerolidol, the main component, represented 68.5% of the total composition of the essential oil. D. floribunda essential oil and nerolidol showed acaricidal activity, with LC_(50) of9.61 mg/m L air and 9.2 mg/m L air, respectively, over a 72 h period. In addition, both the essential oil and nerolidol significantly reduced the fecundity of T. mexicanus.Conclusions: Due to the economic importance of T. mexicanus and the lack of new pesticides, our data are very promising in the search for efficient and safer acaricidal products. 4Proton nuclear magnetic resonance-based metabonomic models for non-invasive diagnosis of liver fibrosis in chronic hepatitis C:Optimizing the classification of intermediate fibrosis显示文摘AIM To develop metabonomic models(MMs), using 1 H nuclear magnetic resonance(NMR) spectra of serum, to predict significant liver fibrosis(SF: Metavir ≥ F2), advanced liver fibrosis(AF: METAVIR ≥ F3) and cirrhosis(C: METAVIR = F4 or clinical cirrhosis) in chronic hepatitis C(CHC) patients. Additionally, to compare the accuracy of the MMs with the aspartate aminotransferase to platelet ratio index(APRI) and fibrosis index based on four factors(FIB-4). METHODS Sixty-nine patients who had undergone biopsy in the previous 12 mo or had clinical cirrhosis were included. The presence of any other liver disease was a criterion for exclusion. The MMs, constructed using partial least squares discriminant analysis and linear discriminant analysis formalisms, were tested by cross-validation, considering SF, AF and C. RESULTS Results showed that forty-two patients(61%) presented SF, 28(40%) AF and 18(26%) C. The MMs showed sensitivity and specificity of 97.6% and 92.6% to predict SF; 96.4% and 95.1% to predict AF; and 100% and 98.0% to predict C. Besides that, the MMs correctly classified all 27(39.7%) and 25(38.8%) patients with intermediate values of APRI and FIB-4, respectively. 5Antifungal activity and antidiarrheal activity via antimotility mechanisms of (-)-fenchone in experimental models显示文摘BACKGROUND(-)-Fenchone is a bicyclic monoterpene present in essential oils of plant species,such as Foeniculum vulgare and Peumus boldus,used to treatment of gastrointestinal diseases.Pharmacological studies report its anti-inflammatory,antioxidant,and antinociceptive activity.AIM To investigate antidiarrheal activity related to gastrointestinal motility,intestinal secretion and antimicrobial activity.METHODS A castor oil-induced diarrhea model was used to evaluate antidiarrheal activity.Intestinal transit and gastric emptying protocols were used to assess a possible antimotility effect.Muscarinic receptors,presynaptic α2-adrenergic and tissue adrenergic receptors,KATP channels,nitric oxide were investigated to uncover antimotility mechanisms of action and castor oil-induced enteropooling to elucidate antisecretory mechanisms.The antimicrobial activity was evaluated in the minimum inhibitory concentration model,the fractional inhibitory concentration index using the(-)-fenchone association method with standard antifungal agents.RESULTS(-)-Fenchone(75,150 and 300 mg/kg)showed antidiarrheal activity,with a significant decrease in the evacuation index.This activity is possibly related to a percentage of reduced intestinal transit(75,150 and 300 mg/kg).The antimotility effect of(-)-fenchone decreased in the presence of pilocarpine,yohimbine,propranolol,L-NG-nitroarginine methyl ester or glibenclamide.In the enteropooling model,no reduction in intestinal fluid weight was observed.(-)-Fenchone did not show antibacterial activity;on the other hand,inhibits the growth of strains of fungi with a minimum fungicide concentration of 32μg/mL.However,when it was associated with amphotericin B,no synergism was observed.CONCLUSION The antidiarrheal effect of(-)-fenchone in this study involves antimotility effect and not involve antisecretory mechanisms.(-)-Fenchone presents antifungal activity;however,it did not show antibacterial activity.Michelle Liz de Souza Pessoa Leiliane Macena Oliveira Silva Maria Elaine Cristina Araruna Catarina Alves de Lima Serafim Edvaldo Balbino Alves Junior Alessa Oliveira Silva Matheus Marley Bezerra Pessoa Hermes Diniz Neto Edeltrudes de Oliveira Lima Leonia Maria Batista 2020World Journal of Gastroenterology2020,26,43:2

8Machine learning applied to road safety modeling:A systematic literature review显示文摘Road safety modeling is a valuable strategy for promoting safe mobility,enabling the development of crash prediction models(CPM)and the investigation of factors contributing to crash occurrence.This modeling has traditionally used statistical techniques despite acknowledging the limitations of this kind of approach(specific assumptions and prior definition of the link functions),which provides an opportunity to explore alternatives such as the use of machine learning(ML)techniques.This study reviews papers that used ML techniques for the development of CPM.A systematic literature review protocol was conducted,that resulted in the analysis of papers and their systematization.Three types of models were identified:crash frequency,crash classification by severity,and crash frequency and severity.The first is a regression problem,the second,a classificatory one and the third can be approached either as a combination of the preceding two or as a regression model for the expected number of crashes by severity levels.The main groups of techniques used for these purposes are nearest neighbor classification,decision trees,evolutionary algorithms,support-vector machine,and artificial neural networks.The last one is used in many kinds of approaches given the ability to deal with both regression and classification problems,and also multivariate response models.This paper also presents the main performance metrics used to evaluate the models and compares the results,showing the clear superiority of the ML-based models over the statistical ones.In addition,it identifies the main explanatory variables used in the models,which shows the predominance of road-environmental aspects as the most important factors contributing to crash occurrence.The review fulfilled its objective,identifying the various approaches and the main research characteristics,limitations,and opportunities,and also highlighting the potential of the usage of ML in crash analyses.Philippe Barbosa Silva Michelle Andrade Sara Ferreira 2020Journal of Traffic and Transportation Engineering(English Edition)2020,7,6:2
9Neutrophile-to-lymphocyte,lymphocyte-to-monocyte,and platelet-tolymphocyte ratios as prognostic and response biomarkers for resectable locally advanced gastric cancer显示文摘BACKGROUND Perioperative fluorouracil plus leucovorin,oxaliplatin,and docetaxel(FLOT)improves prognosis in locally advanced gastric cancer(LAGC).Neutrophil-to-lymphocyte(NLR),lymphocyte-tomonocyte(LMR),and platelet-to-lymphocyte(PLR)ratios are prognostic biomarkers but not predictive factors.AIM To assess blood ratios’(NLR,LMR and PLR)potential predictive response to FLOT and survival outcomes in resectable LAGC patients.METHODS This was a multicentric retrospective study investigating the clinical potential of NLR,LMR,and PLR in resectable LAGC patients,treated with at least one preoperative FLOT cycle,from 12 Portuguese hospitals.Means were compared through non-parametric Mann-Whitney tests.Receiver operating characteristic curve analysis defined the cut-off values as:High PLR>141 for progression and>144 for mortality;high LMR>3.56 for T stage regression(TSR).Poisson and Cox regression models the calculated relative risks/hazard ratios,using NLR,pathologic complete response,TSR,and tumor regression grade(TRG)as independent variables,and overall survival(OS)as the dependent variable.RESULTS This study included 295 patients(mean age,63.7 years;59.7% males).NLR was correlated with survival time(r=0.143,P=0.014).PLR was associated with systemic progression during FLOT(P=0.022)and mortality(P=0.013),with high PLR patients having a 2.2-times higher risk of progression[95% confidence interval(CI):0.89-5.26]and 1.5-times higher risk of mortality(95%CI:0.92-2.55).LMR was associated with TSR,and high LMR patients had a 1.4-times higher risk of achieving TSR(95%CI:1.01-1.99).OS benefit was found with TSR(P=0.015)and partial/complete TRG(P<0.001).Patients without TSR and with no evidence of pathological response had 2.1-times(95%CI:1.14-3.96)and 2.8-times(95%CI:1.6-5)higher risk of death.CONCLUSION Higher NLR is correlated with longer survival time.High LMR patients have a higher risk of decreasing T stage,whereas high PLR patients have higher odds of progressing under FLOT and dying.Patients with TSR and a pathological response have better OS and lower risk of dying.Tiago Cruz Tomás Ines Eiriz Marina Vitorino Rodrigo Vicente Joao Gramaca Alicia Guadalupe Oliveira Paulo Luz Mafalda Baleiras Ana Sofia Spencer Luísa Leal Costa Patrícia Liu Joana Mendonca Magno Dinis Teresa Padrao Marisol Correia Goncalo Atalaia Michelle Silva Teresa Fiúza 2022World Journal of Gastrointestinal Oncology2022,14,7:2

13Effect of transcutaneous electroacupuncture at Neiguan(PC 6) on refractory vomiting in patients in intensive care unit显示文摘OBJECTIVE: To observe the effect of transcutaneous electroacupuncture(TEA) at Neiguan(PC 6) on refractory vomiting in critically ill patients in intensive care(ICU) setting.METHODS: Ten patients admitted in ICU and presenting vomiting refractory to one or more antiemetic drugs were prospectively included in the study. TEA was applied at acupoint of Neiguan(PC6) during 30 min with a neuromuscular transmission monitor(single-twitch stimulation with 1 Hz at a constant current of 10 m A). Nausea and Vomiting were evaluated at the following intervals: immediately after 30 min of TEA at Neiguan(PC 6),30 min-6h and 6-24 h. The presence of nausea and/or vomiting throughout the observational period was defined as the primary end point.RESULTS: The presence of nausea or vomiting throughout the observational period was 10% at the end of TEA, 40% between 30 min and 6 h, and50% between 30 min and 24 h(P < 0.001, P = 0.01 and P = 0.03 vs pre-TEA, respectively).
