| 1 | From bench to bedside: Fecal calprotectin in inflammatory bowel diseases clinical setting显示文摘Fecal calprotectin(FC) has emerged as one of the most useful tools for clinical management of inflammatory bowel diseases(IBD). Many different methods of assessment have been developed and different cutoffs have been suggested for different clinical settings. We carried out a comprehensive literature review of the most relevant FC-related topics: the role of FC in discriminating between IBD and irritable bowel syndrome(IBS) and its use in managing IBD patients In patients with intestinal symptoms, due to the high negative predictive value a normal FC level reliably rules out active IBD. In IBD patients a correlation with both mucosal healing and histology was found, and there is increasing evidence that FC assessment can be helpful in monitoring disease activity and response to therapy as well as in predicting relapse, post-operative recurrence or pouchitis. Recently, its use in the context of a treat-to-target approach led to a better outcome than clinically-based therapy adjustment in patients with early Crohn's disease. In conclusion, FC measurement represents a cheap, safe and reliable test, easy to perform and with a good reproducibility. | 2 | The Virtual Geoscience Workbench, VGW: Open Source tools for discontinuous systems显示文摘In 1989 work began on merging finite and discrete elements. During 1990 algorithmic solutions for the new combined method were developed, together with a C-based implementation known as the Y code. In 2004 these developments and implementations were published as a textbook. Queen Mary University of London (QMUL) and Imperial College are currently engaged in a research project to make Y codes in both 2D and 3D available in Open Source format and with mainly geoscience problems in mind. | 4 | Molecular mechanisms of liver preconditioning显示文摘Ischemia/reperfusion (I/R) injury still represents an important cause of morbidity following hepatic surgery and limits the use of marginal livers in hepatic transplantation. Transient blood flow interruption followed by reperfusion protects tissues against damage induced by subsequent I/R. This process known as ischemic pre-conditioning (IP) depends upon intrinsic cytoprotective systems whose activation can inhibit the progression of irreversible tissue damage. Compared to other organs,liver IP has additional features as it reduces inflammation and promotes hepatic regeneration. Our present understanding of the molecular mechanisms involved in liver IP is still largely incomplete. Experimental studies have shown that the protective effects of liver IP are triggered by the release of adenosine and nitric oxide and the subsequent activation of signal networks involving protein kinases such as phosphatidylinositol 3-kinase,protein kinase C δ/ε and p38 MAP kinase,and transcription factors such as signal transducer and activator of transcription 3,nuclear factor-κB and hypoxia-inducible factor 1. | 5 | Space decomposition based parallelization solutions for the combined finiteediscrete element method in 2D显示文摘The combined finiteediscrete element method(FDEM) belongs to a family of methods of computational mechanics of discontinua. The method is suitable for problems of discontinua, where particles are deformable and can fracture or fragment. The applications of FDEM have spread over a number of disciplines including rock mechanics, where problems like mining, mineral processing or rock blasting can be solved by employing FDEM. In this work, a novel approach for the parallelization of two-dimensional(2D) FDEM aiming at clusters and desktop computers is developed. Dynamic domain decomposition based parallelization solvers covering all aspects of FDEM have been developed. These have been implemented into the open source Y2 D software package and have been tested on a PC cluster. The overall performance and scalability of the parallel code have been studied using numerical examples.
